San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.
• Experienced with Behavioral Health and Recovery Services (BHRS), as well as other mental health community programs.
• Knowledgeable of the systems of care related to older adults, mental health, substance abuse, developmental delay, pediatrics, primary care, palliative care, chronic illnesses, dialysis, conservatorship, and probation and parole.
• Able to work independently with little supervision and demonstrates a high degree of initiative.
• Knowledgeable of housing resources, their requirements for eligibility, and other community services such as In-Home Supportive Services (IHSS), Multipurpose Senior Services Program (MSSP), Institute on Aging (IOA), Health Plan of San Mateo (HPSM), BHRS, and Bridges to Wellness, and more.
• Knowledgeable about County benefit and entitlement programs, such as Medi-Cal and ACE.
• Experienced and comfortable working with the following populations: multi-cultural, homeless, chronic diseases, end-of-life, mental health, domestic violence, parolees, and substance abuse disorders.
• Experienced working with a diverse population (race, ethnicity, age, ability, language, nationality, socioeconomic status, gender, religion, and sexual orientation).
• Able to accurately assess for signs of adult and child abuse and be a mandated reporter.
• Able to work in a politically charged environment under high degrees of stress and meet all deadlines.
• Able to develop effective and positive working relationships with community partners such as Aging and Adult Services, BHRS, and HPSM.
• An effective communicator to a broad range of stakeholders, including doctors, nurses, conservators, health advocates, families, and patients.
• Able to remain calm during crisis situations and perform emergency crisis counseling for families and patients as needed.
• Experienced documenting in an EMR.
• Able to communicate clearly and concisely.
• Spanish-speaking preferred.

Examples Of Duties

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.
• Maintain case files utilizing the electronic medical record.
• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.
• Document interactions with patients/clients in EMR.
• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

PHYSICAL, MENTAL AND ENVIRONMENTAL WORKING CONDITIONS

Position requires sitting, standing, walking on level and slippery surfaces, reaching, twisting, turning, kneeling, bending, stooping, squatting, crouching, grasping and making repetitive hand movement in the performance of daily duties. The position also requires both near and far vision when inspecting work and operating assigned equipment. The need to lift, carry and push tools, equipment and supplies weighing 25 pounds or more is also required. Additionally, the incumbent in this outdoor position works in all weather conditions including wet, hot and cold. The incumbent may use cleaning and lubricating chemicals, pesticides or herbicides which may expose the employee to fumes, dust and air contaminants. THE POSITION

The Parks and Recreation Department is looking for staff to assist with events at the Rocklin Events Center and other locations in the City. The Events Venue Specialist (Program Assistant) is a temporary position and incumbents work on an as-needed basis, not to exceed 28 hours per week and up to 999 hours per fiscal year. Temporary employment does not provide any guarantee of continued employment and the City of Rocklin may end the assignment at any time. DEFINITION

Under the general direction of the Director of Finance and Administrative Services, the Assistant Director serves as part of the management team of this municipal department and assists with planning, directing, coordinating and administering the functions of accounting, budgeting, and a wide range of support services including business licensing, payroll, utility billing, information technology and other functional areas of the Finance Department.

EXAMPLES OF DUTIES

Responsibilities may involve direct or indirect supervision of any or all of the above functions with primary emphasis on accounting, budgeting, financial analysis, payroll, and information technology. 

The Assistant Director plays a critical role in the management of accounting functions and preparation of the Comprehensive Annual Financial Report; development of City operating and capital budgets including preparing complex financial analyses, working with contract financial consultants and actuaries, developing payroll projections and providing support to labor negotiations, developing strategies to improve workflow and service delivery, and overseeing the internal service fund financial management and cost allocation.

The Assistant Director also plays a key role in evaluation of the City’s Enterprise Resource Planning (ERP) software system, including evaluation of the current and any future system, working with Finance staff in accounting, payroll, and information technology as well as Human Resources staff to ensure efficient implementation of all aspects of the system.

In addition, the Assistant Director prepares complex reports and presentations before the City Council and the public, represents the department in various meetings, resolves difficult and sensitive citizen inquiries and complaints, and serves as acting Director in the absence of the Director of Finance and Administrative Services.

MINIMUM QUALIFICATIONS

Knowledge of:   Current principles, practices and techniques of organization, administration, supervision, training, personnel management; principles of accounting and finance, including Generally Accepted Accounting Principles, governmental accounting, auditing, budgeting, financial management, revenue forecasting, risk management, automated financial information systems; and advanced applications of modern office software including Microsoft Excel, Word and PowerPoint.

To perform a variety of tasks in the maintenance of City-owned buildings, equipment and facilities.  The purpose of this recruitment is to fill a current temporary/part-time vacancy in our Community Services Department and to create a list for future vacancies.

Candidates who do not currently meet the minimum qualifications for the Facilities Maintenance Worker I position may be considered for an Intern position instead.

IDEAL CANDIDATE

Additional tasks include cleaning of walkways, street gutters, alleyways, and landscape areas. Conditions range from working outside in all types of weather conditions. Some work will involve hazardous and unsanitary conditions. Flexibility and patience are required. Must be self-motivated and able to complete job assignments without regular direct supervision. Must be able to work under stressful conditions.

Meet stand-by program requirements and serve in a stand-by status after regular working hours and respond to emergency call-outs.  Assist in the preparation of cost estimates, scheduling of work, and employee evaluations.   Requisition of materials and supplies for assigned facilities and projects.   Instruct subordinates in proper work techniques, use of equipment and safety practices and requirements. Bilingual:  Knowledge of a second language is beneficial.

License and Certificate:

Required upon hire, possession of valid Class C California Driver's License.

Forklift Certification, First Aid and CPR certifications, is required prior to permanent appointment.

Possession of a valid Class B or Class A California Driver's License is a plus.

SALARY

Community Enhancement Specialist
$4,804.06 - $6,131.34 /month
$2,217.26 - $2,829.85 /bi-weekly
$27.72 - $35.37 /hour

Future salary growth includes 4% increase effective July 2024, a 1% increase effective January 2025, a 4% increase in July 2025 and a 1% increase effective January 2026.

Retirement:                          
CalPERS Retirement System (2 % @ 60 formula for Classic CalPERS members and 2% @ 62 formula for new CalPERS members).  The City does not participate in social security

Deferred Compensation:                   
Choice of deferred compensation plans (Traditional and Roth), including a 1% match (effective July 2024).  After successful completion of probation, new hires are eligible for a 2% City contribution.
