The Human Services Department (HSD) delivers and supports services by forging long-term community partnerships; engaging with and building the capacity of the community to do its own problem- solving; and leveraging financial and volunteer resources.  The Aging and Family Services Division (AFS) of HSD specifically supports older residents and their caregivers to live active, healthy lives in the Tri-City community.

The Position

As a member of the HSD assigned to the AFS, the Senior Program Coordinator will organize, coordinate, and implement community wellness programs for older adults. The Community Programs include health promotion activities such as health screenings, wellness classes, and educational activities. The position works within a team that includes volunteers, health promoters, nurses, social workers and mental health workers. The position facilitates community awareness and partnerships, with individuals and agencies, to promote an age-friendly environment.

San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

 

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

 

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

 

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.
• Experienced with Behavioral Health and Recovery Services (BHRS), as well as other mental health community programs.
• Knowledgeable of the systems of care related to older adults, mental health, substance abuse, developmental delay, pediatrics, primary care, palliative care, chronic illnesses, dialysis, conservatorship, and probation and parole.
• Able to work independently with little supervision and demonstrates a high degree of initiative.
• Knowledgeable of housing resources, their requirements for eligibility, and other community services such as In-Home Supportive Services (IHSS), Multipurpose Senior Services Program (MSSP), Institute on Aging (IOA), Health Plan of San Mateo (HPSM), BHRS, and Bridges to Wellness, and more.
• Knowledgeable about County benefit and entitlement programs, such as Medi-Cal and ACE.
• Experienced and comfortable working with the following populations: multi-cultural, homeless, chronic diseases, end-of-life, mental health, domestic violence, parolees, and substance abuse disorders.
• Experienced working with a diverse population (race, ethnicity, age, ability, language, nationality, socioeconomic status, gender, religion, and sexual orientation).
• Able to accurately assess for signs of adult and child abuse and be a mandated reporter.
• Able to work in a politically charged environment under high degrees of stress and meet all deadlines.
• Able to develop effective and positive working relationships with community partners such as Aging and Adult Services, BHRS, and HPSM.
• An effective communicator to a broad range of stakeholders, including doctors, nurses, conservators, health advocates, families, and patients.
• Able to remain calm during crisis situations and perform emergency crisis counseling for families and patients as needed.
• Experienced documenting in an EMR.
• Able to communicate clearly and concisely.
• Spanish-speaking preferred.

 

Examples Of Duties

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.
• Maintain case files utilizing the electronic medical record.
• Work with the hospital's multidisciplinary team of staff to identify and address barriers to care, participate in quality assurance efforts such as outcome analysis, chart reviews, and patient and staff satisfaction efforts, and assist with maintenance of Center policies.
• Document interactions with patients/clients in EMR.
• Be a member of the Palliative Care Team.
• Work in the emergency room or any other areas in the hospital that social workers cover.

 

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

The City of Dixon is accepting applications to fill one (1) full-time position of Transit Driver/Dispatcher and to establish an eligibility list for future use.  

This position drives a Readi-Ride General Public Paratransit Vehicle (GPPV) on a demand responsive curb-to-curb transit system and assist patrons boarding and leaving the bus, especially the elderly, disabled, and school age children. The Transportation Supervisor is an important part of the City's transit team that provide services including the Auburn Loop fixed route and Auburn OnDemand ride share programs.

• Plan, prioritize, assign, supervise, review, and participate in the work of staff responsible for Auburn Transit daily operations; perform regular road supervision, vehicle trial checks, and daily vehicle inspection monitoring.
• Establish schedules and methods for providing Auburn Transit services; schedule drivers to ensure routes are completed in a safe and timely manner; coordinate any changes to facilitate coverage for all shifts.
• Coordinates transportation needs for local schools and special events; prepares calendar of trips or special events to be incorporated into the bus timetable; identify resource needs; review needs with appropriate management staff; allocate resources accordingly.

Role description

This recruitment will be used to fill both full-time and part-time positions.

Under general supervision, operates a variety of transit vehicles such as diesel and electric motor coaches, cable cars, streetcars and light rail vehicles; transports passengers along a specific route and makes designated stops as required; accepts fares, issues transfers, gives general directional information; and performs related duties as required.

Examples of Important and Essential Duties
According to Civil Service Commission Rule 409, the duties specified below are representative of the range of duties assigned to this job code/class and are not intended to be an inclusive list.

1. Conducts pre-operational inspection of the transit vehicle, in accordance with federal and state laws and departmental regulations, to ensure it is functioning properly.
2. Inspects the passenger compartment of the vehicle to ensure it is free from hazards (such as spills, loose handrails and stanchions, defective seats, etc.) and that the video camera lenses are not obscured.
3. Safely navigates the vehicle through traffic along assigned routes, under various environmental conditions, in accordance with traffic laws and departmental regulations.
4. Operates the disability access lifts to assist people in wheelchairs onto the vehicle and properly secures their wheelchairs when they are on the vehicle.
5. Collects fares from passengers and issues transfers; checks passenger transfers, fast passes, and electronic passes to ensure they are valid.
6. Uses vehicle's intercom system to communicate instructions to passengers and to call out vehicle stops and transfer points.
7. Responds to customers' questions regarding routes, directions, fares and transfers.
8. Conducts a post-operational inspection of the outside of the vehicle to detect defects and body damage, broken windows, and graffiti.
9. Contacts Central Control regarding various emergencies such as medical emergencies, vehicle malfunctions, vehicle evacuations etc. and/or circumstances that may affect service such as heavy traffic, road hazards, road obstructions etc.
10. Completes accident, incident and/or miscellaneous reports to document unusual occurrences.

Nature of Work: Incumbents in this class may be required to work rotating shifts, holidays and weekends; to work outdoors in all weather conditions; and to use computers. In addition, may require standing and/or sitting for prolonged periods of time; performing different physical activities such as pulling poles, cranking switches in the subway, resetting retrievers to return disabled transit vehicles to service and assisting in the movement of disabled vehicles when possible. The San Francisco Municipal Transportation Agency (SFMTA) has determined that Class 9163 Transit Operator is a “safety-sensitive” position, under regulations issued by the Federal Transit Administration (49 CFR Part 655). Federal law requires that all transit employees who perform safety-sensitive functions be subject to random, post-accident, reasonable suspicion, return-to-duty and follow-up drug and alcohol testing. All applicants for Safety-Sensitive positions shall undergo urine drug testing prior to employment and within 90 days prior to performing Safety-Sensitive functions for the first time. Receipt by the SFMTA of a negative test result is required prior to the employee being placed on the payroll. A positive, adulterated, negative dilute or substituted test will result in a decision not to hire, and the applicant’s name will be removed from the list of eligibles for Safety-Sensitive positions. The SFMTA will not consider hiring any person who tested positive, adulterated, substituted or refused to submit to testing for a minimum of two years following the positive test.

In addition, each applicant who has worked for a Department of Transportation (DOT) – regulated employer(s) within the last two years will be required to sign a consent form, prior to appointment, authorizing SFMTA to obtain information from their prior employers concerning their drug and alcohol test history. Class B Permit with Passenger (P) endorsement and Airbrakes: 
Applicants who are successful in the examination process and who do not possess a Class B Driver License with Passenger (P) endorsement and airbrakes, will be required to obtain a Class B Driver Permit with Passenger (P) endorsement and airbrakes prior to starting the Transit Operator training.

Transit Operator Training:
All employees must successfully complete the SFMTA Operator Training Program consisting of both classroom and behind-the-wheel instruction. The rate of pay for a trainee is $31.0375 per hour.

Attendance to all training sessions is required. If a candidate misses any dates of training without providing proper notification, they will be released from their training class. If proper notification was provided, a candidate released during the training period due to attendance may request to be placed back on the eligible list to be considered for future classes, if the list from which they were hired is still active. Candidates may only request to be placed back on the eligible list one time.  Any candidate not approved to be placed back on the list or not able to be placed in another training class before the list expires will need to re-apply during the next filing period for applications.

JOB SUMMARY: The San Mateo County Transit District is seeking a highly technical IT Operations Supervisor to join our team. Under the supervision of the IT Operations Manager, the Supervisor will coordinate the efficient and effective operations of the Help Desk team for the District, including work direction and supervision of Help Desk staff and coordination with other District information technology staff. This position is responsible for planning, assigning, and directing work; documenting and gaining approval for all help desk support procedures and system documentation; and assisting team members in answering calls, addressing service or escalation issues, and applying independent decision making to resolve problems or escalate problems to management. Under general direction, the Mobile Mental Health Clinician provides field and office based professional behavioral health counseling, psychoeducation, outreach and engagement, and case management services as part of the City’s Hayward Evaluation and Response Teams (HEART) Program improving public safety responses to people experiencing mental illness, substance use disorders and homelessness.

THE PROGRAM

The Hayward Evaluation and Response Teams (HEART) Program is a cross departmental program between the Hayward City Manager’s Office, Police, and Fire Departments and Alameda County Behavioral Health Care Services developed to improve access to medical, mental health, and other support services.  The HEART program aims to achieve two main goals: 1) to reduce the need for police officers to intervene in calls for service involving people experiencing chronic mental illness, substance use disorders, and homelessness and 2) to reduce reliance on emergency services for individuals needing long-term case management and follow-up services.  HEART consists of three distinct response teams: the Mobile Integrated Health Unit (MIHU) made up of Hayward Fire Department Community Paramedics; the Hayward Mobile Evaluation Team (HMET) which pairs a specially trained Hayward Police Officer with an Alameda County behavioral health clinician; and the Case Management and Mental Health Linkages Team (LINK) consisting of Mobile Mental Health Clinicians and a Lead Program Assistant (Case Manager).
