THE POSITION:

The City of San Jose, Animal Care and Services Division is seeking two (2) full-time Animal Shelter Veterinarians and multiple part-time Animal Shelter Veterinarians to work at the Animal Care Center. The Animal Shelter Veterinarian may perform all aspects of shelter medicine in a large animal shelter. Duties may include routine spay/neuter surgeries on cats, dogs, and other animals as indicated. THE POSITION

Tri-City is looking for one (1) individual to serve as a Psychiatrist I/II for the Adult Outpatient Clinic. The Psychiatrist I/II will provide medical and psychiatric treatment and diagnostic services to clients in the caseload of Tri-City and provide collaborative work with a multidisciplinary team in a public mental health agency. Provide integrated, effective, and high quality behavioral health care to the residents of Pomona, Claremont and La Verne with moderate to severe functional impairment due to mental illness.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Perform intake and diagnostic interviews on a scheduled and emergent basis.
• Participate in diagnostic staff evaluation of clients.
• Provide medical psychiatric treatment of clients.

The Position

Under the direction of the Youth and Family Services Administrator, the Senior Program Coordinator plays a unique role overseeing the early childhood mental health program, providing direct counseling services and supervising several staff counselors and interns.

The Senior Program Coordinator typically has specialized experience, training and qualifications that enable the position to work effectively with children 0-8 years, parents and youth caregivers.

The Senior Program Coordinator works closely with the division’s Clinical Supervisors and Quality Assurance Specialist to ensure that clinical services comply with all policies, procedures and legal regulations, and that client services are delivered at the highest standards. The position also coordinates with other key positions in the Human Services Department to help clients have easy and timely access to coordinated services throughout the Human Services Department and/or with other partner agencies.

Examples of Duties

- Oversee and coordinate the direct services under the early childhood mental health program, working closely with the counselors and interns assigned to the program.
- Provide administrative and clinical supervision and consultation to YFS counselors and interns/trainees who are providing direct counseling services to children 0-8 years, parents, and family members, while maintaining close cooperation with the Youth and Family Services Division’s Clinical Supervisors and Quality Assurance Specialist.
- Provide direct counseling and early childhood consultation services to children 0-8 years, parents, and family members.
- Together with the YFS Administrator, understand policies and trends related to the provision of early childhood mental health services; and represent the division at County and/or school district meetings.
- Ensure direct service staff and interns are familiar with and fully utilize client Electronic Health Record System and becomes a key user representative on system implementation team.
- Oversee clinical documentation and compliance with State Medi-Cal documentation standards in partnership with Clinical Supervisors and Quality Assurance Specialist.
- Manage the client intake and referral process for the early childhood mental health program.

The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

The Position

The Case Manager will perform professional and clinical social work with individuals and/or families who are unhoused or housing insecure, and who may have complex social and medical needs including health care, financial assistance, employment opportunities, as well as housing assistance. As a member of the Human Services Department, this position reports to a Clinical Supervisor, under direction of the division manager of Homeless Care and Response (HCR), and is characterized by the responsibility to provide comprehensive psycho- social assessments and develop and implement care plans. These plans may focus on connecting clients  to public benefits, obtaining budgeting skills, accessing medical care, and obtaining more stable living arrangements. The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits in the company of another staff member in the field to locations where homeless persons may reside,  to medical facilities, and other community locations.

Job Description

Under the supervision of a pharmacist, the Pharmacy Technician supports the provision of pharmaceutical care to patients served by the San Francisco Department of Public Health. Assists registered pharmacists in performing nonprofessional duties in an institutional pharmacy; receives, inspects, stores, and fills orders for pharmaceutical supplies. Nature of work involves continuous close attention to detail and some potential exposure to hazardous substances.

Under supervision, the Pharmacy Technician performs the following essential job functions:

• Accurately prepares unit dose medications, compounds a variety of pharmaceuticals under the supervision of a Pharmacist, including intravenous admixtures according to proper aseptic technique and in accordance with, professional standards and departmental policies and procedures, in order to maintain required levels of supply and to provide patients with needed medications;
• Prepares or repackages drugs and pharmaceuticals under the supervision of a Pharmacist, in amounts needed to fill written orders/prescriptions in order to provide patients with needed medication and to maintain required levels of supply;
• Prepares medication labels based on written orders from physicians and other professional staff according to California State Board of Pharmacy requirements;

Qualifications

Minimum Qualifications

1.   Experience: One (1) year (equivalent to 2,000 hours) of experience (in the last 5 years) assisting in the preparation of drug and pharmaceutical items in a licensed pharmacy;

2.   License: Possession of a valid Pharmacy Technician License/Registration issued by the California State Board of Pharmacy. (Please attach a copy)

Role Description

As a Clinical Psychologist for the County Adult Assistance Program(CAAP), you will conduct professional-level work in screening and assessing CalWORKS and CAAP clients to identify and document disabling mental health conditions and associated functional limitations that enable them to qualify for Social Security Benefits.

Essential Duties Include:

• Conducts screening and assessment of clients with mental health related complaints, collateral interviews with primary care and behavioral health staff, and medical records review to support clients’ application for Social Security Benefits.
• Selects, administers, scores, and interprets appropriate psychological tests of intelligence, personality, aptitude, achievement, interest and other specialized areas and conducts diagnostic clinical interviews with emotionally disturbed, mentally ill, chemically dependent, and intellectually disabled adults to assess their impairments to provide medical evidence in support of clients’ application for Social Security Benefits as needed.
• Prepares psychological reports presenting diagnostic findings, interview findings, functional assessments, clinical evaluations, and recommendations for the Clients’ SSI eligibility.
• Reviews and evaluates psychological, psychiatric and/or neuropsychological data and case material.
• Provides education, consultation, and information to community providers, and CAAP’s multi-disciplinary team on adult adjustment problems, mental illness, chemical dependence, culturally relevant services, and other topics related to SSI eligibility.
• Supervises psychology interns and subordinate clinical staff not including class 2574 Clinical Psychologist; conducts staff training programs both within the clinic or unit and for line agency personnel.
