THE POSITION: 

The City of Hayward is excited to announce its recruitment for the position of Mobile Mental Health Clinician. Under general direction, the Mobile Mental Health Clinician provides field and office based professional behavioral health counseling, psychoeducation, outreach and engagement, and case management services as part of the City’s Hayward Evaluation and Response Teams (HEART) Program improving public safety responses to people experiencing mental illness, substance use disorders and homelessness.

THE PROGRAM

The Hayward Evaluation and Response Teams (HEART) Program is a cross departmental program between the Hayward City Manager’s Office, Police, and Fire Departments and Alameda County Behavioral Health Care Services developed to improve access to medical, mental health, and other support services.  The HEART program aims to achieve two main goals: 1) to reduce the need for police officers to intervene in calls for service involving people experiencing chronic mental illness, substance use disorders, and homelessness and 2) to reduce reliance on emergency services for individuals needing long-term case management and follow-up services.  HEART consists of three distinct response teams: the Mobile Integrated Health Unit (MIHU) made up of Hayward Fire Department Community Paramedics; the Hayward Mobile Evaluation Team (HMET) which pairs a specially trained Hayward Police Officer with an Alameda County behavioral health clinician; and the Case Management and Mental Health Linkages Team (LINK) consisting of Mobile Mental Health Clinicians and a Lead Program Assistant (Case Manager).

THE IDEAL CANDIDATE

 As a member of the LINK team, the ideal candidate has experience providing short term crisis intervention, safety assessments, and other mental health services to adults, youth and children experiencing mental illness, substance use disorders and/or homelessness.  They have the ability to work primarily in the field and have the situational awareness to respond alongside emergency response teams. They are knowledgeable about local community resources and skilled at forming relationships quickly.  
  

Essential Duties

What you will do:

Provides direct casework, individual and group therapy; secures and evaluates medical or psychosocial data such as behavior patterns, use of coping skills and family relationships; analyzes factors which exert an adverse effect upon health issues, treatment and recovery which contribute to or are responsible for behavioral health issues and/or affect the potential for recovery and resiliency. 

Performs field work to proactively serve the unhoused population, persons with mental illness and others in crisis.
 
 Interprets the social aspects of physical or behavioral health issues to clients, relatives, other behavioral health and medical care staff and appropriate persons or agencies.
 
 Provides crisis intervention as needed; stabilizes clients in crisis including brief intervention in clinic; arranges for hospitalization and coordinates treatment elements.
 
Assists clients in understanding clinical health challenges and their reaction to them, accepting the need for treatment, building on strengths and working toward the solution of challenges and stresses the importance of engaging in treatment to increase social functioning, modifying unrealistic attitudes, and strengthening client's effort toward satisfactory adjustments.
 
Establishes and maintains effective relationships with contractors and community organizations and other governmental agencies regarding programs and services; participates in committees, meetings and conferences; provides guidance regarding appropriate social-psychological practices and services.
 
Compiles and prepares reports and documentation on clinical activities; maintains records; prepares periodic reports.
 

Identifies emerging trends related to crimes and issues; develops and implements prevention and intervention program approaches; provides community education on current procedures and practices related to mental health and intervention strategies.
 
Serves as a liaison with law enforcement, school officials, social service agencies, and the general public regarding difficult and/or complex situations related to mental health and social services.
 
Provides support and guidance to Fire and Police First Responders concerning victim advocacy, juvenile and adult crime, domestic violence, chemical dependency, and other services as needed.
 
Represents function on committees, outside organizations, other City departments, and outside agencies, as appropriate.
 
 Fosters an environment that embraces diversity, equity, inclusion and collaboration.
 
 Be an integral team player, which involves flexibility, cooperation, tolerance and communication.

Any combination of experience and education that would likely provide the required knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be:
 

Experience:  Two (2) years’ experience working in the social work or counseling field with strong emphasis on social services, resources and referrals using a collaborative and community framework.

Education:  A Master’s degree from an accredited college or university with major course work in social work, psychology, counseling or a related field.

License & Certificates:  Possession of a valid professional mental health therapy license issued by the State of California, such as Licensed Clinical Psychologist, Licensed Clinical Social Worker (LCSW), Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), Associate Social Worker License (ASW), Intern Marriage Family Therapist (IMFT) or Clinical Psychologist or a closely related field is highly desirable.

Possess and maintain a valid California Class C Driver's License.

POSITION INFORMATION

Livermore Area Recreation and Park District is currently recruiting  for Preschool Teachers  who are cheerful, child focused, and creative to plan themed based programs. This position will be responsible for planning curriculum, engaging in conversations with children (age 4 and 5), assist children in projects, supervise indoor and outdoor play, interact with parents, oversee the direction of the assigned teacher assistant, and be responsible for the overall safety of the children in the program.

PHYSICAL DEMANDS

Behavioral Health Clinician I:
Behavioral Health Clinicians at this level provide a variety of community behavioral health services to clients and to the community in an outpatient setting. Responsibilities include conducting assessments; performing case coordination, care management, and rehabilitative services; providing diagnosis and treatment plans; providing psychotherapy; providing linkage to resources and support to maintain resources; and preparing reports, case summaries, and other required forms.

Behavioral Health Clinician II:
At this level, Behavioral Health Clinicians handle more complex or difficult client cases. In addition to the responsibilities of a Behavioral Health Clinician I, incumbents at this level may lead teams conducting intake assessments and crisis intervention.

Both positions require a master's degree from a recognized school of Social Work, Counseling, Psychiatric Nursing, or other Marriage and Family Therapy eligible discipline, or a doctorate degree in Psychology. 

Behavioral Health Clinician I:

Field placement of at least one (1) year in a medical, psychiatric, or community mental health setting

AND

Registration as an Associate Marriage and Family Therapist (AMFT), Associate Clinical Social Worker (ACSW), and/or Associate Professional Clinical Counselor (APCC) with the State of California Board of Behavioral Sciences (BBS) OR a Registered Psychologist with the Board of Psychology. 

Behavioral Health Clinician II: 

Two (2) years of experience in an out-patient clinic, hospital, child guidance clinic, mental health program, or family agency which included regular psychiatric consultation

The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

Examples of Duties

- Manage a caseload of 28-32 clients comprised of older individuals with complex medical conditions.
- Conduct comprehensive client assessments, and develop and monitor service plan.
- Collaborate with team members, Human Service programs, and community agencies.
- Conduct home visits and provide other community interventions as needed.
- Conduct inter-agency and/or family conferences.
- Advocate for clients in accessing services.
- Monitor services provided for each client.
- Maintain timely electronic documentation of services, reporting, and billing.
- Participate in peer chart audits and outside audit preparation.
- Prepare and present training, outreach, and educational sessions and materials.
- Supervise interns in graduate and undergraduate programs.

The Position

The Case Manager will perform professional and clinical social work with individuals and/or families who are unhoused or housing insecure, and who may have complex social and medical needs including health care, financial assistance, employment opportunities, as well as housing assistance. As a member of the Human Services Department, this position reports to a Clinical Supervisor, under direction of the division manager of Homeless Care and Response (HCR), and is characterized by the responsibility to provide comprehensive psycho- social assessments and develop and implement care plans. These plans may focus on connecting clients  to public benefits, obtaining budgeting skills, accessing medical care, and obtaining more stable living arrangements. The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits in the company of another staff member in the field to locations where homeless persons may reside,  to medical facilities, and other community locations.
