San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.
• Experienced with Behavioral Health and Recovery Services (BHRS), as well as other mental health community programs.
• Knowledgeable of the systems of care related to older adults, mental health, substance abuse, developmental delay, pediatrics, primary care, palliative care, chronic illnesses, dialysis, conservatorship, and probation and parole.
• Able to work independently with little supervision and demonstrates a high degree of initiative.
• Knowledgeable of housing resources, their requirements for eligibility, and other community services such as In-Home Supportive Services (IHSS), Multipurpose Senior Services Program (MSSP), Institute on Aging (IOA), Health Plan of San Mateo (HPSM), BHRS, and Bridges to Wellness, and more.
• Knowledgeable about County benefit and entitlement programs, such as Medi-Cal and ACE.
• Experienced and comfortable working with the following populations: multi-cultural, homeless, chronic diseases, end-of-life, mental health, domestic violence, parolees, and substance abuse disorders.
• Experienced working with a diverse population (race, ethnicity, age, ability, language, nationality, socioeconomic status, gender, religion, and sexual orientation).
• Able to accurately assess for signs of adult and child abuse and be a mandated reporter.
• Able to work in a politically charged environment under high degrees of stress and meet all deadlines.
• Able to develop effective and positive working relationships with community partners such as Aging and Adult Services, BHRS, and HPSM.
• An effective communicator to a broad range of stakeholders, including doctors, nurses, conservators, health advocates, families, and patients.
• Able to remain calm during crisis situations and perform emergency crisis counseling for families and patients as needed.
• Experienced documenting in an EMR.
• Able to communicate clearly and concisely.
• Spanish-speaking preferred.

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.
• Maintain case files utilizing the electronic medical record.
• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.
• Document interactions with patients/clients in EMR.
• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

THE POSITION:

The Facilities Carpenter I is an entry level classification in the carpentry series. Carpenters in this class are assigned routine skilled carpentry tasks. The Facilities Carpenter II is responsible for performing a variety of skilled carpentry and related tasks in the construction, remodeling, repair, and maintenance of City facilities. This the journey level classification within the carpenter series. Incumbents at this level act with independence, handle "master" craftworker projects, and assume the more complex activities which would otherwise require the attention and coordination of the Facilities and Building Manager.

4. Fabricates new cabinets, bookshelves and countertops.

8. May perform allied craft work in the course of maintaining City facilities.

The Vocational Rehabilitation Services (VRS) team of the Human Services Agency (HSA) of San Mateo County is seeking qualified candidates for the position of Rehabilitation Production Supervisor I/II. The Department

The Human Services Department (HSD) delivers and supports services by forging long-term community partnerships, engaging with and building the capacity of the community to do its own problem-solving, and leveraging financial and volunteer resources. The Department’s nationally and internationally recognized programs support thousands of residents throughout their life course, from infants to elders. The Aging and Family Services Division (AFS) of HSD specifically supports older residents and their caregivers to live active, healthy lives in the Tri-City community. With input from local, older adults (age 55 years+) as our guide, the AFS team strives to provide and AFS staff work together to provide a continuum of supportive services that range from health promotion activities to home-based end of life care. AFS has a Senior InfoLine (phone and email) that is answered in 4 language (Farsi, English, Spanish and Chinese) that is often an entry point. As an older adult or their caregiver’s situation changes, they seamlessly have access to a continuum of programs that include counseling and emotional support; care coordination and resource assistance; specialized family caregiver support and education. Many of these supportive services are provided in the resident’s home. We also provide regular expert clinical consultation, supervision, opportunities for self-reflection, and team building, fostering a culture of empathy, inclusivity, humility, and respect.

The Position

Under the supervision of the Age Well Center Managers, the Chef/Food Services Manager position manages meal programs for the City of Fremont’s Age Well Centers. Chef responsibilities across both Age Well Center locations include the performance of menu planning, purchasing of foodstuffs, preparing quantity meals, supervising and assisting kitchen employees and volunteers and maintaining clean and orderly kitchens.

The incumbent will be responsible for properly cooked, economical, and wholesome, well balanced meals in City operated Age Well Centers. This position is not limited to being responsible for the assignment of work, supervision and evaluation of paid and volunteer food service workers. Examples of Duties

• Plan menus with consideration for the dietary needs of older adults, wholesomeness, quantity of food to be prepared and budgetary constraints.
• Estimate food requirements and requisitions for food and supplies.
• Prepare, cook, and serve a wide variety of foodstuffs for quantity food service.
• May cut/carve and cook meats; bake breads, cakes and pastries; peel and prepare vegetables; and make salads.
• Place food in serving containers and serves food.
• Maintain proper temperature, rotation and storage of food.
• Direct and assist in cleaning kitchens and kitchen equipment after each meal.
• Estimate food requirements and requisitions for food and supplies.
• Direct and assist in the cleaning of the kitchen and kitchen equipment after each meal.
• Oversee that the kitchen and equipment are kept clean, sanitary and in proper working condition. Schedule regular maintenance of kitchen equipment as needed. Maintain standards of safety and sanitation in kitchen area.
• Recruit, train, supervise, instruct, and direct employees and volunteers assigned to working in the kitchens.
• Develop and support additional meal programs, including, but not limited to fundraisers, catering events and cooking demonstrations and special events.
• Keep records and prepare reports as required. Inclusive of any certification requirements and/or medical clearances required of subordinate employees and volunteers.
• Oversee community garden program, including crop rotation, seasonal planting, and maintenance assignment.
• Coordinate with office staff, marketing and sales strategies to increase visibility and participation for meal programs.
• Ensure kitchen compliance with health regulations and participate in health inspections.
• Other duties as assigned.

Community Outreach: Summer Youth Volunteers Project Plan 

The Community Outreach team is seeking a summer intern for a 10–12-week assignment.  The goal of the internship is to gain valuable experience managing high school aged volunteers that will support a variety of summer camps and programs throughout the district.  

Key duties 

• Be primary point of contact for Summer Youth Volunteers. 

• Assist with volunteer orientation and ongoing training sessions throughout the summer. 

• Oversee volunteer schedule. 

• Track volunteer hours logging. 

• Create program completion documents. 

• Assist with planning end of summer recognition event. 

Key deliverables will be: 

• Maintain volunteer hour tracking and create end of summer recap report. 

• Complete summer training and check-in reports. 

• Create and distribute volunteer completion certification with hours totals for all summer volunteers. 

• Report any concerns from staff and volunteers or development opportunities for volunteers through regular check-ins. 

What learning will occur for the intern?

Community Services Intern Project Plan

The Community Services team is seeking a summer intern for a 7-10-week assignment.  The goal of the internship is to allow a student to gain hands-on experience working alongside experienced youth development and recreation staff and assisting in promoting a safe, healthy environment within the camp setting by providing emotional support to the participants. 

Key duties

• Provides guidance, support, and assistance to staff throughout the week, which includes managing camper behavior issues, stress, and personal care needs
• Process critical incidents with supervisory staff.
• Participate in the intern group project.
• Provide support and be a role model to all participants.

Key deliverables will be:

• A training plan for camp staff to better understand best practices in working with children with special needs.
• Protocols to be followed when a situation arises with a child and what steps to take
• A template to be used for a behavior plan to include goals and actions. 

What learning will occur for the intern?

• Working in a recreation setting for a special district
• Being part of a strong staff team
• The creation of action plans to be used in a camp setting
• Communication with staff, participants, and parents
