The City of Sunnyvale is recruiting for one Senior Park Utility Worker: Tree Maintenance in the Department of Public Works.  This position will lead a small crew (2 to 4 person crew), where the incumbent performs the work duties assigned as well as directs and oversees the work of the crew members.

Under general supervision, leads and performs journey-level landscape maintenance tasks and does related work, as required.

The Department

The Human Services Department (HSD) delivers and supports services by forging long-term community partnerships; engaging with and building the capacity of the community to do its own problem- solving; and leveraging financial and volunteer resources.  The Aging and Family Services Division (AFS) of HSD specifically supports older residents and their caregivers to live active, healthy lives in the Tri-City community.

The Position

As a member of the HSD assigned to the AFS, the Senior Program Coordinator will organize, coordinate, and implement community wellness programs for older adults. The Community Programs include health promotion activities such as health screenings, wellness classes, and educational activities. The position works within a team that includes volunteers, health promoters, nurses, social workers and mental health workers. San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.

Examples Of Duties

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.
• Maintain case files utilizing the electronic medical record.
• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.
• Document interactions with patients/clients in EMR.
• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

THE POSITION: 

The City of Hayward is excited to announce its recruitment for the position of Mobile Mental Health Clinician. Under general direction, the Mobile Mental Health Clinician provides field and office based professional behavioral health counseling, psychoeducation, outreach and engagement, and case management services as part of the City’s Hayward Evaluation and Response Teams (HEART) Program improving public safety responses to people experiencing mental illness, substance use disorders and homelessness.

THE PROGRAM

The Hayward Evaluation and Response Teams (HEART) Program is a cross departmental program between the Hayward City Manager’s Office, Police, and Fire Departments and Alameda County Behavioral Health Care Services developed to improve access to medical, mental health, and other support services.  The HEART program aims to achieve two main goals: 1) to reduce the need for police officers to intervene in calls for service involving people experiencing chronic mental illness, substance use disorders, and homelessness and 2) to reduce reliance on emergency services for individuals needing long-term case management and follow-up services.  HEART consists of three distinct response teams: the Mobile Integrated Health Unit (MIHU) made up of Hayward Fire Department Community Paramedics; the Hayward Mobile Evaluation Team (HMET) which pairs a specially trained Hayward Police Officer with an Alameda County behavioral health clinician; and the Case Management and Mental Health Linkages Team (LINK) consisting of Mobile Mental Health Clinicians and a Lead Program Assistant (Case Manager).

THE IDEAL CANDIDATE

 As a member of the LINK team, the ideal candidate has experience providing short term crisis intervention, safety assessments, and other mental health services to adults, youth and children experiencing mental illness, substance use disorders and/or homelessness.  They have the ability to work primarily in the field and have the situational awareness to respond alongside emergency response teams. They are knowledgeable about local community resources and skilled at forming relationships quickly.

San Mateo County Health's Behavioral Health and Recovery Services Division is seeking a responsible, innovative leader and skilled clinician for the position of Supervising Mental Health Clinician - East Palo Alto Services. This Supervising Mental Health Clinician will supervise and coordinate the work of mental health professionals and interns in the East Palo Alto and East Menlo Park geographic area in San Mateo County. This position works closely with community members and community providers to develop and sustain programming that is culturally and community responsive.

This position will participate in program administration, planning and evaluation. The Supervising Mental Health Clinician supervises and guides the clinical team who provides services to seriously emotionally disturbed youth and their families, and seriously mentally ill adults. Services provided include: medication assessment and management; individual, group and family therapy; psychological assessment; case management; rehabilitation services; and crisis intervention.

The current vacancy is in the East Palo Alto Community Counseling Center, located in East Palo Alto but will travel throughout San Mateo County for client visits, trainings and meetings.

The East Palo Alto Community Counseling Center offers a unique setting to provide services to a diverse population in a community-oriented clinic.

This position supervises a multidisciplinary team that provides clinical outpatient services to adults and children and their families. The team may include psychiatrists, psychologists, psychiatric nurses and social workers and marriage and family therapists. The Department’s nationally and internationally recognized programs support thousands of residents throughout their life course, from infants to elders. The Aging and Family Services Division (AFS) of HSD specifically supports older residents and their caregivers to live active, healthy lives in the Tri-City community. With input from local, older adults (age 55 years+) as our guide, the AFS team strives to provide and AFS staff work together to provide a continuum of supportive services that range from health promotion activities to home-based end of life care. AFS has a Senior InfoLine (phone and email) that is answered in 4 language (Farsi, English, Spanish and Chinese) that is often an entry point. As an older adult or their caregiver’s situation changes, they seamlessly have access to a continuum of programs that include counseling and emotional support; care coordination and resource assistance; specialized family caregiver support and education. Many of these supportive services are provided in the resident’s home. We also provide regular expert clinical consultation, supervision, opportunities for self-reflection, and team building, fostering a culture of empathy, inclusivity, humility, and respect.

The Position

Under the supervision of the Age Well Center Managers, the Chef/Food Services Manager position manages meal programs for the City of Fremont’s Age Well Centers. Chef responsibilities across both Age Well Center locations include the performance of menu planning, purchasing of foodstuffs, preparing quantity meals, supervising and assisting kitchen employees and volunteers and maintaining clean and orderly kitchens.

The incumbent will be responsible for properly cooked, economical, and wholesome, well balanced meals in City operated Age Well Centers. This position is not limited to being responsible for the assignment of work, supervision and evaluation of paid and volunteer food service workers. The review and approval of proposed menus, food purchases, control and monitoring of expenditure rates; and institution of disciplinary actions are functions reserved for the Program Coordinator or Age Well Center Managers.

Examples of Duties

• Plan menus with consideration for the dietary needs of older adults, wholesomeness, quantity of food to be prepared and budgetary constraints.
• Estimate food requirements and requisitions for food and supplies.
• Prepare, cook, and serve a wide variety of foodstuffs for quantity food service.
• May cut/carve and cook meats; bake breads, cakes and pastries; peel and prepare vegetables; and make salads.
• Place food in serving containers and serves food.
• Maintain proper temperature, rotation and storage of food.
• Direct and assist in cleaning kitchens and kitchen equipment after each meal.
• Estimate food requirements and requisitions for food and supplies.
• Direct and assist in the cleaning of the kitchen and kitchen equipment after each meal.
• Oversee that the kitchen and equipment are kept clean, sanitary and in proper working condition. Schedule regular maintenance of kitchen equipment as needed. Maintain standards of safety and sanitation in kitchen area.
• Recruit, train, supervise, instruct, and direct employees and volunteers assigned to working in the kitchens.
• Develop and support additional meal programs, including, but not limited to fundraisers, catering events and cooking demonstrations and special events.
• Keep records and prepare reports as required. Inclusive of any certification requirements and/or medical clearances required of subordinate employees and volunteers.
• Oversee community garden program, including crop rotation, seasonal planting, and maintenance assignment.
• Coordinate with office staff, marketing and sales strategies to increase visibility and participation for meal programs.
• Ensure kitchen compliance with health regulations and participate in health inspections.
• Other duties as assigned.
