The Position

As a member of the HSD assigned to the AFS, the Senior Program Coordinator will organize, coordinate, and implement community wellness programs for older adults. The Community Programs include health promotion activities such as health screenings, wellness classes, and educational activities. The position works within a team that includes volunteers, health promoters, nurses, social workers and mental health workers. The position facilitates community awareness and partnerships, with individuals and agencies, to promote an age-friendly environment.

This position performs a variety of professional administrative and confidential support functions for the Assistant General Manager and his/her staff. ESSENTIAL DUTIES AND RESPONSIBILITIES

Coordinates manager's schedule/calendar and makes appointments. Greets scheduled visitors and conducts to appropriate area or person. Arrange and coordinate travel schedules and reservations as needed.

Coordinates and arranges meetings, prepares agendas, reserves and prepares facilities, and records and transcribes minutes of meetings.

Organizes and maintains file system and files correspondence and other records.

Conducts research and compiles and types statistical reports.

Administers committee meetings in accordance with Ralph M. Brown Act requirements.

Makes copies of correspondence or other printed materials.

Prepares outgoing mail and correspondence, including e-mail and faxes.

Within budget, orders and maintains supplies and arranges for equipment maintenance.

Provides support or backup to the Receptionist, other Administrative Assistants, and Executive Assistant as needed. Participates in Administrative Assistants meetings. 

Assists in coordinating the monthly Agenda Packet materials for the Commission.

Maintains the business unit SharePoint workspace and associated systems, including business web page.

Develops, maintains, and administers a regulatory tracking system to support agency regulatory requirements.

Develops, prepares, and administers Agency contracts and agreements, including the development of contract templates, tracking of contract records in the Agency records-keeping system, and processing various contractual requirements.

Assist with the development and preparation of Agency staff reports and resolutions, including supporting documents.

Provide support for Agency contract administration functions, including tracking, storage, review, and validation of agency contracts and agreements.

May prepare or review expense reports, check requests, and purchase requisitions.

May perform other duties as needed.

MINIMUM QUALIFICATIONS
Education and/or Experience
Associate's degree (A. A.) or equivalent from two-year college or technical school; and a minimum of three years related administrative experience supporting a high-level manager and his/her staff; or equivalent combination of education and experience. 

KNOWLEDGE AND ABILITIES 
Knowledge:
Knowledge of modern office methods, supplies and equipment; business English and correspondence, principles of effective training; Ability to work effectively and collaborate cross-functionally with multiple internal groups; Strong organizational and time-management skills; Fluency with Microsoft Word, Excel, PowerPoint, database activities, SharePoint, etc.

Qualifications

Knowledge of:

• English language, spelling, grammar, and punctuation.
• Customer service and interpersonal communications principles.
• Computer operation and use in office and Communications Center environments.
• Basic principles in communicating by means of face-to-face contact, telephone, and/or data terminals.

The CritiCall exam is a computer based Public Safety Dispatcher Pre-Employment Skills test. The wide array of tests within the CritiCall exam provides a comprehensive inventory of a candidate's skills and abilities.  Achieving a passing score on the exam is a requirement for employment.  

The examination simulates some of the duties and situations a 911 Communications Dispatcher might encounter on the job.  No specialized knowledge or skills are required to take the exam. 

The CritiCall exam tests for skills and abilities in areas such as decision-making, data entry, call summarization, multi-tasking, cross-referencing, vocalization, character comparison, memory recall, prioritization, mathematics, probability, map reading, reading comprehension, spelling, and sentence clarity.
 
Once the recruitment closes and all applications have been screened, applicants whose applications pass the screening will be sent a link to take the CritiCall exam.  Applicants will have up to ten days to complete the exam.  Complete instructions will be sent in the correspondence regarding registering for and taking the exam.  The exam can be taken at the convenience of the test taker, during the ten specified days.  

Candidates who receive a passing score on the CritiCall exam will be placed on the Hiring List (Eligible List) in the order of the scores from the exam.  The top seven candidates from the Hiring List will be considered for one vacancy.  One additional candidate will be included for each additional vacancy.

San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.
• Experienced with Behavioral Health and Recovery Services (BHRS), as well as other mental health community programs.

• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.

• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.
• Document interactions with patients/clients in EMR.
• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

What You’ll Do

The Assistant to the City Clerk position serves as the welcoming and friendly face of government for both the City Council and the City Clerk’s office as this position is the first contact for the front counter and the primary receptionist. The position requires an ability to help demystify government services and processes, learn and maintain an understanding of all government services and procedures and help citizens get to the right service. The position’s administrative functions include responsibility for records management including filing and scanning; managing off-site records inventory; managing and ordering office supply inventory; writing correspondence, creating certificates of commendation and writing proclamations on behalf of the Mayor. This position is also charged with maintaining multiple calendars, processing reservations and invoices and acting as primary administrative support for Council and the City Clerk. Excellent attention to detail, writing and proofreading skills are required and a positive upbeat attitude with the ability to be flexible, resilient, and creative when situations change is essential. The successful candidate will have a strong work ethic, a passion for making a difference and helping citizens, and a desire to grow in this field.

Some of the duties may include, but are not limited to, the following:

• Attend meetings as required, may prepare meeting minutes, may assist in City Council agenda preparation and follow-up, as needed; manage Council reservations, correspondence and calendars
• Manage, implement, and monitor the Clerk’s records management programs, including document imaging, the receipt, storage, retrieval and disposition of official City records; work with records representatives from other departments and train them in the process
• Assist with coordinating the bi-annual City-wide Records Clean-up Event; maintain comprehensive indexing and filing system of City Council actions, including resolutions, ordinances, deeds, contracts, and agreements; provide research to staff and the public on information needs, as needed
• Provide information and answer inquiries from officials, the public, and City staff on laws, procedures, and policies, such as the City Municipal Code, the Brown Act, and the Public Records Act; organize and assist with the filing of Statements of Economic Interests and Campaign Statements for candidates, office holders and City employees; record and file deeds and other documents requiring recordation; maintain and monitor agreements and insurance certificates; perform special projects as needed; and ensure the provision of official notification of public hearings, including legal notices in accordance with City, State, and Federal laws.
• Apply applicable policies and procedures in determining completeness of applications, records and reports
• Ensure that meeting facilities are properly prepared
• Maintain logs and manage Council and Clerk correspondence and responsiveness from other departments; receive, sort and distribute incoming and outgoing correspondence
• Assist Deputy with the preparation and distribution of notices, action reports, letters, and other correspondence as required by action taken in the meetings
• Act as receptionist and primary responsibility for staffing the front counter; answer the telephone and assist the public and internal customers by responding to requests for information; issue, receive, process various applications
• Order office supplies, manage supply inventory; submit expense claims, pay bills as needed
• Perform general clerical work including filing, scheduling appointments, and processing personnel, payroll and purchasing information
• May maintain petty cash fund; accept payment of fees; maintain and process cash records

The Assistant to the City Clerk receives general supervision from the Deputy City Clerk or a higher-level position and may exercise direct or functional and technical supervision over lower-level clerical positions, volunteers, and interns.
