San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

 

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

 

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

 

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.
• Experienced with Behavioral Health and Recovery Services (BHRS), as well as other mental health community programs.
• Knowledgeable of the systems of care related to older adults, mental health, substance abuse, developmental delay, pediatrics, primary care, palliative care, chronic illnesses, dialysis, conservatorship, and probation and parole.
• Able to work independently with little supervision and demonstrates a high degree of initiative.
• Knowledgeable of housing resources, their requirements for eligibility, and other community services such as In-Home Supportive Services (IHSS), Multipurpose Senior Services Program (MSSP), Institute on Aging (IOA), Health Plan of San Mateo (HPSM), BHRS, and Bridges to Wellness, and more.
• Knowledgeable about County benefit and entitlement programs, such as Medi-Cal and ACE.
• Experienced and comfortable working with the following populations: multi-cultural, homeless, chronic diseases, end-of-life, mental health, domestic violence, parolees, and substance abuse disorders.
• Experienced working with a diverse population (race, ethnicity, age, ability, language, nationality, socioeconomic status, gender, religion, and sexual orientation).
• Able to accurately assess for signs of adult and child abuse and be a mandated reporter.
• Able to work in a politically charged environment under high degrees of stress and meet all deadlines.
• Able to develop effective and positive working relationships with community partners such as Aging and Adult Services, BHRS, and HPSM.
• An effective communicator to a broad range of stakeholders, including doctors, nurses, conservators, health advocates, families, and patients.
• Able to remain calm during crisis situations and perform emergency crisis counseling for families and patients as needed.
• Experienced documenting in an EMR.
• Able to communicate clearly and concisely.
• Spanish-speaking preferred.

 

Examples Of Duties

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.
• Maintain case files utilizing the electronic medical record.
• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.
• Document interactions with patients/clients in EMR.
• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

 

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

In this role, you will have the opportunity to collaborate with the Zero Waste Operations Manager and the Solid Waste Supervisor(s) on a daily basis to ensure that all operations are carried out safely and in compliance with environmental obligations. The Senior Solid Waste Supervisor also works closely with peers across all operations to ensure consistent safety and operational standards and performance. Furthermore, this position leads the implementation of new routing and customer data management systems, participates in equipment purchase committees, and determines the staffing and resources needed to achieve desired operational goals.

MINIMUM QUALIFICATIONS

A typical way of gaining the knowledge, skills, and abilities for this position is equivalent to graduation from high school and five (5) years of increasingly responsible journey level experience in integrated solid waste collection, diversion and disposal which included two (2) years at the level equivalent to the Solid Waste Supervisor class.

THE POSITION:

The Facilities Carpenter I is an entry level classification in the carpentry series. Carpenters in this class are assigned routine skilled carpentry tasks. The Facilities Carpenter II is responsible for performing a variety of skilled carpentry and related tasks in the construction, remodeling, repair, and maintenance of City facilities. This the journey level classification within the carpenter series. Incumbents at this level act with independence, handle "master" craftworker projects, and assume the more complex activities which would otherwise require the attention and coordination of the Facilities and Building Manager.

The Human Services Department (HSD) delivers and supports services by forging long-term community partnerships, engaging with and building the capacity of the community to do its own problem-solving, and leveraging financial and volunteer resources. The Department’s nationally and internationally recognized programs support thousands of residents throughout their life course, from infants to elders. The department offers a working environment which is friendly, supportive, collegial, and where staff of many disciplines come together to problem solve, compassionately assist those in need, and share successes.

 

The Aging and Family Services Division (AFS) of HSD specifically supports older residents and their caregivers to live active, healthy lives in the Tri-City community. With input from local, older adults (age 55 years+) as our guide, the AFS team strives to provide and AFS staff work together to provide a continuum of supportive services that range from health promotion activities to home-based end of life care. AFS has a Senior InfoLine (phone and email) that is answered in 4 language (Farsi, English, Spanish and Chinese) that is often an entry point. As an older adult or their caregiver’s situation changes, they seamlessly have access to a continuum of programs that include counseling and emotional support; care coordination and resource assistance; specialized family caregiver support and education. Many of these supportive services are provided in the resident’s home. We also provide regular expert clinical consultation, supervision, opportunities for self-reflection, and team building, fostering a culture of empathy, inclusivity, humility, and respect.

 

The Position

 

Under the supervision of the Age Well Center Managers, the Chef/Food Services Manager position manages meal programs for the City of Fremont’s Age Well Centers. Chef responsibilities across both Age Well Center locations include the performance of menu planning, purchasing of foodstuffs, preparing quantity meals, supervising and assisting kitchen employees and volunteers and maintaining clean and orderly kitchens.

The incumbent will be responsible for properly cooked, economical, and wholesome, well balanced meals in City operated Age Well Centers. This position is not limited to being responsible for the assignment of work, supervision and evaluation of paid and volunteer food service workers. The review and approval of proposed menus, food purchases, control and monitoring of expenditure rates; and institution of disciplinary actions are functions reserved for the Program Coordinator or Age Well Center Managers.

The City of Foster City is looking for a Building Maintenance Worker I who is able to perform semi-skilled maintenance and repair work on municipal buildings, facilities, and equipment in the Building Maintenance Division. This position performs routine and standardized tasks and duties such as plumbing, carpentry, painting and electrical activities.

Education and Training Guidelines
Any combination equivalent to experience and education that could likely provide the required knowledge, skills and abilities. A typical way would be:

Education: Completion of the twelfth grade or the equivalent, supplemented by specialized training in maintenance, construction, or facilities maintenance.

Experience: One (1) year of full time experience in general maintenance, construction, or facilities maintenance.

Selection Process
Applications will be reviewed for relevant education, experience, training and other job related qualifications. Based upon a review of employment applications received, a limited number of the qualified applicants will be invited to participate in the examination process.

The process will consist of an Oral Panel Interview. An eligibility list will be established from those who pass the examination process with a score of 70% or higher. Current and future vacancies may be filled from this list. The list will remain in effect for at least one year unless exhausted sooner.

The department will invite the most qualified candidates from the eligibility list to a final department interview. The Building Maintenance Manager will make the hiring decision.

The Community Services team is seeking a summer intern for a 7-10-week assignment.  The goal of the internship is to allow a student to gain hands-on experience working alongside experienced youth development and recreation staff and assisting in promoting a safe, healthy environment within the camp setting by providing emotional support to the participants. 

 

Key duties

• Provides guidance, support, and assistance to staff throughout the week, which includes managing camper behavior issues, stress, and personal care needs

Work hours will be Monday – Friday, 8:30 am – 2:30 pm, 30 hours per week during the ten weeks of employment.  The pay rate will be $20 per hour.  All interns will be required to successfully complete a Department of Justice fingerprint clearance and undergo a TB test as a condition of employment.

The City of Berkeley is seeking a Senior Public Health Nurse who is passionate about working with community members and partners! In this role, you will take an equity-focused approach to supporting good health for families and individuals across the lifespan.

The Senior Public Health Nurse will provide health consultations and case management in a multidisciplinary team for families facing complex health challenges. In addition to providing consultations and trainings for community based organizations and providers, this position will also lead initiatives to enhance professional development for a team of nurses and support program planning for Public Health Nursing services.

 

MINIMUM QUALIFICATIONS

A typical way of gaining the knowledge, skills and abilities for this position is:
Equivalent to graduation from a four year college or university accredited by the National League for Nursing, with major course work in nursing, and two (2) years of public health nursing experience. 

Other requirements include:

• Possession of a valid California Registered Nurse License

• Possession of a valid California State Public Health Nursing Certificate

• Willing to work evenings or weekends

• Ability to travel to various locations within and outside the City of Berkeley to meet the program needs and to fulfill the job responsibilities. When driving on City business, must maintain a valid California driver's license and a satisfactory driving record.
