San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.
• Experienced with Behavioral Health and Recovery Services (BHRS), as well as other mental health community programs.
• Knowledgeable of the systems of care related to older adults, mental health, substance abuse, developmental delay, pediatrics, primary care, palliative care, chronic illnesses, dialysis, conservatorship, and probation and parole.
• Able to work independently with little supervision and demonstrates a high degree of initiative.
• Knowledgeable of housing resources, their requirements for eligibility, and other community services such as In-Home Supportive Services (IHSS), Multipurpose Senior Services Program (MSSP), Institute on Aging (IOA), Health Plan of San Mateo (HPSM), BHRS, and Bridges to Wellness, and more.
• Knowledgeable about County benefit and entitlement programs, such as Medi-Cal and ACE.
• Experienced and comfortable working with the following populations: multi-cultural, homeless, chronic diseases, end-of-life, mental health, domestic violence, parolees, and substance abuse disorders.
• Experienced working with a diverse population (race, ethnicity, age, ability, language, nationality, socioeconomic status, gender, religion, and sexual orientation).
• Able to accurately assess for signs of adult and child abuse and be a mandated reporter.
• Able to work in a politically charged environment under high degrees of stress and meet all deadlines.
• Able to develop effective and positive working relationships with community partners such as Aging and Adult Services, BHRS, and HPSM.
• An effective communicator to a broad range of stakeholders, including doctors, nurses, conservators, health advocates, families, and patients.
• Able to remain calm during crisis situations and perform emergency crisis counseling for families and patients as needed.
• Experienced documenting in an EMR.
• Able to communicate clearly and concisely.
• Spanish-speaking preferred.

Examples Of Duties

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.
• Maintain case files utilizing the electronic medical record.
• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.
• Document interactions with patients/clients in EMR.
• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

SUMMARY DESCRIPTION

DISTINGUISHING CHARACTERISTICS

Street Maintenance Operator I: This is the entry level class in the Street Maintenance Operator series. This class is distinguished from the Street Maintenance Operator II by the performance of the more routine tasks and duties assigned to workers in this series. Employees at this level are not expected to perform with the same independence of direction and judgment on matters related to established procedures and guidelines as are positions allocated to the II level. Since this class is typically used as a training class, employees may have only limited or non directly related work experience. Employees work under immediate supervision while learning job tasks.

Street Maintenance Operator II: This is the full journey level class within the Street Maintenance Operator series. This class is distinguished from the Street Maintenance Operator I by the assignment of the full range of duties assigned. Employees at this level receive only occasional instruction or assistance as new or unusual situations arise, and are fully aware of the operating procedures and policies within the work unit. Positions in this class are flexibly staffed and are normally filled by advancement from the I level.

REPRESENTATIVE DUTIES

Glenn County is recruiting individuals to fill three full time and multiple Part Time/ Extra Help positions in the Public Works Agency. The Public Works Maintenance Worker I/II/III position performs a variety of semi-skilled and skilled labor in the construction, maintenance and repair of County roads, bridges, and other related structures, and in various traffic control maintenance and  repair activities of the Public Works Agency; operates a variety of construction, maintenance and transportation equipment as necessary to complete daily tasks at the Glenn County Public Works Agency. The Public Works Maintenance Worker III position is the journey-level class in the series, and is distinguished from the Public Works Maintenance Worker II by the primary responsibility to operate heavy diesel and gasoline-powered construction equipment in the maintenance of roadways, drainage systems.

THE POSITION

THE POSITION

Tri-City is looking for a special individual to serve as a Clinical Therapist I/II for the Children’s Full Service Partnership/Children and Transitional Age Youth (FSP/ C TAY). JOB-RELATED QUALIFICATIONS

Knowledge of: Principles, practices, and techniques of the installation, maintenance, and repair of water treatment plan systems; operation of various hand and power tools; proper procedures to safely perform maintenance on machinery and equipment in the plant; maintenance for MP2 program; recording and updating repair history on water pollution control plant equipment; how to create new work orders and close out completed work orders; forklift operations and scissor lift; replacing, repairing, and installing piping, fittings, and valves on water pollution control equipment; data entry of important, vital information regarding maintenance repairs; performance and evaluation on future preventative maintenance in the WPCP MP2 program; proper protective equipment.

EDUCATION AND TRAINING GUIDELINES

Any combination of experience and training that would likely provide the required knowledge and abilities. A typical way would be:

Education: High school diploma or equivalent (GED). Technical training in a water treatment plant system or related field is desirable.

Experience: One (1) year of full-time experience in the installation, maintenance, and repair of pollution control plant and sewage pumping facilities.

Licenses: Possession of a valid California Class C driver's license, obtain and maintain a California Class B Driver’s License, and a satisfactory driving record. CWEA Certification as a Grade I Mechanical Technologist.

SPECIAL REQUIREMENTS

Respond to work at any hour of the day or night, within designated timeframe, as required during afterhours, weekend service, and/or emergency situations; carry communication equipment in order to respond after work hours. Vision, hearing, and physical ability adequate to obtain and maintain a California Class B Driver’s License; speak clearly and understandably. Requires moderate and sometimes strenuous exertion including bending, stooping, crouching, crawling, climbing, lifting, twisting, digging, carrying, and/or handling material weighing up to one hundred (100) pounds. Work in confined spaces and wear protective equipment. A Dispatcher II is a civilian employee of the Police Department who performs a variety of tasks in the Department’s Support Services Division. The Dispatcher II acts as the Department’s primary dispatcher, receiving and relaying incoming calls for emergency and non-emergency assistance; dispatching police units; operating a variety of telecommunications equipment including radio, telephone, and computer-aided dispatch systems; status monitoring, records and file maintenance in a computer-aided environment; and entry level dispatch training. A Dispatcher II may be assigned to work various hours, shifts, holidays, weekends and overtime.

Additional duties may include, but are not limited to, the following: prioritize requests for service based upon available resources, receive radio requests and handle accordingly, type, process and file a wide variety of reports and materials including memos, letters, complaints, declarations, warrants, citations, crime and traffic reports.  Assists Department personnel and the public in person and by phone performing record checks, collecting fees and mailing requested reports to outside agencies in accordance with established regulations.

QUALIFICATIONS

Have a combination of education and work experience, including a minimum of 1 year of dispatch work; general office work involving typing of varied materials, public contact, general filing and basic record keeping; type at least 40 wpm (net).  Have knowledge of business English, spelling; modern office practices and procedures; record keeping. Ability to dispatch police units quickly and effectively; work under pressure; exercise independent judgment; work with minimal supervision; make sound decisions in emergency situations; learn assigned clerical tasks readily and to adhere to prescribed routine; write English legibly; establish and maintain effective working relationships with other employees and the general public; operate computer-aided dispatch system, 911 emergency telephone, and other multi-line telephone systems, various law enforcement telecommunications systems, radio console with headsets and handsets, computer keyboards, cameras, intercoms, security. • Salary:  $6,538 to $8,468 per month. Placement within the range depends on experience and qualifications.
