THE POSITION: 

The City of Hayward is excited to announce its recruitment for the position of Mobile Mental Health Clinician. Under general direction, the Mobile Mental Health Clinician provides field and office based professional behavioral health counseling, psychoeducation, outreach and engagement, and case management services as part of the City’s Hayward Evaluation and Response Teams (HEART) Program improving public safety responses to people experiencing mental illness, substance use disorders and homelessness.

THE PROGRAM

The Hayward Evaluation and Response Teams (HEART) Program is a cross departmental program between the Hayward City Manager’s Office, Police, and Fire Departments and Alameda County Behavioral Health Care Services developed to improve access to medical, mental health, and other support services.  The HEART program aims to achieve two main goals: 1) to reduce the need for police officers to intervene in calls for service involving people experiencing chronic mental illness, substance use disorders, and homelessness and 2) to reduce reliance on emergency services for individuals needing long-term case management and follow-up services.  HEART consists of three distinct response teams: the Mobile Integrated Health Unit (MIHU) made up of Hayward Fire Department Community Paramedics; the Hayward Mobile Evaluation Team (HMET) which pairs a specially trained Hayward Police Officer with an Alameda County behavioral health clinician; and the Case Management and Mental Health Linkages Team (LINK) consisting of Mobile Mental Health Clinicians and a Lead Program Assistant (Case Manager).

THE IDEAL CANDIDATE

 As a member of the LINK team, the ideal candidate has experience providing short term crisis intervention, safety assessments, and other mental health services to adults, youth and children experiencing mental illness, substance use disorders and/or homelessness.  They have the ability to work primarily in the field and have the situational awareness to respond alongside emergency response teams. They are knowledgeable about local community resources and skilled at forming relationships quickly.  
  

Essential Duties

What you will do:

Provides direct casework, individual and group therapy; secures and evaluates medical or psychosocial data such as behavior patterns, use of coping skills and family relationships; analyzes factors which exert an adverse effect upon health issues, treatment and recovery which contribute to or are responsible for behavioral health issues and/or affect the potential for recovery and resiliency. 

Performs field work to proactively serve the unhoused population, persons with mental illness and others in crisis.
 
 Interprets the social aspects of physical or behavioral health issues to clients, relatives, other behavioral health and medical care staff and appropriate persons or agencies.
 
 Provides crisis intervention as needed; stabilizes clients in crisis including brief intervention in clinic; arranges for hospitalization and coordinates treatment elements.
 
Assists clients in understanding clinical health challenges and their reaction to them, accepting the need for treatment, building on strengths and working toward the solution of challenges and stresses the importance of engaging in treatment to increase social functioning, modifying unrealistic attitudes, and strengthening client's effort toward satisfactory adjustments.
 
Establishes and maintains effective relationships with contractors and community organizations and other governmental agencies regarding programs and services; participates in committees, meetings and conferences; provides guidance regarding appropriate social-psychological practices and services.
 
Compiles and prepares reports and documentation on clinical activities; maintains records; prepares periodic reports.
 

Identifies emerging trends related to crimes and issues; develops and implements prevention and intervention program approaches; provides community education on current procedures and practices related to mental health and intervention strategies.
 
Serves as a liaison with law enforcement, school officials, social service agencies, and the general public regarding difficult and/or complex situations related to mental health and social services.
 
Provides support and guidance to Fire and Police First Responders concerning victim advocacy, juvenile and adult crime, domestic violence, chemical dependency, and other services as needed.
 
Represents function on committees, outside organizations, other City departments, and outside agencies, as appropriate.
 
 Fosters an environment that embraces diversity, equity, inclusion and collaboration.
 
 Be an integral team player, which involves flexibility, cooperation, tolerance and communication.

Any combination of experience and education that would likely provide the required knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be:
 

Experience:  Two (2) years’ experience working in the social work or counseling field with strong emphasis on social services, resources and referrals using a collaborative and community framework.

Education:  A Master’s degree from an accredited college or university with major course work in social work, psychology, counseling or a related field.

License & Certificates:  Possession of a valid professional mental health therapy license issued by the State of California, such as Licensed Clinical Psychologist, Licensed Clinical Social Worker (LCSW), Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), Associate Social Worker License (ASW), Intern Marriage Family Therapist (IMFT) or Clinical Psychologist or a closely related field is highly desirable.

Possess and maintain a valid California Class C Driver's License.

POSITION

Under general supervision, provides highly responsible administrative support to the City Clerk to assist with the day-to-day operations of the City Clerk’s Office. This is a part-time, non-benefited position. Work schedule is flexible up to 30 hours per week.

ESSENTIAL FUNCTIONS

The County of Yuba is currently recruiting for the position of Public Health Nurse Intern/Public Health Nurse in the Health and Human Services Department. This position provides the full range of public health nursing services including teaching, health assessment, and counseling services in homes, clinics, schools and community centers in connection with the prevention and control of diseases and adverse health conditions. Public Health Nurse Intern: is a recruiting and training level class established to qualify candidates for duties as a PHN in Yuba County Health & Human Services Department. Under close supervision, incumbents learn and perform a variety of public health nursing duties in assigned program areas while pursuing a Public Health Nurse Certification. Upon satisfactory completion of the PHN Certification, the PHN Intern are eligible for promotion to the level of PHN. Incumbents who do not successfully complete the PHN certification as prescribed herein may be released from employment.

Public Health Nurse: is the entry level in this class series. Initially under close supervision, incumbents learn and perform a variety of public health nursing duties in assigned program areas. As experience is gained, assignments become more complex and are performed under more general supervision.

THE POSITION

Tri-City is looking for a special individual to serve as a Clinical Therapist I/II for the Children’s Outpatient (COP) Department. The Clinical Therapist I/II provides behavioral health clinical intervention and recovery services to assigned clients and their families; develops treatment plans and performs individual, group, and family psychotherapeutic counseling, routine and emergency assessment, crisis intervention, and related treatment services; manages an assigned caseload of adults, juveniles, and/or children; provides consultation to other staff, programs, and agencies related to treatment needs and plans; and performs related duties as assigned.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Provides individual, group, and family interventions, rehabilitation, and treatment services including counseling, therapy, case management, and referral and linkage to community services to a variety of clients including high acuity clients.

• Within scope of authority, assesses, diagnoses, and treats a variety of clients with mental health disorders; collaborates with clients and creates individualized treatments plans according to client’s needs and circumstances.

• Initiates and conducts individual, group, and family counseling and biopsychosocial therapy sessions utilizing a variety of techniques and modalities; provides case management services in the office and in the field.

• Performs routine and emergency intake evaluations according to Authority procedure; completes appropriate forms and documents relating to the intake process; and prepares medical and demographic case history for use in diagnosis.

• Assists clients and family members in understanding behavioral health diagnosis, contributing factors/causes, effects, prognosis, rehabilitation, and treatment.

• Works with clients to address stressors and barriers to treatment and/or social functioning; identifies and focuses on client strengths and personal resources to effectively engage and treat client.

The Department

The Human Services Department (HSD) delivers and supports services by forging long-term community partnerships, engaging with and building the capacity of the community to do its own problem-solving, and leveraging financial and volunteer resources. The Department’s nationally and internationally recognized programs support thousands of residents throughout their life course, from infants to elders. The Aging and Family Services Division (AFS) of HSD specifically supports older residents and their caregivers to live active, healthy lives in the Tri-City community. With input from local, older adults (age 55 years+) as our guide, the AFS team strives to provide and AFS staff work together to provide a continuum of supportive services that range from health promotion activities to home-based end of life care. AFS has a Senior InfoLine (phone and email) that is answered in 4 language (Farsi, English, Spanish and Chinese) that is often an entry point. As an older adult or their caregiver’s situation changes, they seamlessly have access to a continuum of programs that include counseling and emotional support; care coordination and resource assistance; specialized family caregiver support and education. Many of these supportive services are provided in the resident’s home. We also provide regular expert clinical consultation, supervision, opportunities for self-reflection, and team building, fostering a culture of empathy, inclusivity, humility, and respect.

The Position

Under the supervision of the Age Well Center Managers, the Chef/Food Services Manager position manages meal programs for the City of Fremont’s Age Well Centers. Chef responsibilities across both Age Well Center locations include the performance of menu planning, purchasing of foodstuffs, preparing quantity meals, supervising and assisting kitchen employees and volunteers and maintaining clean and orderly kitchens.

The incumbent will be responsible for properly cooked, economical, and wholesome, well balanced meals in City operated Age Well Centers. This position is not limited to being responsible for the assignment of work, supervision and evaluation of paid and volunteer food service workers. The review and approval of proposed menus, food purchases, control and monitoring of expenditure rates; and institution of disciplinary actions are functions reserved for the Program Coordinator or Age Well Center Managers.

Examples of Duties

• Plan menus with consideration for the dietary needs of older adults, wholesomeness, quantity of food to be prepared and budgetary constraints.
• Estimate food requirements and requisitions for food and supplies.
• Prepare, cook, and serve a wide variety of foodstuffs for quantity food service.
• May cut/carve and cook meats; bake breads, cakes and pastries; peel and prepare vegetables; and make salads.
• Place food in serving containers and serves food.
• Maintain proper temperature, rotation and storage of food.
• Direct and assist in cleaning kitchens and kitchen equipment after each meal.
• Estimate food requirements and requisitions for food and supplies.

• Oversee that the kitchen and equipment are kept clean, sanitary and in proper working condition. Schedule regular maintenance of kitchen equipment as needed. Maintain standards of safety and sanitation in kitchen area.
• Recruit, train, supervise, instruct, and direct employees and volunteers assigned to working in the kitchens.
• Develop and support additional meal programs, including, but not limited to fundraisers, catering events and cooking demonstrations and special events.
• Keep records and prepare reports as required. Inclusive of any certification requirements and/or medical clearances required of subordinate employees and volunteers.
• Oversee community garden program, including crop rotation, seasonal planting, and maintenance assignment.
• Coordinate with office staff, marketing and sales strategies to increase visibility and participation for meal programs.

Role description

Under direction, the 2595 Senior EAP Counselor provides direction and supervision to counselors providing organizational wellness services and engaged in direct client counseling; provides clinical assessments, short-term counseling, and referral of City and County employees experiencing difficulties related to personal or work-related problems; conducts seminars, trainings, and consultations on issues related to personal and organizational wellness; prepares records and reports; and performs related duties as required.

Provides clinical assessment, short-term counseling, and referral of City and County employees experiencing difficulties related to personal or work-related problems, such as stress, depression, anxiety, substance abuse, emotional concerns or relationship issues; provides counseling to family members and significant others as appropriate.
3. Educates employees, managers, supervisors, and union personnel regarding employee assistance program service offerings, how to access services, early identification of personal and workplace problems, and referrals to the program; directs the development of programs, resources, and EAP initiatives.
4. Provides consultation to managers, supervisors, union personnel, human resources professionals, clinical specialists, and family members regarding specific issues of individual employees in conformance with program and confidentiality policies; recommends specific methods or techniques for resolving performance problems, including appropriate managerial and supervisory interventions.
5. Directs development of, creates, and facilitates seminars and trainings for employees and management on personal, interpersonal and organizational issues such as the effective management of stress, anger and emotions, workplace negativity and dealing with difficult people.
6. Provides crisis intervention, trauma response, and mediation/conflict resolution services as necessary.
7. Leads evaluation efforts to determine the effectiveness of the overall program and specific services provided and prepares the reports.
