San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.
• Experienced with Behavioral Health and Recovery Services (BHRS), as well as other mental health community programs.
• Knowledgeable of the systems of care related to older adults, mental health, substance abuse, developmental delay, pediatrics, primary care, palliative care, chronic illnesses, dialysis, conservatorship, and probation and parole.
• Able to work independently with little supervision and demonstrates a high degree of initiative.
• Knowledgeable of housing resources, their requirements for eligibility, and other community services such as In-Home Supportive Services (IHSS), Multipurpose Senior Services Program (MSSP), Institute on Aging (IOA), Health Plan of San Mateo (HPSM), BHRS, and Bridges to Wellness, and more.
• Knowledgeable about County benefit and entitlement programs, such as Medi-Cal and ACE.
• Experienced and comfortable working with the following populations: multi-cultural, homeless, chronic diseases, end-of-life, mental health, domestic violence, parolees, and substance abuse disorders.
• Experienced working with a diverse population (race, ethnicity, age, ability, language, nationality, socioeconomic status, gender, religion, and sexual orientation).
• Able to accurately assess for signs of adult and child abuse and be a mandated reporter.
• Able to work in a politically charged environment under high degrees of stress and meet all deadlines.
• Able to develop effective and positive working relationships with community partners such as Aging and Adult Services, BHRS, and HPSM.
• An effective communicator to a broad range of stakeholders, including doctors, nurses, conservators, health advocates, families, and patients.
• Able to remain calm during crisis situations and perform emergency crisis counseling for families and patients as needed.
• Experienced documenting in an EMR.
• Able to communicate clearly and concisely.

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.

• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.

• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.

• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

San Mateo Medical Center (SMMC), part of San Mateo County Health, is seeking a qualified and experienced Diabetes Nurse Educator for its Innovative Care Clinic's (ICC), Diabetes Self-Management Education. The Innovative Care Clinic (ICC) is a fully functional adult care clinic providing an interdisciplinary and integrated model of care emphasizing collaboration across professional roles.  The Diabetes Nurse Educator will also serve the Ron Robinson Senior Care Center, Coastside, and Mental Health Primary Care clinics.

The Diabetes Nurse Educator functions as a patient educator collaborating with patients with a diagnosis of diabetes to educate them on areas such as weight management, food intake, medications and the proper usage of a glucometer.  The goal is to assist patients with high A1C to lower their numbers to be within normal range.  The Diabetes Nurse Educator will follow the American Diabetes Association guidelines for Diabetes Self-Management Education (DSME).  The educator is responsible for submitting regular data to the ADA to retain SMMC's certification.  The educator will lead a team of diabetes educators across the SMMC system by conducting regular meetings to discuss any updates on DSME.

The ideal candidate has at least one year of experience in ambulatory care setting addressing chronic disease management; is skilled in patient diabetes self-management education, triage, and assessment; has worked with the underserved population and on interdisciplinary teams. The ideal candidate is also a critical thinker, fast learner and thrives in a dynamic environment.  Computer literacy, prior experience with electronic medical records (E-Clinical Works and/or EPIC preferred).  Experience in data tracking/management is highly desirable.  Bilingual in Spanish and English is preferred.

Street Maintenance Operator II: This is the full journey level class within the Street Maintenance Operator series. This class is distinguished from the Street Maintenance Operator I by the assignment of the full range of duties assigned. Glenn County is recruiting individuals to fill three full time and multiple Part Time/ Extra Help positions in the Public Works Agency. The Public Works Maintenance Worker I/II/III position performs a variety of semi-skilled and skilled labor in the construction, maintenance and repair of County roads, bridges, and other related structures, and in various traffic control maintenance and  repair activities of the Public Works Agency; operates a variety of construction, maintenance and transportation equipment as necessary to complete daily tasks at the Glenn County Public Works Agency. The Aging and Adult Services Division of San Mateo County Health is seeking a full-time Deputy Public Guardian Conservator II/III (DPG). Deputy Public Guardians manage the income and assets and coordinate the care, including the arrangement of placement and treatment, of elderly and gravely disabled persons under the jurisdiction of the Public Guardian as appointed by the Superior Court.  Deputy Public Guardians visit clients on a regular basis, which includes travel to various locations both within and outside of San Mateo County.
 
The current vacancy is in the LPS (Lanterman Petris Short Act) conservatorship unit; however, this recruitment may be used to fill vacancies in either Probate or the LPS conservatorship unit.
 
DPG II is at the journey level with responsibility for managing estates and providing casework services. DPG II administers complex property and litigation cases and may develop comprehensive care plans. DPG III also provides lead direction to other deputies, serves as liaison with specific County and State facilities, handles those cases that present the greatest liability to the client and/or the County, and may provide 24-hour emergency response services. All DPGs must accept very diverse roles in the areas of estate management and care coordination.
 
This person will report to the Supervising Deputy Public Guardian of Aging and Adult Services.
 
The ideal candidate will be able to develop and maintain collaborative partnerships with a variety of organizations; have strong assessment, motivational interviewing, investigative, oral and written communication skills; have familiarity working with Adult Protective Services (APS), law enforcement, San Mateo County Superior Court system, Behavioral Health and Recovery Services, San Mateo Medical Center and an understanding of the Probate Code. The ideal candidate should also have experience conducting psychosocial assessments and meeting the needs of persons suffering from serious mental illness, as well as experience with asset management and knowledge of various public benefits programs. Lastly the ability to be politically astute, perform client engagement/ case management tasks from a strength-based, culturally sensitive, and racially equitable framework, is ideal as well as the ability to perform duties in a fast-paced environment and exercise discernment and critical thinking to problem-solve immediate client-centered challenges.

The Deputy Public Guardian may have to facilitate application for a reverse mortgage, identify and marshal Trust and conservatorship assets, and reconcile these two separate legal entities, as well as develop an estate plan, while ensuring that the client receives care appropriate to his/her needs.
 
 Client Care Coordination:
  

• Arranges for the person's care and protection.
• Decides where the person will live.
• Makes arrangements for health care, meals, clothing, personal care, housekeeping, transportation, and recreation.
• Makes placement decisions that allows for clients to be at the least restrictive placement while ensuring the safety of the public.
• Represents or arranges representation in all legal matters.
• Performs de-escalation and motivational interviewing interventions on an as needed basis.
• Collaborates with the case managers, psychiatrists, social workers, and facility administrators to put together a plan to address emergent and/or behavioral issues and find alternative placement options, as needed.

For example, a client in a board and care home or locked facility may be having a behavioral problem. The Deputy Public Guardian will work with the case manager, psychiatrist, social worker, and facility administrator to put together a plan to address the client's behavioral issues and may also need to work with the treatment team on finding alternative placement, if needed.
 
 Court:

• Arranges appropriate representation in all legal matters.
• Carries out legal and regulation requirements pertaining to the Lanterman-Petris-Short Act (LPS).
• Period reviews of LPS reports to ensure quality assurance/performance.
• Prepares and submits comprehensive reports to the Probate Court providing updates and/or addressing financial, social, placement and medical matters.

Lanterman-Petris-Short Act (LPS) requires the review of client's current needs based on documentation of visits and care conferences, as well as annual report writing regarding that information. Probate requires a review of the past two years of care through the writing of a comprehensive report on financial transactions, social interactions, and medical conditions.

WORK ENVIRONMENT

Work is conducted both in a fire station and in the field in various settings. The Analyst I/II position in the Bureau of EMS & Training (BET) reports directly to the EMS Division Battalion Chief . The BET EMS Division Analyst I/II position will primarily support EMS Performance Analytics, contractual performanc e compliance , special studies, grant administration, and other EMS general administrati ve activities.

San Mateo County Health's Correctional Health Services is seeking qualified and experienced Correctional Health Nurses to perform varied professional nursing duties with primary emphasis on professional nursing care for inmates within a locked correctional facility; treat and educate patients about various medical conditions; record medical histories and symptoms; perform diagnostic tests and analyze results; operate medical machinery; administer treatment and medications to over 100 patients in an 8-hour shift; assist with patient follow-up and rehabilitation; and perform other duties as assigned and required. Correctional facilities include the Maguire and Maple Street Correctional Centers (Redwood City) and Youth Services Center (San Mateo).

The current vacancies are for five benefitted regular full- time (1.0 FTE) Correctional Health Nurses at our Maguire and Maple Street Correctional Centers in Redwood City and at our Youth Services Center in San Mateo for a variety of shifts.

Under general direction, Correctional Health Nurses are fully competent to independently perform a full range of professional nursing duties and procedures requiring one to exercise a high degree clinical judgment. Incumbents also provide lead direction to other nursing and paraprofessional personnel.

Correctional Health Services provides integrated medical, dental, mental health, and chemical dependency treatment to the incarcerated adult population, as well as medical and dental care to the juvenile hall population of San Mateo County, as described below:

• Medical services include health screening, treatment, education, and 24-hour emergency response for adults.
• Clinic appointments are available by referral and upon request and include chronic care for hypertension, diabetes, asthma, and HIV, as well as obstetric, orthopedic, and urgent care.
• Dental services include treatment and medication for urgent dental conditions and extractions for adults and juveniles.
• Mental health services include crisis intervention, evaluation, treatment, psychiatric medication, behavior management, and continuing care referrals.
• Day treatment services are provided to mentally ill men and women in the Life Skills Program at the Maguire and Maple Street Correctional Centers.
• CHS also provides weekly group therapy; cognitive behavioral approach to addiction and recovery; and re-entry services for those transitioning back into the community.

The ideal candidates:

Will be well-rounded and will have knowledge of Correctional Health and Mental Health nursing; Emergency Room or ICU experience; or a strong Medical/Surgical and Telemetry foundation. OB/GYN, pediatric experience and psychiatric care experience are also ideal, but not required. Candidates must be able to administer medications to over 100 patients within an 8- hour shift, in a locked correctional facility and on weekends.

Examples Of Duties

Duties may include, but are not limited to, the following:

• Administer treatment and medications to over 100 patients within an 8-hour shift.
• Complete patient assessments and triage, as well as record patient histories, symptoms, and observations.
• Establish and implement nursing care plans for each patient through assessment, development, activation, and evaluation.
• Participate in patient case conferences and rounds.
• Organize, assign, and supervise duties and responsibilities of other staff members as a team leader.
• Orient and teach new staff members of the nursing team and students.
• Perform nursing care following prescribed rules, policies, protocol, and procedures.
• Identify problems, assist in data gathering, propose solutions and participate in evaluation of quality assurance issues.
• Work with patients and their families to provide in-hospital and after-hospital follow-up through teaching and referral.
• Work with patients and their families to provide in-custody and after custody follow-up through teaching and referral.
• Participate in educational programs to maintain level of expertise.
• Participate in non-clinical assignments, which may include maintaining records, preparing reports, and/or serving on special committees.
• Maintain records and prepare reports.
• Operate a variety of medical machinery and equipment.
• Maintain knowledge of outpatient resources.
• Develop discharge plans for patients.
• Perform related duties as assigned and as required by assigned work setting, as described below.

Duties for nurses in this series may also include the following:

• Perform professional nursing duties within a locked correctional facility.
• Check vital signs; give injections; perform catheterizations, IZ Registry, Point of Care Testing; and draw blood for medical purposes.
• Start, maintain, and discontinue intravenous lines.
• Monitor patient rehabilitation.
• Perform mental status exams and implement plan of care based on assessment findings.
• Monitor and document on patients in restraint and seclusion while maintaining patient safety.
• Determine medical or psychiatric emergency need and provide intervention.
• Recognize symptoms of major psychiatric diagnosis.
• Perform related duties as assigned.

Qualifications

Licensure/Certification:

Possession of a California license as a Registered Nurse or equivalent and a valid Basic Life Support (BLS) certification.
