San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.

• Knowledgeable about County benefit and entitlement programs, such as Medi-Cal and ACE.
• Experienced and comfortable working with the following populations: multi-cultural, homeless, chronic diseases, end-of-life, mental health, domestic violence, parolees, and substance abuse disorders.
• Experienced working with a diverse population (race, ethnicity, age, ability, language, nationality, socioeconomic status, gender, religion, and sexual orientation).
• Able to accurately assess for signs of adult and child abuse and be a mandated reporter.
• Able to work in a politically charged environment under high degrees of stress and meet all deadlines.
• Able to develop effective and positive working relationships with community partners such as Aging and Adult Services, BHRS, and HPSM.
• An effective communicator to a broad range of stakeholders, including doctors, nurses, conservators, health advocates, families, and patients.
• Able to remain calm during crisis situations and perform emergency crisis counseling for families and patients as needed.
• Experienced documenting in an EMR.
• Able to communicate clearly and concisely.
• Spanish-speaking preferred.

Examples Of Duties

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.
• Maintain case files utilizing the electronic medical record.
• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.
• Document interactions with patients/clients in EMR.
• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

This position drives a Readi-Ride General Public Paratransit Vehicle (GPPV) on a demand responsive curb-to-curb transit system and assist patrons boarding and leaving the bus, especially the elderly, disabled, and school age children. San Mateo Medical Center (SMMC), part of San Mateo County Health, is seeking a qualified and experienced Diabetes Nurse Educator for its Innovative Care Clinic's (ICC), Diabetes Self-Management Education. The Innovative Care Clinic (ICC) is a fully functional adult care clinic providing an interdisciplinary and integrated model of care emphasizing collaboration across professional roles.  The Diabetes Nurse Educator will also serve the Ron Robinson Senior Care Center, Coastside, and Mental Health Primary Care clinics.

The Diabetes Nurse Educator functions as a patient educator collaborating with patients with a diagnosis of diabetes to educate them on areas such as weight management, food intake, medications and the proper usage of a glucometer.  The goal is to assist patients with high A1C to lower their numbers to be within normal range.  The Diabetes Nurse Educator will follow the American Diabetes Association guidelines for Diabetes Self-Management Education (DSME).  The educator is responsible for submitting regular data to the ADA to retain SMMC's certification.  The educator will lead a team of diabetes educators across the SMMC system by conducting regular meetings to discuss any updates on DSME.

The ideal candidate has at least one year of experience in ambulatory care setting addressing chronic disease management; is skilled in patient diabetes self-management education, triage, and assessment; has worked with the underserved population and on interdisciplinary teams. The ideal candidate is also a critical thinker, fast learner and thrives in a dynamic environment.  Computer literacy, prior experience with electronic medical records (E-Clinical Works and/or EPIC preferred).  Experience in data tracking/management is highly desirable.  Bilingual in Spanish and English is preferred.

• Certified Diabetic Educator Certification (preferred but not required) 
• Proficiency in using Excel in order to track key data for ongoing certification with the American Diabetes Association.

DISTINGUISHING CHARACTERISTICS

Street Maintenance Operator I: This is the entry level class in the Street Maintenance Operator series. Employees at this level are not expected to perform with the same independence of direction and judgment on matters related to established procedures and guidelines as are positions allocated to the II level. Since this class is typically used as a training class, employees may have only limited or non directly related work experience. Employees work under immediate supervision while learning job tasks.

Street Maintenance Operator II: This is the full journey level class within the Street Maintenance Operator series. This class is distinguished from the Street Maintenance Operator I by the assignment of the full range of duties assigned. Employees at this level receive only occasional instruction or assistance as new or unusual situations arise, and are fully aware of the operating procedures and policies within the work unit. Positions in this class are flexibly staffed and are normally filled by advancement from the I level.

Glenn County is recruiting individuals to fill three full time and multiple Part Time/ Extra Help positions in the Public Works Agency. The Aging and Adult Services Division of San Mateo County Health is seeking a full-time Deputy Public Guardian Conservator II/III (DPG). Deputy Public Guardians manage the income and assets and coordinate the care, including the arrangement of placement and treatment, of elderly and gravely disabled persons under the jurisdiction of the Public Guardian as appointed by the Superior Court.  Deputy Public Guardians visit clients on a regular basis, which includes travel to various locations both within and outside of San Mateo County.
 
The current vacancy is in the LPS (Lanterman Petris Short Act) conservatorship unit; however, this recruitment may be used to fill vacancies in either Probate or the LPS conservatorship unit.
 
DPG II is at the journey level with responsibility for managing estates and providing casework services. DPG II administers complex property and litigation cases and may develop comprehensive care plans. DPG III also provides lead direction to other deputies, serves as liaison with specific County and State facilities, handles those cases that present the greatest liability to the client and/or the County, and may provide 24-hour emergency response services. All DPGs must accept very diverse roles in the areas of estate management and care coordination.
 
This person will report to the Supervising Deputy Public Guardian of Aging and Adult Services.
 
The ideal candidate will be able to develop and maintain collaborative partnerships with a variety of organizations; have strong assessment, motivational interviewing, investigative, oral and written communication skills; have familiarity working with Adult Protective Services (APS), law enforcement, San Mateo County Superior Court system, Behavioral Health and Recovery Services, San Mateo Medical Center and an understanding of the Probate Code. The ideal candidate should also have experience conducting psychosocial assessments and meeting the needs of persons suffering from serious mental illness, as well as experience with asset management and knowledge of various public benefits programs. The Deputy Public Guardian may have to facilitate application for a reverse mortgage, identify and marshal Trust and conservatorship assets, and reconcile these two separate legal entities, as well as develop an estate plan, while ensuring that the client receives care appropriate to his/her needs.
 
 Client Care Coordination:
  

• Arranges for the person's care and protection.
• Decides where the person will live.
• Makes arrangements for health care, meals, clothing, personal care, housekeeping, transportation, and recreation.
• Makes placement decisions that allows for clients to be at the least restrictive placement while ensuring the safety of the public.
• Represents or arranges representation in all legal matters.
• Performs de-escalation and motivational interviewing interventions on an as needed basis.
• Collaborates with the case managers, psychiatrists, social workers, and facility administrators to put together a plan to address emergent and/or behavioral issues and find alternative placement options, as needed.

For example, a client in a board and care home or locked facility may be having a behavioral problem. The Deputy Public Guardian will work with the case manager, psychiatrist, social worker, and facility administrator to put together a plan to address the client's behavioral issues and may also need to work with the treatment team on finding alternative placement, if needed.
 
 Court:

• Arranges appropriate representation in all legal matters.
• Carries out legal and regulation requirements pertaining to the Lanterman-Petris-Short Act (LPS).
• Period reviews of LPS reports to ensure quality assurance/performance.
• Prepares and submits comprehensive reports to the Probate Court providing updates and/or addressing financial, social, placement and medical matters.

Lanterman-Petris-Short Act (LPS) requires the review of client's current needs based on documentation of visits and care conferences, as well as annual report writing regarding that information. Probate requires a review of the past two years of care through the writing of a comprehensive report on financial transactions, social interactions, and medical conditions.

The Human Resources Department is seeking well qualified candidates for the position of HRIM /Workday Application Support Analyst II. We are seeking highly skilled and motivated individuals to join our Human Resources Information Management /Workday team.

As the HRIM /Workday Application Support Analyst II, you will collaborate and consult with colleagues in various departments to understand business and functional drivers across all Workday HR modules. Your responsibilities will include hands-on configuration of Workday features, security, development of reports and dashboards, and provision of sound solutions to support end-to-end business processes. Additionally, you will assist with Workday tenant management, deliver exceptional customer support, and contribute to system testing and issue resolution.

Once job duties are learned, this position has the option of hybrid work, two or three days in the office and two days of remote work.

The base hourly salary range for this position is $56.47 - $70.58. Because the position is classified as "confidential" there is a 5% allowance added to the salary. This allowance has been added into the advertised salary range.

Examples Of Duties

Duties and responsibilities may include, but are not limited to the following:

• Collaborate and consult with HR, Controller's Office, and other County departmental partners to understand business and functional drivers across all Workday HR modules.
• Configure Workday calculated fields, business processes, domain and business process security policies, and role assignments.
• Develop Workday reports and dashboards.
• Recommend efficient security and organization solutions to support end-to-end business processes.
• Assist with Workday tenant management.
• Provide the highest level of customer support through internal ticketing system and other communication channels.
• Support system functionality testing and issue resolution and develop proof of concepts based on business requirements.
• Build relationships with other Workday domain experts and critical users and collaborate with internal teams and vendors for process alignment.
• Assist in granting security access to Workday, following security protocols.
• Act as an escalation point for unresolved security or business process issues as directed.
• Assist with compliance reporting and system monitoring.
• Help manage Workday releases, identifying impact and enhancement opportunities, and communicate with functional owners and application leads.
• Support regression testing and introduction of required changes for new Workday releases.
• Continuously monitor Workday community for updates and valuable changes.
• Demonstrate effective interpersonal, written, and verbal communication skills to foster productive work relationships.
• Configure Workday security, including security groups, roles and permissions, user access, and implementation of security best practices.
• Perform other related duties as assigned.

Qualifications

Experience/Education:

• 3-5 years of proven Workday expertise in designing, implementing, and supporting business process and security solutions.
• Workday Pro certification in a relevant area is preferred but not required.
• Bachelor's Degree or higher in Information Technology or an equivalent field of study, is preferred.

Knowledge, Skills, and Abilities:

• Proven experience leading multiple end-to-end Workday projects and implementing solutions for operational improvements and automation.
• Knowledge of basic and advanced security configuration including Workday delivered and custom based security roles based on the organization.
• Thorough knowledge of various security group types (role based, user based, segment based, etc.) and how they are applied in the system.
• Thorough understanding of Workday business process configurations.
• Ability to build and modify custom Workday reports and calculated fields.
• Experience in creating, loading, and troubleshooting EIBs.
• Strong interpersonal and communication skills, both verbal and written.
• Attention to detail and ability to handle multiple projects and shifting priorities effectively.
• Demonstrates confidentiality and professionalism in handling sensitive matters.
• Thrives in a demanding and fast-paced environment, both independently and as part of a team.
• Displays discretion and exercises independent judgment.
• Excellent analytical, troubleshooting, organizational, and planning skills.
• Flexibility to adjust hours and accommodate business needs.
• Experience in BIRT and Workday Integrations including Workday Studio.
