San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

 

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

 

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

 

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.
• Experienced with Behavioral Health and Recovery Services (BHRS), as well as other mental health community programs.
• Knowledgeable of the systems of care related to older adults, mental health, substance abuse, developmental delay, pediatrics, primary care, palliative care, chronic illnesses, dialysis, conservatorship, and probation and parole.
• Able to work independently with little supervision and demonstrates a high degree of initiative.
• Knowledgeable of housing resources, their requirements for eligibility, and other community services such as In-Home Supportive Services (IHSS), Multipurpose Senior Services Program (MSSP), Institute on Aging (IOA), Health Plan of San Mateo (HPSM), BHRS, and Bridges to Wellness, and more.
• Knowledgeable about County benefit and entitlement programs, such as Medi-Cal and ACE.
• Experienced and comfortable working with the following populations: multi-cultural, homeless, chronic diseases, end-of-life, mental health, domestic violence, parolees, and substance abuse disorders.
• Experienced working with a diverse population (race, ethnicity, age, ability, language, nationality, socioeconomic status, gender, religion, and sexual orientation).
• Able to accurately assess for signs of adult and child abuse and be a mandated reporter.
• Able to work in a politically charged environment under high degrees of stress and meet all deadlines.
• Able to develop effective and positive working relationships with community partners such as Aging and Adult Services, BHRS, and HPSM.
• An effective communicator to a broad range of stakeholders, including doctors, nurses, conservators, health advocates, families, and patients.
• Able to remain calm during crisis situations and perform emergency crisis counseling for families and patients as needed.
• Experienced documenting in an EMR.
• Able to communicate clearly and concisely.
• Spanish-speaking preferred.

 

Examples Of Duties

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.
• Maintain case files utilizing the electronic medical record.
• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.
• Document interactions with patients/clients in EMR.
• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

 

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

Job Description

Under direct supervision, the Development Strategy & Finance Sr. Analyst works within the Port’s Real Estate and Development Division leading analytical work related to the Port’s Public-Private Partnership Development Projects. Key tasks will include preparing complex financial analyses and reports, analyzing development pro formas and appraisals, coordinating the issuance and disbursement of bonds and associated record keeping documentation, supporting with development negotiations, and managing division-wide contracts, billings, and budgets.

The Sr. Analyst will support Project Managers with all aspects of Port Development projects. The role will receive support in learning and performing all tasks with the goal of independently managing specific areas over time. The Development team and this position are collaborative in nature, and the Sr. Analyst will work with both internal and external stakeholders including all Port Divisions, specifically Port Finance and the Port’s Resilience team, the Office of Economic and Workforce Development, the Office of Public Finance, and private development partners.

The essential functions of this position include:

• Analyzes Port-wide development opportunities to prioritize projects and determine value to support Port-wide Sea Level Rise resilience projects.
• Reviews project pro formas, appraisals, and other financial analyses to determine fiscal feasibility and economic value to the Port.
• Supports Port Development, Finance, and Office of Public Finance teams with tax district formation and bond issuances.
• Tracks ongoing Project finances to ensure projects stay on budget, public sources are sufficient for funding, and Port meets revenue expectations.

Qualifications

Education

Baccalaureate degree from an accredited college or university; AND

Experience

Three (3) years full-time equivalent experience performing professional-level analytical work. Qualifying professional-level analytical work includes analysis, development, administration, and reporting in major programs and functions of an organization in the areas of budgets, contracts, grants, policy, or other functional areas related to the duties of positions in the 1823 Class series; AND

License and Certification

The Facilities Carpenter I is an entry level classification in the carpentry series. Carpenters in this class are assigned routine skilled carpentry tasks. The Facilities Carpenter II is responsible for performing a variety of skilled carpentry and related tasks in the construction, remodeling, repair, and maintenance of City facilities. This the journey level classification within the carpenter series. Incumbents at this level act with independence, handle "master" craftworker projects, and assume the more complex activities which would otherwise require the attention and coordination of the Facilities and Building Manager.

The Human Services Department (HSD) delivers and supports services by forging long-term community partnerships, engaging with and building the capacity of the community to do its own problem-solving, and leveraging financial and volunteer resources. The Department’s nationally and internationally recognized programs support thousands of residents throughout their life course, from infants to elders. The Aging and Family Services Division (AFS) of HSD specifically supports older residents and their caregivers to live active, healthy lives in the Tri-City community. With input from local, older adults (age 55 years+) as our guide, the AFS team strives to provide and AFS staff work together to provide a continuum of supportive services that range from health promotion activities to home-based end of life care. AFS has a Senior InfoLine (phone and email) that is answered in 4 language (Farsi, English, Spanish and Chinese) that is often an entry point. As an older adult or their caregiver’s situation changes, they seamlessly have access to a continuum of programs that include counseling and emotional support; care coordination and resource assistance; specialized family caregiver support and education. Many of these supportive services are provided in the resident’s home. The Position

 

Under the supervision of the Age Well Center Managers, the Chef/Food Services Manager position manages meal programs for the City of Fremont’s Age Well Centers. Chef responsibilities across both Age Well Center locations include the performance of menu planning, purchasing of foodstuffs, preparing quantity meals, supervising and assisting kitchen employees and volunteers and maintaining clean and orderly kitchens.

The incumbent will be responsible for properly cooked, economical, and wholesome, well balanced meals in City operated Age Well Centers. This position is not limited to being responsible for the assignment of work, supervision and evaluation of paid and volunteer food service workers. The review and approval of proposed menus, food purchases, control and monitoring of expenditure rates; and institution of disciplinary actions are functions reserved for the Program Coordinator or Age Well Center Managers.

• Plan menus with consideration for the dietary needs of older adults, wholesomeness, quantity of food to be prepared and budgetary constraints.

The City of Foster City is looking for a Building Maintenance Worker I who is able to perform semi-skilled maintenance and repair work on municipal buildings, facilities, and equipment in the Building Maintenance Division. This position performs routine and standardized tasks and duties such as plumbing, carpentry, painting and electrical activities.

Experience: One (1) year of full time experience in general maintenance, construction, or facilities maintenance.

Selection Process
Applications will be reviewed for relevant education, experience, training and other job related qualifications. Based upon a review of employment applications received, a limited number of the qualified applicants will be invited to participate in the examination process.

The process will consist of an Oral Panel Interview. An eligibility list will be established from those who pass the examination process with a score of 70% or higher. Current and future vacancies may be filled from this list. The list will remain in effect for at least one year unless exhausted sooner.

The department will invite the most qualified candidates from the eligibility list to a final department interview. The Building Maintenance Manager will make the hiring decision.

Department Interview: May 2024 (exact date TBD)

Please note that this is a flexibly staffed position intended to be filled at the entry level Building Maintenance Worker I position. The higher level Building Maintenance Worker II position may be offered at management's discretion.

DEFINITION

Under limited direction, the Housing and Economic Development Manager directs and administers the division to which they are assigned. More specifically, the Housing and Economic Development and Housing Manager develops, coordinates, and implements the City’s housing and economic development programs and related activities; initiates key long range planning activities based on the City’s General Plan, Housing Element, Belmont Village Specific Plan, Economic Development Strategic Plan, and regional initiatives; provides housing and economic data to inform key policy decisionmakers; develops and implements marketing, branding and outreach activities for business engagement; and performs related duties as assigned. 

 

DISTINGUISHING CHARACTERISTICS 

The Housing and Economic Development Manager functions as manager over the Citywide area of practice to which they are assigned, i.e., housing/economic development activities and provides technical expertise to other City departments and related agencies. The Housing and Economic Development Manager is distinguished from the next lower classification of either Management Analyst I/II in that the former is a division level manager, with vast programmatic responsibilities, expected to function with limited supervision.

Role description

Under direction, the 2595 Senior EAP Counselor provides direction and supervision to counselors providing organizational wellness services and engaged in direct client counseling; provides clinical assessments, short-term counseling, and referral of City and County employees experiencing difficulties related to personal or work-related problems; conducts seminars, trainings, and consultations on issues related to personal and organizational wellness; prepares records and reports; and performs related duties as required.

Typical duties include, but are not limited to:

1. Participates in the selection of, trains and evaluates a team of journey level counselors.
2. Provides clinical assessment, short-term counseling, and referral of City and County employees experiencing difficulties related to personal or work-related problems, such as stress, depression, anxiety, substance abuse, emotional concerns or relationship issues; provides counseling to family members and significant others as appropriate.
3. Educates employees, managers, supervisors, and union personnel regarding employee assistance program service offerings, how to access services, early identification of personal and workplace problems, and referrals to the program; directs the development of programs, resources, and EAP initiatives.
4. Provides consultation to managers, supervisors, union personnel, human resources professionals, clinical specialists, and family members regarding specific issues of individual employees in conformance with program and confidentiality policies; recommends specific methods or techniques for resolving performance problems, including appropriate managerial and supervisory interventions.
5. Directs development of, creates, and facilitates seminars and trainings for employees and management on personal, interpersonal and organizational issues such as the effective management of stress, anger and emotions, workplace negativity and dealing with difficult people.
6. Provides crisis intervention, trauma response, and mediation/conflict resolution services as necessary.
7. Leads evaluation efforts to determine the effectiveness of the overall program and specific services provided and prepares the reports.

Under general supervision, the Neighborhood Preservation Inspector. ensures compliance with applicable sections of the State, Federal, and Municipal Codes, ordinances, and in relevant areas, such as property maintenance, abandoned vehicles, planning, zoning, signs, community nuisance, and related areas.  This is a classification in the Economic and Community Development Department.

 

CLASS CHARACTERISITICS:

The Neighborhood Preservation Inspector is the full journey-level classification.  This position requires experience in both Municipal and Zoning Codes and Regulations. Employees at this level are expected to work with property owners independently and to coordinate City resources to solve neighborhood issues.
