THE DEPARTMENT

The Parks and Recreation Department is the third largest department in the City and is organized by program area, with each program administered by a supervisor, coordinator, or specialist. The Parks and Recreation Department is composed of a Director and 22 full-time management positions. The department is comprised of the Recreation, Parks, and Building Maintenance divisions. The Parks and Recreation Department mission is to provide opportunities for physical, cultural and social well being; protect and enhance the physical environment; and ensure the effective and efficient use of public facilities and open space.

The Building Maintenance Division maintains 32 facilities totaling more than 563,000 square feet and over 275 acres of park land. The division provides janitorial, preventative maintenance services, and necessary building and related equipment repairs to all of the City's facilities, including interior and exterior work. The division also performs a variety of tasks, which includes moving furniture, set-ups, and many other miscellaneous tasks as required.

The Behavioral Health Advocate I position provides advocacy, guidance, feedback, outreach and support to help families to access and work more effectively with the systems involved.   These positions report to a Program Supervisor.

This position will perform the following essential duties, including, but not limited to:

• Provides peer to peer support services in the office and/or in the field including, but not limited
  to outreach, support, encouragement, guidance, advocacy, and referrals to external resources
  and services for clients and their families; builds rapport with clients and their families through
  sharing stories of lived experience and recovery.

• Encourages and supports clients to fully engage in services; provides outreach and assists
  clients in addressing and overcoming barriers to treatment.

• Prepares and facilitates a variety of wellness groups and group activities; monitors groups to ensure compliance with group rules and encourages client participation.

• Assists clients and families to understand behavioral health problems and steps to recovery.

• Serves as a role model to build and sustain connections and relationships with clients and the
  community.

• Accompanies clients to meetings and appointments to support their self-advocacy and
  facilitate better communication with service providers.

• Perform other duties as assigned.

The current vacancy is for a part-time (20 hours per week) position is the Youth and Family Services (YFS) Division with prorated benefits working with children, youth, and their families.

The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

- Manage a caseload of 28-32 clients comprised of older individuals with complex medical conditions.
- Conduct comprehensive client assessments, and develop and monitor service plan.
- Collaborate with team members, Human Service programs, and community agencies.
- Conduct home visits and provide other community interventions as needed.
- Conduct inter-agency and/or family conferences.
- Advocate for clients in accessing services.

Role Description

As a Clinical Psychologist for the County Adult Assistance Program(CAAP), you will conduct professional-level work in screening and assessing CalWORKS and CAAP clients to identify and document disabling mental health conditions and associated functional limitations that enable them to qualify for Social Security Benefits.

Essential Duties Include:

• Conducts screening and assessment of clients with mental health related complaints, collateral interviews with primary care and behavioral health staff, and medical records review to support clients’ application for Social Security Benefits.
• Selects, administers, scores, and interprets appropriate psychological tests of intelligence, personality, aptitude, achievement, interest and other specialized areas and conducts diagnostic clinical interviews with emotionally disturbed, mentally ill, chemically dependent, and intellectually disabled adults to assess their impairments to provide medical evidence in support of clients’ application for Social Security Benefits as needed.
• Prepares psychological reports presenting diagnostic findings, interview findings, functional assessments, clinical evaluations, and recommendations for the Clients’ SSI eligibility.
• Reviews and evaluates psychological, psychiatric and/or neuropsychological data and case material.
• Provides education, consultation, and information to community providers, and CAAP’s multi-disciplinary team on adult adjustment problems, mental illness, chemical dependence, culturally relevant services, and other topics related to SSI eligibility.
• Supervises psychology interns and subordinate clinical staff not including class 2574 Clinical Psychologist; conducts staff training programs both within the clinic or unit and for line agency personnel.

THE DEPARTMENT

The South San Francisco Fire Department provides its residents with service in fire suppression; first-responder and ambulance transport emergency medical services; hazardous materials response; urban search and rescue; marine rescue and fire prevention, investigation and public education. The Department operates five (5) fire stations throughout the City, plus an Emergency Operations Center and Fire Training Tower and is comprised of 92 dedicated staff members.
