The City of Sunnyvale’s Department of Public Works is recruiting for two Senior Maintenance Worker positions within the Street Operations division. One Senior Maintenance Worker position will be assigned to the pavement team responsible for maintenance, repair, sweeping and cleaning of City streets, curbs, gutters, and similar right-of-way infrastructure. One Senior Maintenance Worker position will be assigned to the traffic markings and signs team responsible for a variety of tasks involving installing and maintaining traffic signs, painting pavement markings and striping, and repairing concrete sidewalk for the city. This classification reports to a Public Works Supervisor and receives assignments related to the above-mentioned maintenance tasks.

The Department

The Human Services Department (HSD) delivers and supports services by forging long-term community partnerships; engaging with and building the capacity of the community to do its own problem- solving; and leveraging financial and volunteer resources.  The Aging and Family Services Division (AFS) of HSD specifically supports older residents and their caregivers to live active, healthy lives in the Tri-City community.

The Position

As a member of the HSD assigned to the AFS, the Senior Program Coordinator will organize, coordinate, and implement community wellness programs for older adults. The Community Programs include health promotion activities such as health screenings, wellness classes, and educational activities. The position works within a team that includes volunteers, health promoters, nurses, social workers and mental health workers. The position facilitates community awareness and partnerships, with individuals and agencies, to promote an age-friendly environment.

Day to day, the Deputy Director will assist in the management and administration of the Recreation Department. • Prepares a variety of regular and special reports and records on operations and activities related to the Recreation Department; develops professional service contracts, requests for qualification and proposal and completes grant applications and proposals.
• Responds to and resolves citizen inquiries and complaints and controversial issues.
• Represents the Department and the Director on various occasions and serves on a variety of internal and external committees. Acts in the absence of the Director.

San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.
• Experienced with Behavioral Health and Recovery Services (BHRS), as well as other mental health community programs.
• Knowledgeable of the systems of care related to older adults, mental health, substance abuse, developmental delay, pediatrics, primary care, palliative care, chronic illnesses, dialysis, conservatorship, and probation and parole.
• Able to work independently with little supervision and demonstrates a high degree of initiative.
• Knowledgeable of housing resources, their requirements for eligibility, and other community services such as In-Home Supportive Services (IHSS), Multipurpose Senior Services Program (MSSP), Institute on Aging (IOA), Health Plan of San Mateo (HPSM), BHRS, and Bridges to Wellness, and more.
• Knowledgeable about County benefit and entitlement programs, such as Medi-Cal and ACE.
• Experienced and comfortable working with the following populations: multi-cultural, homeless, chronic diseases, end-of-life, mental health, domestic violence, parolees, and substance abuse disorders.
• Experienced working with a diverse population (race, ethnicity, age, ability, language, nationality, socioeconomic status, gender, religion, and sexual orientation).
• Able to accurately assess for signs of adult and child abuse and be a mandated reporter.
• Able to work in a politically charged environment under high degrees of stress and meet all deadlines.
• Able to develop effective and positive working relationships with community partners such as Aging and Adult Services, BHRS, and HPSM.
• An effective communicator to a broad range of stakeholders, including doctors, nurses, conservators, health advocates, families, and patients.
• Able to remain calm during crisis situations and perform emergency crisis counseling for families and patients as needed.
• Experienced documenting in an EMR.
• Able to communicate clearly and concisely.
• Spanish-speaking preferred.

Examples Of Duties

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.
• Maintain case files utilizing the electronic medical record.
• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.
• Document interactions with patients/clients in EMR.
• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

The County of Yuba is currently recruiting for the position of a Limited Term Project Manager in the Administrative Services Department, with a focus on the scheduled Library Renovation. This limited term appointment is set to end in June 2025. 

The incumbent will plan, direct and coordinate activities concerned with the implementation or administration of a project; participate in the conceptual development of a project and oversee its organization, scheduling, budgeting, and implementation; coordinate project activities or functions with other County staff, associated agencies, and contractors as appropriate; may direct, review and evaluate the work of assigned staff. About Administrative Services:

The Administrative Services Department provides logistical and resource support to all County departments. We are the primary provider of Custodial Services, Fleet Services, Capital Improvement Planning and Acquisition and Facilities Maintenance. We provide purchasing guidance and serve as Purchasing Agent for County procurement needs as well as assist vendors interested in doing business with the County.

THE POSITION

Tri-City is looking for a special individual to serve as a Clinical Therapist I/II for the Children’s Full Service Partnership/Children and Transitional Age Youth (FSP/ C TAY). The Clinical Therapist I/II provides behavioral health clinical intervention and recovery services to assigned clients and their families; develops treatment plans and performs individual, group, and family psychotherapeutic counseling, routine and emergency assessment, crisis intervention, and related treatment services; manages an assigned caseload of adults, juveniles, and/or children; provides consultation to other staff, programs, and agencies related to treatment needs and plans; and performs related duties as assigned.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Provides individual, group, and family interventions, rehabilitation, and treatment services including counseling, therapy, case management, and referral and linkage to community services to a variety of clients including high acuity clients.

• Within scope of authority, assesses, diagnoses, and treats a variety of clients with mental health disorders; collaborates with clients and creates individualized treatments plans according to client’s needs and circumstances.

• Initiates and conducts individual, group, and family counseling and biopsychosocial therapy sessions utilizing a variety of techniques and modalities; provides case management services in the office and in the field.

• Performs routine and emergency intake evaluations according to Authority procedure; completes appropriate forms and documents relating to the intake process; and prepares medical and demographic case history for use in diagnosis.

• Assists clients and family members in understanding behavioral health diagnosis, contributing factors/causes, effects, prognosis, rehabilitation, and treatment.

• Works with clients to address stressors and barriers to treatment and/or social functioning; identifies and focuses on client strengths and personal resources to effectively engage and treat client.

DESCRIPTION/RESPONSIBILITIES:
The City of San José Housing Department is a leader in the development of affordable housing. Its mission is to strengthen and to revitalize our community through housing and neighborhood investment. The Housing Department provides loans for the development of affordable housing and manages a portfolio of over 17,000 housing units with a total value that exceeds $700 million. Over the next five years, the City Council has a goal of creating 25,000 housing units, 10,000 of which will be affordable. The Housing Department has an operating budget of approximately $19 million and 110 employees. The Development Officer will be assigned to the Multifamily Project Development Program and will manage all phases of affordable housing lending. The position will oversee activities and programs within the Affordable Housing Production Division. The Development Officer reports to a Senior Development Officer. The Development Officer will be responsible for reviewing the structure of multi-family financing proposals for new construction and preservation of affordable housing. Under the direction of the Recreation Supervisor, the Recreation Coordinator II – Aquatics Safety manages the City’s Aquatics lifeguards and scheduling, including lap swim, water-walking, lifeguard certification, community CPR/First Aid classes and JR Lifeguard program.  This position also supervises all lifeguards, senior lifeguards and provides key assistance to the Recreation Supervisor on all related matters (works closely with Recreation Coordinator I- Aquatics programs to support division goals and activities).

The Aquatics & Fitness Center is a 7-day a week operation.

Recreation Coordinator II schedule may include Saturdays and Sundays and is subject to change depending on facility and supervisor needs.
