THE POSITION

THE POSITION

Tri-City is looking for a special individual to serve as a Clinical Therapist I/II for the Children’s Full Service Partnership/Children and Transitional Age Youth (FSP/ C TAY). THE JOB

COMPENSATION AND BENEFITS:

The City of Hayward is excited to announce its recruitment for the position of Mobile Mental Health Clinician. Under general direction, the Mobile Mental Health Clinician provides field and office based professional behavioral health counseling, psychoeducation, outreach and engagement, and case management services as part of the City’s Hayward Evaluation and Response Teams (HEART) Program improving public safety responses to people experiencing mental illness, substance use disorders and homelessness.

THE PROGRAM

The Hayward Evaluation and Response Teams (HEART) Program is a cross departmental program between the Hayward City Manager’s Office, Police, and Fire Departments and Alameda County Behavioral Health Care Services developed to improve access to medical, mental health, and other support services.  The HEART program aims to achieve two main goals: 1) to reduce the need for police officers to intervene in calls for service involving people experiencing chronic mental illness, substance use disorders, and homelessness and 2) to reduce reliance on emergency services for individuals needing long-term case management and follow-up services.  HEART consists of three distinct response teams: the Mobile Integrated Health Unit (MIHU) made up of Hayward Fire Department Community Paramedics; the Hayward Mobile Evaluation Team (HMET) which pairs a specially trained Hayward Police Officer with an Alameda County behavioral health clinician; and the Case Management and Mental Health Linkages Team (LINK) consisting of Mobile Mental Health Clinicians and a Lead Program Assistant (Case Manager).

THE IDEAL CANDIDATE

 As a member of the LINK team, the ideal candidate has experience providing short term crisis intervention, safety assessments, and other mental health services to adults, youth and children experiencing mental illness, substance use disorders and/or homelessness.  They have the ability to work primarily in the field and have the situational awareness to respond alongside emergency response teams. They are knowledgeable about local community resources and skilled at forming relationships quickly.  
  

Essential Duties

What you will do:

Provides direct casework, individual and group therapy; secures and evaluates medical or psychosocial data such as behavior patterns, use of coping skills and family relationships; analyzes factors which exert an adverse effect upon health issues, treatment and recovery which contribute to or are responsible for behavioral health issues and/or affect the potential for recovery and resiliency. 

Performs field work to proactively serve the unhoused population, persons with mental illness and others in crisis.
 
 Interprets the social aspects of physical or behavioral health issues to clients, relatives, other behavioral health and medical care staff and appropriate persons or agencies.

Establishes and maintains effective relationships with contractors and community organizations and other governmental agencies regarding programs and services; participates in committees, meetings and conferences; provides guidance regarding appropriate social-psychological practices and services.

Identifies emerging trends related to crimes and issues; develops and implements prevention and intervention program approaches; provides community education on current procedures and practices related to mental health and intervention strategies.

Education:  A Master’s degree from an accredited college or university with major course work in social work, psychology, counseling or a related field.

License & Certificates:  Possession of a valid professional mental health therapy license issued by the State of California, such as Licensed Clinical Psychologist, Licensed Clinical Social Worker (LCSW), Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), Associate Social Worker License (ASW), Intern Marriage Family Therapist (IMFT) or Clinical Psychologist or a closely related field is highly desirable.

Possess and maintain a valid California Class C Driver's License.

The Human Services Department (HSD) delivers and supports services by forging long-term community partnerships, engaging with and building the capacity of the community to do its own problem-solving, and leveraging financial and volunteer resources. The Department’s nationally and internationally recognized programs support thousands of residents throughout their life course, from infants to elders. The department offers a working environment which is friendly, supportive, collegial, and where staff of many disciplines come together to problem solve, compassionately assist those in need, and share successes.

The Aging and Family Services Division (AFS) of HSD specifically supports older residents and their caregivers to live active, healthy lives in the Tri-City community. With input from local, older adults (age 55 years+) as our guide, the AFS team strives to provide and AFS staff work together to provide a continuum of supportive services that range from health promotion activities to home-based end of life care. AFS has a Senior InfoLine (phone and email) that is answered in 4 language (Farsi, English, Spanish and Chinese) that is often an entry point. As an older adult or their caregiver’s situation changes, they seamlessly have access to a continuum of programs that include counseling and emotional support; care coordination and resource assistance; specialized family caregiver support and education. Many of these supportive services are provided in the resident’s home. We also provide regular expert clinical consultation, supervision, opportunities for self-reflection, and team building, fostering a culture of empathy, inclusivity, humility, and respect.

The Position

Under the supervision of the Age Well Center Managers, the Chef/Food Services Manager position manages meal programs for the City of Fremont’s Age Well Centers. Chef responsibilities across both Age Well Center locations include the performance of menu planning, purchasing of foodstuffs, preparing quantity meals, supervising and assisting kitchen employees and volunteers and maintaining clean and orderly kitchens.

The incumbent will be responsible for properly cooked, economical, and wholesome, well balanced meals in City operated Age Well Centers. This position is not limited to being responsible for the assignment of work, supervision and evaluation of paid and volunteer food service workers. The review and approval of proposed menus, food purchases, control and monitoring of expenditure rates; and institution of disciplinary actions are functions reserved for the Program Coordinator or Age Well Center Managers.

Examples of Duties

• Plan menus with consideration for the dietary needs of older adults, wholesomeness, quantity of food to be prepared and budgetary constraints.
• Estimate food requirements and requisitions for food and supplies.
• Prepare, cook, and serve a wide variety of foodstuffs for quantity food service.
• May cut/carve and cook meats; bake breads, cakes and pastries; peel and prepare vegetables; and make salads.
• Place food in serving containers and serves food.
• Maintain proper temperature, rotation and storage of food.
• Direct and assist in cleaning kitchens and kitchen equipment after each meal.

Minimum Qualifications:

EXPERIENCE AND EDUCATION:

Any combination equivalent to experience and education that could likely provide the required knowledge and abilities would be qualifying. A typical way to obtain the knowledge and abilities would be:

Experience: Five (5) years of increasingly responsible Human Resources management experience including assignments in the areas of recruitment, selection, classification, training, compensation, employee and labor relations, worker's compensation and safety administration, and employee benefits or other pertinent job-related areas, including two (2) years of supervisory or lead experience.

Education: Equivalent to a Bachelor's Degree from an accredited college or university with major course work in human resources, business, or public administration or closely related field.

ESSENTIAL JOB DUTIES

Facility duties may include, but are not limited to:

• Unlock and lock buildings for renters and programs.
• Be present and assist renters during events.
• Set up and breakdown tables and chairs for rentals and program classes.
• Maintain and keep facilities clean during and after events.
• Move furniture/equipment and return to appropriate storage areas after events.
• Assist renters with facility equipment usage.
• Clean sinks, counters, mirrors, stalls, floors, restrooms, and garbage disposal before and after rentals and program classes to ensure facilities are clean inside and outside.
• Performs duties within OSHA standards.

Works under general supervision of the Facilities Supervisor.

The positions are responsible for a variety of ongoing responsibilities including, but not limited to:

• Provide individual, family, group therapy for adults, children, adolescents, and youth with mental illness and their families; facilitate child and family team meetings;

• Within scope of authority, assesses, diagnoses, and treats a variety of clients with mental health disorders; collaborates with clients and creates individualized treatments plans according to client’s needs and circumstances.

• Provide home, field, office and/or school services; complete all DMH documentation; participate in the on-call rotation;

• Perform routine and emergency intake evaluations according to Authority procedure; completes appropriate forms and documents relating to the intake process; and prepares medical and demographic case history for use in diagnosis.

• Assists clients and family members in understanding behavioral health diagnosis, contributing factors/causes, effects, prognosis, rehabilitation, and treatment.
