San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.
• Experienced with Behavioral Health and Recovery Services (BHRS), as well as other mental health community programs.
• Knowledgeable of the systems of care related to older adults, mental health, substance abuse, developmental delay, pediatrics, primary care, palliative care, chronic illnesses, dialysis, conservatorship, and probation and parole.
• Able to work independently with little supervision and demonstrates a high degree of initiative.
• Knowledgeable of housing resources, their requirements for eligibility, and other community services such as In-Home Supportive Services (IHSS), Multipurpose Senior Services Program (MSSP), Institute on Aging (IOA), Health Plan of San Mateo (HPSM), BHRS, and Bridges to Wellness, and more.
• Knowledgeable about County benefit and entitlement programs, such as Medi-Cal and ACE.
• Experienced and comfortable working with the following populations: multi-cultural, homeless, chronic diseases, end-of-life, mental health, domestic violence, parolees, and substance abuse disorders.
• Experienced working with a diverse population (race, ethnicity, age, ability, language, nationality, socioeconomic status, gender, religion, and sexual orientation).
• Able to accurately assess for signs of adult and child abuse and be a mandated reporter.
• Able to work in a politically charged environment under high degrees of stress and meet all deadlines.
• Able to develop effective and positive working relationships with community partners such as Aging and Adult Services, BHRS, and HPSM.
• An effective communicator to a broad range of stakeholders, including doctors, nurses, conservators, health advocates, families, and patients.
• Able to remain calm during crisis situations and perform emergency crisis counseling for families and patients as needed.
• Experienced documenting in an EMR.
• Able to communicate clearly and concisely.
• Spanish-speaking preferred.

Examples Of Duties

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.
• Maintain case files utilizing the electronic medical record.
• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.
• Document interactions with patients/clients in EMR.
• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

The San Francisco Municipal Transportation Agency (SFMTA) has determined that Class 9163 Transit Operator is a “safety-sensitive” position, under regulations issued by the Federal Transit Administration (49 CFR Part 655). Federal law requires that all transit employees who perform safety-sensitive functions be subject to random, post-accident, reasonable suspicion, return-to-duty and follow-up drug and alcohol testing. Attendance to all training sessions is required. If a candidate misses any dates of training without providing proper notification, they will be released from their training class. If proper notification was provided, a candidate released during the training period due to attendance may request to be placed back on the eligible list to be considered for future classes, if the list from which they were hired is still active. JOB SUMMARY: The San Mateo County Transit District is seeking a highly technical IT Operations Supervisor to join our team. Under the supervision of the IT Operations Manager, the Supervisor will coordinate the efficient and effective operations of the Help Desk team for the District, including work direction and supervision of Help Desk staff and coordination with other District information technology staff. This position is responsible for planning, assigning, and directing work; documenting and gaining approval for all help desk support procedures and system documentation; and assisting team members in answering calls, addressing service or escalation issues, and applying independent decision making to resolve problems or escalate problems to management. The IT Operations Supervisor develops regular reports on help desk contacts, ensures that service levels are met, identifies trends and makes recommendations for service improvements, and provides budgetary recommendations to management for service growth and/or improvement.

IT Operations Supervisor will work closely with various users in San Mateo County Transit District (SamTrans), the Peninsula Corridor Joint Powers Board (Caltrain), and the San Mateo County Transportation Authority (TA) and other departments and is responsible for managing all procedures related to the identification, prioritization, and resolution of end user help requests, including the monitoring, tracking, and coordination of Help Desk functions. • Supervise help desk support staff’s daily activities ensuring timely and professional delivery of technical support for calls on computers, core software products, and related hardware peripherals.
• Plan, organize, and direct the work of help desk support staff, including Help Desk Specialists; schedule shift coverage; evaluate and document performance and recommend disciplinary action when necessary; and review staff work for accuracy and timely completion of assigned duties.

Position Summary

 

Under general supervision, operates a variety of transit vehicles to transport passengers along specific, assigned routes according to the District's requirements.  Provide service in a safe, courteous and reliable manner.   Responsible for written reports and other information as required.  Upon completion of the Golden Gate Transit Training Program, the employee operates regularly scheduled assignments.  All runs will originate in either Santa Rosa, Novato or San Rafael.

Hourly Pay: $30.23/hour to $35.57/hour


 If selected for the position, new hires will need to complete Golden Gate Bridge, Highway & Transportation District’s Bus Operator Apprenticeship Program ($30.23/hr benefits not included) which is approximately twelve (12) weeks, Monday through Friday, 6:30 a.m. to 3:30 p.m.

Upon successful completion of training: $32.01 per hour, plus benefits. All new hires will be required to complete a 120 probationary period upon completion of training.

Essential Responsibilities

 

• Transports passengers along assigned routes, making designated stops as required or requested.
• Uses the fare box to accurately collect fares, issues transfers and gives general information.
• Completes reports on patron count, non-scheduled mileage, vehicular deficiencies, fare box, accidents and other reports as required.
• Operates automatic lift on buses and assists passengers in wheelchairs, those using crutches or walkers, or who have speech or mental difficulty.
• Ensures that Driver’s license and all other documents required of a commercial licensee are maintained and are current and valid.
• Knows and follows the safety and health rules and safe working practices applicable to his or her job.
• Establishes and maintains effective working relationships with District employees, customers, vendors and all others contacted during the course of work using principles of excellent customer service.

Minimum Qualifications

 

• No experience is necessary. 
• Must be a licensed driver for at least seven (7) years prior to the date of this posting. Candidates who are currently enrolled in the Bus Operator Pre-Apprenticeship Program and a licensed driver for at least 3 years are eligible to apply, pending successful completion of the pre-apprenticeship program.

Role description

Under direction, the 2595 Senior EAP Counselor provides direction and supervision to counselors providing organizational wellness services and engaged in direct client counseling; provides clinical assessments, short-term counseling, and referral of City and County employees experiencing difficulties related to personal or work-related problems; conducts seminars, trainings, and consultations on issues related to personal and organizational wellness; prepares records and reports; and performs related duties as required.

Typical duties include, but are not limited to:

1. Participates in the selection of, trains and evaluates a team of journey level counselors.
2. Provides clinical assessment, short-term counseling, and referral of City and County employees experiencing difficulties related to personal or work-related problems, such as stress, depression, anxiety, substance abuse, emotional concerns or relationship issues; provides counseling to family members and significant others as appropriate.
3. Educates employees, managers, supervisors, and union personnel regarding employee assistance program service offerings, how to access services, early identification of personal and workplace problems, and referrals to the program; directs the development of programs, resources, and EAP initiatives.
