San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. JOB SUMMARY: The San Mateo County Transit District is seeking a highly technical IT Operations Supervisor to join our team. Under the supervision of the IT Operations Manager, the Supervisor will coordinate the efficient and effective operations of the Help Desk team for the District, including work direction and supervision of Help Desk staff and coordination with other District information technology staff. This position is responsible for planning, assigning, and directing work; documenting and gaining approval for all help desk support procedures and system documentation; and assisting team members in answering calls, addressing service or escalation issues, and applying independent decision making to resolve problems or escalate problems to management. The IT Operations Supervisor develops regular reports on help desk contacts, ensures that service levels are met, identifies trends and makes recommendations for service improvements, and provides budgetary recommendations to management for service growth and/or improvement.

IT Operations Supervisor will work closely with various users in San Mateo County Transit District (SamTrans), the Peninsula Corridor Joint Powers Board (Caltrain), and the San Mateo County Transportation Authority (TA) and other departments and is responsible for managing all procedures related to the identification, prioritization, and resolution of end user help requests, including the monitoring, tracking, and coordination of Help Desk functions. The workweek may consist of four workdays of 10.5 hours followed by 3 consecutive days off or five workdays of 8.5 hours followed by 2 consecutive days off. Each workday includes two paid 15 minute breaks and a 1/2 hour unpaid meal breaks. OSSD employees wear uniforms. This position is eligible for a uniform allowance which will not exceed $500.00 annually. Payments shall be made during the first two (2) pay periods of each month, in the amount of $20.83 per biweekly pay period. Trainees are expected to purchase and begin wearing their uniforms by the start of the 16th week of training.

Trainees are rotated through different units of OSSD and will be assigned to various shifts (Day, Swing and Midnight) and days off. New PDS's should expect to work night shifts, weekends and holidays. A shift differential of $2.25 per hour for Swing Shift and $2.50 per hour for Midnight shift is added to the base salary and compensatory time or pay will be granted for holiday work in accordance with the memorandum of agreement between the City of San Jose and the Municipal Employees Federation.

After training, PDS's bid for assignments, shifts (day, swing or midnight) and days off every six months in order of seniority as a Police Data Specialist. Senior employees typically select preferred shifts and days off and may have the option of a five day work week and/or alternate start times available to them. New employees may work nights, weekends and holidays for several years before they attain the seniority required to bid for more desirable assignments, shifts, days off and/or start times.

Special Requirements:  Current Lifeguarding, Standard First Aid, and CPR for the Professional Rescuer certificates are required. Current certifications for this program are not required prior to applying for position.

• Per the American Red Cross, the minimum age for Lifeguard certification is 15 years old.  Certifications shall be maintained during employment.

SALARY AND HOURS:

$17.06 – $17.91– $18.80 – $19.75 - $20.72 per hour.  This is a part-time position, paid sick leave benefit available. Initial appointments are generally made at the first step of this five step scale.  Hours will range from 15-25 hours per week.  Candidates that make the eligibility list may be selected for shifts as needed throughout the year.  Must be able to work flexible hours, including evenings, weekends, and holidays throughout the entire year.

SELECTION PROCESS:

Applications will be screened and a limited number of the most qualified candidates will be invited to participate in the examination process, which may consist of an oral panel interview, department interview, or in the form of a practical demonstration of skill and ability, or any combination of these.  A passing score of 70% is required to make the eligibility list.   Recommendations for appointment from this list will be made to the Recreation Supervisor, who will make the final decision.  Depending on the number of applicants, this process may be altered.  Prior to appointment, a background check, including fingerprinting for submission to the Department of Justice must be completed, as well as TB screening.
