The County of Yuba is currently recruiting for the position of Public Health Nurse Intern/Public Health Nurse in the Health and Human Services Department. This position provides the full range of public health nursing services including teaching, health assessment, and counseling services in homes, clinics, schools and community centers in connection with the prevention and control of diseases and adverse health conditions. Public Health Nurses are assigned to specific programs such as Maternal, Child and Adolescent Health, California Children’s Services, Child Health and Disability Prevention, Foster Care and communicable disease control. Public Health Nurse Intern: is a recruiting and training level class established to qualify candidates for duties as a PHN in Yuba County Health & Human Services Department. Under close supervision, incumbents learn and perform a variety of public health nursing duties in assigned program areas while pursuing a Public Health Nurse Certification. Upon satisfactory completion of the PHN Certification, the PHN Intern are eligible for promotion to the level of PHN. Incumbents who do not successfully complete the PHN certification as prescribed herein may be released from employment.

Public Health Nurse: is the entry level in this class series. Initially under close supervision, incumbents learn and perform a variety of public health nursing duties in assigned program areas. San Mateo County Health is seeking an experienced individual for the position of Public Health Clinics Process Improvement Manager that plans, oversees, and provides a wide variety of administrative, analytical, and work-flow support on a County-wide level; analyzes practices and procedures and makes recommendations for organizational, operational, policy, and procedural improvements; conducts needs analysis, feasibility studies, and program evaluation for assigned projects and programs; develops, summarizes, and maintains administrative and fiscal records; fosters cooperative working relationships among County departments and acts as liaison with various community, public, and regulatory agencies; and performs related work as required.

There is currently one regular vacancy in the Public Health, Policy and Planning Division, located in San Mateo, but the position will include travel/driving throughout the county to perform duties of the job.

This management position will report to the Public Health Clinics Manager, overseeing Edison Clinics and Mobile Clinics and will have broad project management responsibility for key strategic efforts affecting the Public Health clinics, and will exercise direct and general supervision over assigned professional, technical, and office support staff.

Primary responsibilities will include, but are not limited to, the following:

• Quality assurance and improvement: build and visualize data in management reports and dashboards; convert data to information and use it to guide process improvement.
• Process improvement: facilitate, document, track, organize, and measure progress, e.g., conduct improvement work within daily huddles.
• Project management: lead projects and provide ad hoc support of projects.
• Knowledge management: inventory and maintain infrastructure of core programmatic documents and processes, how files are stored and shared.
• Partnership building: support internal staff operational needs; coordinate with other stakeholders, such as staff from Behavioral Health & Recovery Services, Human Services Agency, Health IT, San Mateo Medical Center, grant makers, state agencies, etc.
• Management of staff: supervise and influence when appropriate on projects.
• Contract and MOU support: assist Clinics Manager to write scopes of work, produce required reports and invoices for categorical funding and operational agreements.
• Revenue optimization support: oversee billing quality assurance and improvement, facilitate braided funding opportunities and oversight.
• Policy and procedures facilitation: maintain inventory and lead update processes through completion, including occasional initial literature reviews or research and coordination of stakeholders.
• Change management and leadership synchronization: coordinate staff engagement, training, feedback to ensure successful system changes, facilitate leadership development and decision making, contribute to agendas, take meeting minutes, track on-going project work in support of meeting goals.

Ideal candidate will possess:

• Experience with project management and process improvement as the lead.
• Knowledge and/or experience with: County of San Mateo; Public Health; healthcare clinics; vulnerable population care management including people experiencing homelessness; and HIV, Sexually Transmitted Infections (STIs), and other Communicable Diseases.
• Data skills including requirements gathering; ad hoc report building; data and dashboard management.
• Knowledge of and/or experience with information technology systems including Power Business Intelligence (BI); advanced Excel; eClinical Works (eCW); ARIES; Microsoft SharePoint; Epic; Workday reporting.

Qualifications

Education and Experience :

Equivalent to graduation from an accredited four-year college or university with major coursework in business or public administration, economics, planning, finance, or a related field, and five (5) years of increasingly responsible professional administrative and analytical experience in municipal government, including experience in budget, strategic planning, policy analysis, and organizational development.

Responsibilities

Correctional Health Services (CHS) therapists provide clinical mental health services utilizing Trauma Informed Care to inmates with mental illness, serious mental illness, or substance use disorders and/or cooccurring disorders of mental illness, serious mental illness, and chemical dependency. They also provide case management services and discharge planning by interviewing inmate patients to assess mental health needs and formulating treatment plans. They function as a team member in a multi-disciplinary work setting where they collaborate with Sheriff's Office staff to manage inmate patients' daily activities. Positions require some weekend hours.

Ideal Candidate

• A current license as either a Licensed Clinical Social Worker or Marriage and Family Therapist.
• Solid clinical treatment, assessment, and diagnostic skills.
• Knowledge of principles, techniques and trends in counseling, psychotherapy, clinical case management, crisis management and various treatment modalities as they pertain to the justice involved adult population.
• Experience providing treatment for adults with serious mental illness.
• Experience delivering services to adult incarcerated persons who have co-occurring mental health and substance use disorders.
• Excellent documentation skills: progress notes, assessments, and treatment planning, etc.
• Demonstrated expertise, skill and interest in working with a broad range of clients of diverse cultures and backgrounds, such as Latino, Pacific Islander, African American, and LGBTQ, and an ability to provide culturally appropriate and proficient services.
• Skill in establishing and maintaining productive working relationships with families, co-workers, other professionals, the Sheriff’s office, collaborative partners and the public.
• Extensive knowledge of available community resources within the County of San Mateo and its neighboring counties.
• Experience using electronic health/medical record systems (e.g. AVATAR, SOARIAN, Epic).
• Fluency in any of the threshold languages: Spanish, Tagalog, Chinese (Cantonese/Mandarin) or Russian and experience providing clinical services using bilingual skills.

THE DEPARTMENT

The Parks and Recreation Department is the third largest department in the City and is organized by program area, with each program administered by a supervisor, coordinator, or specialist. The Parks and Recreation Department is composed of a Director and 22 full-time management positions. The department is comprised of the Recreation, Parks, and Building Maintenance divisions. The Parks and Recreation Department mission is to provide opportunities for physical, cultural and social well being; protect and enhance the physical environment; and ensure the effective and efficient use of public facilities and open space.

The Building Maintenance Division maintains 32 facilities totaling more than 563,000 square feet and over 275 acres of park land. The division provides janitorial, preventative maintenance services, and necessary building and related equipment repairs to all of the City's facilities, including interior and exterior work. The division also performs a variety of tasks, which includes moving furniture, set-ups, and many other miscellaneous tasks as required. 

The Public Authority for In-Home Supportive Services (IHSS) is seeking well-qualified individuals for the position of Public Authority Registry Social Worker II/III with fluency in Mandarin or Cantonese. There is one vacancy in the Public Authority. The current vacancy would be responsible for supporting IHSS consumers who do not have caregiver resources, in hiring a homecare provider from the Public Authority Registry and helping the client and provider establish a successful working relationship. The position is located in San Mateo, CA, but will require travel throughout the County to perform duties of the positions.

NOTE: The Public Authority Registry Social Worker II/III is not a County position; the Public Authority unit reports to the San Mateo County Director of Aging and Adult Services.

PUBLIC AUTHORITY

The Public Authority serves as the employer of record for IHSS homecare workers, with the IHSS consumer retaining the right to hire, fire and train their worker. The Public Authority's responsibilities include operating a Registry that recruits, screens and refers potential homecare providers to IHSS consumers needing personal assistance in their home; screening the backgrounds of IHSS providers in accordance with IHSS policies and regulations; providing training opportunities to IHSS providers to enhance their skills in serving IHSS consumers; and offering support and assistance to Registry consumers and providers to ensure the employer-employee relationship between the Registry consumer and provider are functioning well. The IHSS consumer is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive. Consumers who do not have caregiver resources, may choose to hire a caregiver from the Public Authority Registry. The Public Authority Social Worker will assist those recipients with the hiring of a Public Authority Registry worker and help them to manage their role as the employer.

Examples of duties may include, but are not limited to, the following:

• Conduct home visits to support consumers' hiring needs.
• Educate, support, and perform administrative duties for IHSS consumers and care providers to understand/comply with program regulations.
• Provide conflict mediation between consumers and providers to help stabilize cases.
• Collaborate with program integrity team to ensure proper adherence to regulations.
• Provide education, outreach, and community resources information for consumers.
• Attend case conferences.
• Collaborate and facilitate services with other partner agencies.
• Effective caseload management

DEFINITION: Under general supervision the aide assists in planning, organizing, and implementing preschool programming for children ages 3-5 years old. Program hours are M-F PM 8:30am – 1:00pm and/or MWF 8:30am - 1:00pm. Approximately 15 – 30 hours a week. Substitute and floating positions also available 8:30am - 1:00pm. 

EXAMPLES OF DUTIES:

• Assists in the development, coordination, planning, implementing and supervising the day-to-day activities, themes and special events for an ongoing preschool program.
• Create and maintain a safe, challenging and creative environment.
• Supervise and engage preschool students during preschool hours.
• Demonstrate excellent customer service and communicate effectively with co-workers, parents, and children.
• Maintain registration, attendance, and activity records; ensure the proper pick-up/drop-off of children. 
• Encourage good behavior, appropriate manners, read books and provide a safe and nurturing environment.
• Implement and lead appropriate activities for preschool-aged children.
• Possess strong leadership skills in one or more of the following areas: art, crafts, sports, drama, cooking.
• Motivate children by example; create and maintain a positive environment.
• Must be a team player and have the ability to work independently with minimum supervision.
• Practice proper hygiene, with self and children, to deter the spread of disease (colds, flu, etc).
• Render first aid in case of minor injuries.
• Ability to communicate clearly both orally and in writing.
• The ability to speak English clearly and understandably.
• Perform related duties as required.

TRAINING & EXPERIENCE: Any combination of education, training, and experience, which likely provides the required knowledge and abilities.

PRE-EMPLOYMENT CONDITIONS: Candidates receiving a conditional job offer must pass a pre-placement fingerprint review. Candidate must provide a TB negative test result document. Candidates must possess a current CPR/AED and First Aid certificate and be comfortable calling 911 in an emergency.

• Possess strong leadership skills in one or more of the following areas: arts, crafts, sports, playground games, drama, songs/music, nature or special events. 
• Manage the task of checking in and checking out each child to the proper guardian. 
• Encourage good behavior and appropriate manners. 
• Encourage active play, read books and provide a safe and nurturing environment. 

• Ability to lead groups of campers in various day camp activities. 
• Motivate campers through example. 
• Must be a team player and have the ability to work independently with minimum supervision. 
• Create and maintain a positive day camp environment. 
• Accurately maintain attendance records. 

• Practice proper hygiene, with oneself and children, to deter the spread of disease (colds, flu, etc). 
• Respond professionally to emergencies, provide care according to the level of training.  
• The ability to speak English clearly and understandably. 
• Perform related duties as required.
