THE POSITION

Are you interested in assisting individuals to get connected to the resources and support they need to work on their mental health?  Tri-City is currently looking for an enthusiastic and motivated individual to be a part of the Access to Care (ATC) team. The Clinical Therapist I/II position for the ATC team will appeal to individuals who are interested in assisting participants and clients with enrolling into mental health services and getting the support they need.  We are looking for individuals inspired by challenges and opportunities presented in working with an engaged and active community.  The Clinical Therapist I/II will report to a Clinical Supervisor or Clinical Program Manager.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Conduct same day biopsychosocial assessments with both adults and/or children and carry no caseload;

• Will formulate a differential diagnosis concerning the nature of the mental health challenge;

• Create treatment plans and complete all necessary Electronic Health Record documentation;

• Perform screenings, linkage to resources and provide crisis interventions and crisis assessments;

• Work on a multidisciplinary team to promote recovery and ensure Network Adequacy;

• Ability to work in a fast-paced environment and must have excellent attendance;

• Collaborate with outside agencies to coordinate care for individuals seeking services.

THE POSITION

Tri-City is looking for a special individual to serve as a Clinical Therapist I/II for the Children’s Full Service Partnership/Children and Transitional Age Youth (FSP/ C TAY). The Clinical Therapist I/II provides behavioral health clinical intervention and recovery services to assigned clients and their families; develops treatment plans and performs individual, group, and family psychotherapeutic counseling, routine and emergency assessment, crisis intervention, and related treatment services; manages an assigned caseload of adults, juveniles, and/or children; provides consultation to other staff, programs, and agencies related to treatment needs and plans; and performs related duties as assigned.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Provides individual, group, and family interventions, rehabilitation, and treatment services including counseling, therapy, case management, and referral and linkage to community services to a variety of clients including high acuity clients.

• Within scope of authority, assesses, diagnoses, and treats a variety of clients with mental health disorders; collaborates with clients and creates individualized treatments plans according to client’s needs and circumstances.

• Initiates and conducts individual, group, and family counseling and biopsychosocial therapy sessions utilizing a variety of techniques and modalities; provides case management services in the office and in the field.

• Performs routine and emergency intake evaluations according to Authority procedure; completes appropriate forms and documents relating to the intake process; and prepares medical and demographic case history for use in diagnosis.

• Assists clients and family members in understanding behavioral health diagnosis, contributing factors/causes, effects, prognosis, rehabilitation, and treatment.

• Works with clients to address stressors and barriers to treatment and/or social functioning; identifies and focuses on client strengths and personal resources to effectively engage and treat client.

THE POSITION:

The Facilities Carpenter I is an entry level classification in the carpentry series. Carpenters in this class are assigned routine skilled carpentry tasks. Under the training concept of growing excellence in craft, positions allocated to the Facilities Carpenter II level which become vacant may reasonably be filled at the I level. 
 

The Facilities Carpenter II is responsible for performing a variety of skilled carpentry and related tasks in the construction, remodeling, repair, and maintenance of City facilities. This the journey level classification within the carpenter series. Incumbents at this level act with independence, handle "master" craftworker projects, and assume the more complex activities which would otherwise require the attention and coordination of the Facilities and Building Manager. 

 

 

Essential Duties

What you will do:

1. Remodels offices in City facilities. Takes down walls, doors and windows, reinstalls new walls and cuts in new doorways.

2. Installs, repairs and replaces doors, door closures, windows, floors, roofs, and other structural appurtenances.

3. Builds and repairs wooden fixtures and office furniture of all kinds.

4. Fabricates new cabinets, bookshelves and countertops.

5. Performs maintenance and repair work on all city buildings, rental units, and on private residences involving Police damage to the premises.

6. Rescreens screen doors and window screens.

7. Fabricates tack boards and bulletin boards.

8. May perform allied craft work in the course of maintaining City facilities.

9. Performs related work as necessary.

THE POSITION: 

The City of Hayward is excited to announce its recruitment for the position of Mobile Mental Health Clinician. Under general direction, the Mobile Mental Health Clinician provides field and office based professional behavioral health counseling, psychoeducation, outreach and engagement, and case management services as part of the City’s Hayward Evaluation and Response Teams (HEART) Program improving public safety responses to people experiencing mental illness, substance use disorders and homelessness.

THE PROGRAM

The Hayward Evaluation and Response Teams (HEART) Program is a cross departmental program between the Hayward City Manager’s Office, Police, and Fire Departments and Alameda County Behavioral Health Care Services developed to improve access to medical, mental health, and other support services.  The HEART program aims to achieve two main goals: 1) to reduce the need for police officers to intervene in calls for service involving people experiencing chronic mental illness, substance use disorders, and homelessness and 2) to reduce reliance on emergency services for individuals needing long-term case management and follow-up services.  HEART consists of three distinct response teams: the Mobile Integrated Health Unit (MIHU) made up of Hayward Fire Department Community Paramedics; the Hayward Mobile Evaluation Team (HMET) which pairs a specially trained Hayward Police Officer with an Alameda County behavioral health clinician; and the Case Management and Mental Health Linkages Team (LINK) consisting of Mobile Mental Health Clinicians and a Lead Program Assistant (Case Manager).

 As a member of the LINK team, the ideal candidate has experience providing short term crisis intervention, safety assessments, and other mental health services to adults, youth and children experiencing mental illness, substance use disorders and/or homelessness.  They have the ability to work primarily in the field and have the situational awareness to respond alongside emergency response teams. They are knowledgeable about local community resources and skilled at forming relationships quickly.

Equivalent to graduation from a four-year college or university with major course work in civil or traffic engineering and five years of increasingly responsible traffic engineering experience, three years of which were at a level equivalent to the Assistant Transportation Engineer class. Progressively responsible related experience may be substituted for the college course work on a year for year basis.

Other requirements include possession of a valid California Registration as a professional Traffic Engineer or a professional Civil Engineer. A CA Registration as a Traffic Engineer is preferred. Candidates must also possess a valid California driver's license and have a satisfactory driving record. Desirable qualifications include a master's degree in Transportation Engineering/Planning or a related field.

The Human Services Department (HSD) delivers and supports services by forging long-term community partnerships, engaging with and building the capacity of the community to do its own problem-solving, and leveraging financial and volunteer resources. The Department’s nationally and internationally recognized programs support thousands of residents throughout their life course, from infants to elders. The department offers a working environment which is friendly, supportive, collegial, and where staff of many disciplines come together to problem solve, compassionately assist those in need, and share successes.

The Aging and Family Services Division (AFS) of HSD specifically supports older residents and their caregivers to live active, healthy lives in the Tri-City community. With input from local, older adults (age 55 years+) as our guide, the AFS team strives to provide and AFS staff work together to provide a continuum of supportive services that range from health promotion activities to home-based end of life care. AFS has a Senior InfoLine (phone and email) that is answered in 4 language (Farsi, English, Spanish and Chinese) that is often an entry point. As an older adult or their caregiver’s situation changes, they seamlessly have access to a continuum of programs that include counseling and emotional support; care coordination and resource assistance; specialized family caregiver support and education. Many of these supportive services are provided in the resident’s home. The Position

Under the supervision of the Age Well Center Managers, the Chef/Food Services Manager position manages meal programs for the City of Fremont’s Age Well Centers. Chef responsibilities across both Age Well Center locations include the performance of menu planning, purchasing of foodstuffs, preparing quantity meals, supervising and assisting kitchen employees and volunteers and maintaining clean and orderly kitchens.

The incumbent will be responsible for properly cooked, economical, and wholesome, well balanced meals in City operated Age Well Centers. This position is not limited to being responsible for the assignment of work, supervision and evaluation of paid and volunteer food service workers. The review and approval of proposed menus, food purchases, control and monitoring of expenditure rates; and institution of disciplinary actions are functions reserved for the Program Coordinator or Age Well Center Managers.

Examples of Duties

• Plan menus with consideration for the dietary needs of older adults, wholesomeness, quantity of food to be prepared and budgetary constraints.
• Estimate food requirements and requisitions for food and supplies.
• Prepare, cook, and serve a wide variety of foodstuffs for quantity food service.
• May cut/carve and cook meats; bake breads, cakes and pastries; peel and prepare vegetables; and make salads.
• Place food in serving containers and serves food.
• Maintain proper temperature, rotation and storage of food.
• Direct and assist in cleaning kitchens and kitchen equipment after each meal.
• Estimate food requirements and requisitions for food and supplies.
• Direct and assist in the cleaning of the kitchen and kitchen equipment after each meal.
• Oversee that the kitchen and equipment are kept clean, sanitary and in proper working condition. Schedule regular maintenance of kitchen equipment as needed. Maintain standards of safety and sanitation in kitchen area.
• Recruit, train, supervise, instruct, and direct employees and volunteers assigned to working in the kitchens.
• Develop and support additional meal programs, including, but not limited to fundraisers, catering events and cooking demonstrations and special events.
• Keep records and prepare reports as required. Inclusive of any certification requirements and/or medical clearances required of subordinate employees and volunteers.
• Oversee community garden program, including crop rotation, seasonal planting, and maintenance assignment.
• Coordinate with office staff, marketing and sales strategies to increase visibility and participation for meal programs.
• Ensure kitchen compliance with health regulations and participate in health inspections.
• Other duties as assigned.
