San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.
• Experienced with Behavioral Health and Recovery Services (BHRS), as well as other mental health community programs.
• Knowledgeable of the systems of care related to older adults, mental health, substance abuse, developmental delay, pediatrics, primary care, palliative care, chronic illnesses, dialysis, conservatorship, and probation and parole.
• Able to work independently with little supervision and demonstrates a high degree of initiative.
• Knowledgeable of housing resources, their requirements for eligibility, and other community services such as In-Home Supportive Services (IHSS), Multipurpose Senior Services Program (MSSP), Institute on Aging (IOA), Health Plan of San Mateo (HPSM), BHRS, and Bridges to Wellness, and more.
• Knowledgeable about County benefit and entitlement programs, such as Medi-Cal and ACE.
• Experienced and comfortable working with the following populations: multi-cultural, homeless, chronic diseases, end-of-life, mental health, domestic violence, parolees, and substance abuse disorders.
• Experienced working with a diverse population (race, ethnicity, age, ability, language, nationality, socioeconomic status, gender, religion, and sexual orientation).
• Able to accurately assess for signs of adult and child abuse and be a mandated reporter.
• Able to work in a politically charged environment under high degrees of stress and meet all deadlines.
• Able to develop effective and positive working relationships with community partners such as Aging and Adult Services, BHRS, and HPSM.
• An effective communicator to a broad range of stakeholders, including doctors, nurses, conservators, health advocates, families, and patients.
• Able to remain calm during crisis situations and perform emergency crisis counseling for families and patients as needed.
• Experienced documenting in an EMR.
• Able to communicate clearly and concisely.
• Spanish-speaking preferred.

Examples Of Duties

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.
• Maintain case files utilizing the electronic medical record.
• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.
• Document interactions with patients/clients in EMR.
• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

ESD is seeking a dynamic, creative, and collaborative leader for the position of Microsoft Server Support Engineer (Network Engineer). This position will act as lead to three Network Technicians who collectively provide end-user support. The ideal candidate must have the ability to effectively resolve difficult customer situations, function independently within the scope of their responsibilities, manage complex technology projects, and communicate effectively with all levels of the organization. Possible projects include developing requirements for technology procurements, planning local area network upgrades, coordinating the rollout of mobile devices using a management system, and managing deployment and maintenance of department-level applications or systems.

Typical duties for ESD's Microsoft Server Support Engineer (Network Engineer) may include but are not limited to the following:

Manage Active Directory (AD)

     * Perform standard AD user, group, and group policy update tasks.

Job Description

Under direct supervision, the Development Strategy & Finance Sr. Analyst works within the Port’s Real Estate and Development Division leading analytical work related to the Port’s Public-Private Partnership Development Projects. Key tasks will include preparing complex financial analyses and reports, analyzing development pro formas and appraisals, coordinating the issuance and disbursement of bonds and associated record keeping documentation, supporting with development negotiations, and managing division-wide contracts, billings, and budgets.

The Sr. Analyst will support Project Managers with all aspects of Port Development projects. The role will receive support in learning and performing all tasks with the goal of independently managing specific areas over time. The Development team and this position are collaborative in nature, and the Sr. Analyst will work with both internal and external stakeholders including all Port Divisions, specifically Port Finance and the Port’s Resilience team, the Office of Economic and Workforce Development, the Office of Public Finance, and private development partners.

The essential functions of this position include:

• Analyzes Port-wide development opportunities to prioritize projects and determine value to support Port-wide Sea Level Rise resilience projects.

San José Mineta International Airport is hiring for an Analyst I/II in its Planning and Development Division. This position reports directly to the Senior Analyst and is a crucial member of the Airport's Planning and Development Division. The Analyst is part of the fiscal team in the Planning and Development Division of the Airport. The position requires the ability to type 55 words per minute with proficiency using MS Word, or equivalent, with the ability to follow strict formatting requirements for the preparation of meeting agenda, synopsis, and/or minutes for each Council, Committee, Board or Commission meeting. Agenda : The Legislative Secretary is responsible for preparing a variety of documents (Agendas for City Council, Council Committee, Boards or Commissions) utilizing wording and layout based upon standard formats to ensure consistency and compliance with the City's legislative procedures; posting documents in accordance with State Brown Act and City Open Government requirements. The Legislative Secretary may be responsible for the assembly and distribution of those documents to elected officials, City Administration, media and the public under tight deadlines.

Minutes : Responsibilities include attending City Council, Council Committee, Board or Commission meetings and taking complete notes of actions taken while utilizing Granicus software and/or other audio recording equipment to provide for storage and playback. Subsequently, the Legislative Secretary is responsible for preparing a synopsis and/or minutes of the meeting which accurately reflect the course of the proceedings and actions taken by the body for the approval by the City Clerk for publication and distribution within prescribed deadlines. Records Management : The Office of the City Clerk is responsible for maintaining records and documents and other public disclosures required under State and local laws to be filed by elected officials, candidates for public office, lobbyists, campaigns, Board and Commission members, designated employees, and other governmental agencies. The Legislative Secretary is responsible for reading, indexing, filing, researching and retrieving documents considered and acted upon by the City Council, Council Committee, Board or Commission, as well as other documents required to be filed with the City Clerk including contracts, agreements, maps, disclosure forms, etc. The Office must respond to requests under the California Public Records Act for information from members of the public and media and/or research information, as well as records and documents requested by various City departments and other public agencies.

THE DEPARTMENT

The Community Development department is responsible for ensuring a sustainable and vibrant community that supports a high quality of life for residents, businesses and visitors. It includes the Building, Planning and Housing divisions, and core services include building permits, plan check, inspections, affordable housing administration and housing initiatives, as well as long-range planning and development review. The Building Division is responsible for managing the building permit, plan check and inspection processes for compliance with all applicable State laws, local municipal codes and related policies involving building construction, which requires a high level of customer service with the public and coordination within and across departments and agencies.

THE POSITION

The Building Official oversees the development of programs, projects, operations, and services of the Building Division, and is a part of the Community Development management team. This position will manage a 13-member team, including a Permit Manager, Building Permit Technicians, Plan Check Engineer, and Building Inspectors, and will receive general direction from the Assistant Community Development Director or the Community Development Director. Successful performance in this role requires professional and technical skills in the core functional areas of permitting, plan checking and building inspections, as well as knowledge of public policy and municipal services, and excellence in communication and leadership. The individual must be able to assess the current needs of the department and look for innovative ways to help the department advance to improve service delivery and meet the changing needs of the community. This will require guiding modifications to the City’s land management permitting system (Accela) to improve processes, enhance customer service and monitor data.

With the adoption of the General Plan Update in 2016 and, most recently, the Housing Element zoning-related changes, the City is expected to see physical change as plans come to fruition. A key priority for this position will be overseeing the building permit process for a diverse range of projects from tenant improvements to new single-family residential construction to hotels to large-scale, comprehensive planned mixed-use campuses. Currently, the City has approximately 775 residential units and 260,000 square feet of life science uses under construction, with over 2,000 residential units, two hotels and more than 1.8 million square feet of commercial square footage approved, but not yet under construction, along with a number of large development projects currently under review. The Environmental Services Department is currently seeking to hire three (3) Associate Engineer positions:1 (one) vacancy at Water Resources Division is located at 3025 Tuers Road, San Jose and 2 (two) vacancies at Capital Improvement Project (CIP) division is located at 700 Los Esteros Road, San Jose.

About the Water Resources Division: The Water Resources Division is comprised of three sections: Municipal Water System, South Bay Water Recycling, and Operations and Maintenance. The Water Resources Division provides potable water service through the Municipal Water System to the communities of Evergreen, North San José/Alviso, Edenvale, and Coyote. The Water Resources Division also provides recycled water service through the South Bay Water Recycling system to the cities of San Jose, Santa Clara, and Milpitas. South Bay Water Recycling includes 143 miles of pipeline and facilities, including four pump stations, three reservoirs, and Retail Agency Operations and Maintenance activities for over 800 individual customer connections. In addition, the Water Resources Division also leads water conservation efforts for the City of San José.

About the Capital Improvement Program Division: Join the San José-Santa Clara Regional Wastewater Facility (RWF) Capital Improvement Program (CIP) division in leading one of the largest public works programs in San José's history! Check out CIP's website for more information on the $1.4 billion infrastructure construction program currently in its first 10-year phase. While all this planning and construction is ongoing, the facility is in continuous operation, serving 1.4 million residents and over 17,000 businesses in eight cities.

Associate Engineer Position at South Bay Water Recycling (SBWR) Section

The SBWR section is responsible for system operation and maintenance engineering support, and design and construction of the SBWR Recycling Water System pipeline and facilities. This includes managing the water quality programs, and supporting the conservation program, providing development related support services, ensuring availability of recycling water supply, capital improvements, construction inspection, and engineering support to operations and maintenance. Typical duties for the position include but are not limited to:

• Supervises staff including prioritizing and assigning work; conducting performance evaluations; ensuring staff is trained; making hiring, termination, and disciplinary recommendations.Prepares engineering plans, details, project schedule, engineer's cost estimates and specifications.
• Directs the collection and analysis of water quality samples and reviews and interprets the resulting data including preparing recommendations as may be appropriate.
• Prepares, coordinates, and implements consultant, contractor, and laboratory analysis agreements.
• Research technical documents and assist in developing and conducting water quality studies to achieve business objectives and goals.
• Conducts and/or delegates data analysis, summarizes data with tables and graphs, and prepares reports and/or presentations to explain causes and effects on water quality as required by management.
• Provides direct support and/or oversees provision of direct support to customers as necessary to respond to water quality-related inquiries.
• Maintains and assists in optimizing databases that may include but not limited to water quality database, water quality compliant database, etc.
• Delegates and/or develops programs to maintain full compliance with applicable requirements in water quality, including updating existing plans or creating new plans as necessary to ensure compliance with new water quality regulations.
• Prepares and/or approves reports for compliance purposes that may include but not limited to: California Division of Drinking Water (DDW) Annual Report, Consumer Confidence Report, Public Health Goal Report.
• Represents the City with contractors, consulting firms, DDW, EPA, Regional Water Quality Control Board, and other water quality and environmental regulating bodies, as required, and at public meetings concerning City policy on engineering projects.
• Oversees and prepares plan documents, including but not limited to, Assembling and reviewing plans and specifications for conformance. directs the inspection of major engineering projects, prepares bid documents, solicits bids, facilitates award of contracts, administers the contract, interprets plans and specifications, resolves field problems, gives final inspection, and follows through with acceptance of the project.

Associate Engineer Position at Capital Improvement Program

The Capital Project Delivery section has a wide variety of responsibilities, including preparing and/or reviews designs, improvement plans, condition assessments, feasibility studies, construction estimates, specifications, and contract documents for capital improvement projects that are prepared by subordinates and/or consultants engaged by the City. Typical duties for the position include but are not limited to:

• Manages capital improvement projects from concept development to completion, i.e., overseeing, controlling, and guiding projects.
• Prepares engineering plans, project schedule, engineer's cost estimates and specifications.
• Negotiates, drafts, and manages consultant agreements and/or construction contracts.
• Supervises staff including work prioritization, performance evaluations, training and involvement in hiring and disciplinary processes.
• Oversees the preparation and/or reviews of designs and contracts documents for various of construction projects or engineering related projects.
• Evaluates and coordinates responses for bid documents and construction documents.
• Interprets plans and specifications, resolving field problems, conducting inspection, and facilitate project acceptance procedures.
• Collaborates with construction inspectors and attends field meetings regarding problems in the field during the construction.

The Human Resources Department is seeking well qualified candidates for the position of HRIM /Workday Application Support Analyst II. We are seeking highly skilled and motivated individuals to join our Human Resources Information Management /Workday team.

As the HRIM /Workday Application Support Analyst II, you will collaborate and consult with colleagues in various departments to understand business and functional drivers across all Workday HR modules. Your responsibilities will include hands-on configuration of Workday features, security, development of reports and dashboards, and provision of sound solutions to support end-to-end business processes. Additionally, you will assist with Workday tenant management, deliver exceptional customer support, and contribute to system testing and issue resolution.

Once job duties are learned, this position has the option of hybrid work, two or three days in the office and two days of remote work.

The base hourly salary range for this position is $56.47 - $70.58. Because the position is classified as "confidential" there is a 5% allowance added to the salary. This allowance has been added into the advertised salary range.

Qualifications

Experience/Education:

• 3-5 years of proven Workday expertise in designing, implementing, and supporting business process and security solutions.
• Workday Pro certification in a relevant area is preferred but not required.
• Bachelor's Degree or higher in Information Technology or an equivalent field of study, is preferred.

Knowledge, Skills, and Abilities:

• Proven experience leading multiple end-to-end Workday projects and implementing solutions for operational improvements and automation.
• Knowledge of basic and advanced security configuration including Workday delivered and custom based security roles based on the organization.
• Thorough knowledge of various security group types (role based, user based, segment based, etc.) and how they are applied in the system.
• Thorough understanding of Workday business process configurations.
• Ability to build and modify custom Workday reports and calculated fields.
• Experience in creating, loading, and troubleshooting EIBs.
• Strong interpersonal and communication skills, both verbal and written.
• Attention to detail and ability to handle multiple projects and shifting priorities effectively.
• Demonstrates confidentiality and professionalism in handling sensitive matters.
• Thrives in a demanding and fast-paced environment, both independently and as part of a team.
• Displays discretion and exercises independent judgment.
• Excellent analytical, troubleshooting, organizational, and planning skills.
