San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

 

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

 

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

 

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.
• Experienced with Behavioral Health and Recovery Services (BHRS), as well as other mental health community programs.
• Knowledgeable of the systems of care related to older adults, mental health, substance abuse, developmental delay, pediatrics, primary care, palliative care, chronic illnesses, dialysis, conservatorship, and probation and parole.
• Able to work independently with little supervision and demonstrates a high degree of initiative.
• Knowledgeable of housing resources, their requirements for eligibility, and other community services such as In-Home Supportive Services (IHSS), Multipurpose Senior Services Program (MSSP), Institute on Aging (IOA), Health Plan of San Mateo (HPSM), BHRS, and Bridges to Wellness, and more.
• Knowledgeable about County benefit and entitlement programs, such as Medi-Cal and ACE.
• Experienced and comfortable working with the following populations: multi-cultural, homeless, chronic diseases, end-of-life, mental health, domestic violence, parolees, and substance abuse disorders.
• Experienced working with a diverse population (race, ethnicity, age, ability, language, nationality, socioeconomic status, gender, religion, and sexual orientation).
• Able to accurately assess for signs of adult and child abuse and be a mandated reporter.
• Able to work in a politically charged environment under high degrees of stress and meet all deadlines.
• Able to develop effective and positive working relationships with community partners such as Aging and Adult Services, BHRS, and HPSM.
• An effective communicator to a broad range of stakeholders, including doctors, nurses, conservators, health advocates, families, and patients.
• Able to remain calm during crisis situations and perform emergency crisis counseling for families and patients as needed.
• Experienced documenting in an EMR.
• Able to communicate clearly and concisely.
• Spanish-speaking preferred.

 

Examples Of Duties

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.
• Maintain case files utilizing the electronic medical record.
• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.
• Document interactions with patients/clients in EMR.
• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

 

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

The Department of Public Works is currently recruiting to fill a Communications Technician position for the Radio Communications Shop. The Radio Communications shop maintains the communication system for the City of San José that is used by multiple departments, such as: Police, Fire, Department of Transportation, Parks Recreation Neighborhood Services (Park Rangers), and Public Works (Animal Care & Services).

 

The City of San José public safety and city services depend upon the use of commercial radio communications equipment. For police officers and firefighters, radio communications are the primary means of communicating with the 9-1-1 dispatch center in performing their duties. These vast stand-alone radio systems with fault-tolerant designs are essential to providing the public with reliable emergency response services 24 hours a day, 365 days a year with demand for system survivability through any catastrophic event.

 

The key responsibilities of the Communications Technician may include:

     * Installation, maintenance and repair of mobile and portable radios, base and control stations, microwave systems, dispatch consoles, voting receivers and comparators, digital Microwave system and P25 LMR systems.

     * Respond to trouble reports, perform preventative maintenance and project assignments. Update technical documentation including wiring block assignments, CEB programming and configurations, as-built drawings of installed equipment at City radio sites, specialized vehicular installations, and diagrams of the City's various communications systems.

     * Provide recommendations for process improvement changes with the input of technical staff.

     * Construct and/or modify communications equipment and perform research on new equipment.

About the Position:  Midpen's Board approved the addition of two new Planner III positions to the Planning Department. The Planner III focuses on a wide variety of planning and capital improvement projects that include long-range planning; land use planning; and conceptual site design to increase public access, outdoor recreation, and enjoyment of Midpen’s lands. The Planner III is tasked with performing complex professional level planning, analysis, and design; developing and administering project scopes, schedules and budgets; directing the feasibility and conceptual design phase of capital improvement projects such as trails, staging areas and staff facilities; developing recommendations for land use policies; preparing long-range master plans to guide the use and management of Midpen lands; conducting or overseeing studies to identify and evaluate environmental issues related to public access and natural and cultural resources; conducting public outreach and engagement; preparing requests for proposals; assisting in development of grant applications; preparing and/or assisting in the preparation of environmental review documents; and obtaining planning permits and initial approvals for projects. Work Environment:  This position is part of the Planning Department at the District’s offices in Los Altos, California.  Planner III project presentations may be held in the evenings and weekends. The City of Foster City is looking for a Building Maintenance Worker I who is able to perform semi-skilled maintenance and repair work on municipal buildings, facilities, and equipment in the Building Maintenance Division. This position performs routine and standardized tasks and duties such as plumbing, carpentry, painting and electrical activities.

Education and Training Guidelines
Any combination equivalent to experience and education that could likely provide the required knowledge, skills and abilities. A typical way would be:

Education: Completion of the twelfth grade or the equivalent, supplemented by specialized training in maintenance, construction, or facilities maintenance.

Experience: One (1) year of full time experience in general maintenance, construction, or facilities maintenance.

Selection Process
Applications will be reviewed for relevant education, experience, training and other job related qualifications. Based upon a review of employment applications received, a limited number of the qualified applicants will be invited to participate in the examination process.

The process will consist of an Oral Panel Interview. An eligibility list will be established from those who pass the examination process with a score of 70% or higher. Current and future vacancies may be filled from this list. The list will remain in effect for at least one year unless exhausted sooner.

Role description

The City of Santa Clara is recruiting for the Electric Utility Chief Operating Officer for its Electric Utility, Silicon Valley Power (SVP).  Reporting to the Chief Electric Utility Officer (CEUO), the Electric Utility Chief Operating Officer (COO) is a senior management position in the City's unclassified service responsible for the oversight and direction for the fiscal, administrative, and managerial operations of the utility. The COO has supervisory and support responsibilities for the utility's divisions: Utility Operations, Customer Development and Project Management, Resource Planning and Customer Engagement, and Business Services. The COO will help guide SVP through the extraordinary period of growth related by doubling the existing capacity of the electrical system to meet new customer demands over the next 10 years. The annual salary range for this position is $156,770.64 - $202,878.00.  This position will be filled at or near the bottom of the salary range. 

 

 

Typical Duties:

Under general direction, duties may include, but are not limited to:
• Screens and recommends the selection of operations personnel; 
• Deals effectively with a wide range of personnel matters;
• Assists in the preparation of the annual budget for Communications Operations;
• Supervises operations personnel;
• Prepares statistical data;
• Conducts and oversees training programs for operations personnel;
• Performs liaison duties with other City departments;
• Coordinates emergency operations between various departmental personnel and equipment;
• Operates computer equipment for information essential to the operation of the department;
• Acts as the custodian of record for retention of recorded data;
• Maintains statistical information and other necessary data;
• Investigates and resolves complaints received from the public, user departments and other agencies;
• Counsels employees when appropriate; and
• Performs related duties as required for the position, including dispatching in times of emergency or when needed.

 

 

Minimum Qualifications:

EDUCATION AND EXPERIENCE
• Equivalent to graduation from high school and thirty (30) semester units of college level work with an emphasis in public safety, supervision and management, public administration or related subjects; and
• Five years' experience dispatching in the public safety field, at least two (2) years of which having been at the supervisory level.
Possible Substitutions:
• Administrative or supervisory experience in a public safety department may be substituted for the dispatching experience on a year for year basis.

 

LICENSE
Possession of a valid California Class C driver's license is required at time of appointment and for duration of employment.

 

SPECIAL CONDITIONS
• Willing to respond to callback in case of major emergencies or local disasters;
• Willing to work Saturdays and odd, irregular and unusual hours;
• Will be required to pass a City background investigation, which will include fingerprinting, polygraph and/or psychological testing; and
• Must be able to perform all of the essential functions of the job.

Protective Services Workers play a crucial role in ensuring the safety and well-being of children and families. They engage in case planning, risk assessments, and deliver vital support services.

 

Essential Duties but Not Limited:

The City of Sunnyvale invites applications for Public Safety Dispatcher in Training. Under supervision and in a trainee capacity, processes emergency and non-emergency calls for service; dispatches police, fire, medical, rescue, animal control or other emergency equipment, by telephone or radio, utilizing complex technical aids, such as computer aided dispatch systems (CAD); and performs other related duties as assigned.

 

Public Safety Dispatcher-In-Training is the entry and trainee level in the public safety dispatcher series. Under close supervision, incumbents are trained in dispatching techniques, equipment, and protocols. Work assignments change frequently to facilitate training. Employees are subject to daily and weekly observation reports until they demonstrate the ability to perform independently. This is an alternately staffed classification. Public Safety Dispatcher-In-Training incumbents are required to achieve the knowledge, skills, and abilities necessary to advance to the Public Safety Dispatcher level within the twelve-month probationary period.
