The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

Examples of Duties

- Manage a caseload of 28-32 clients comprised of older individuals with complex medical conditions.

The Position

The Case Manager will perform professional and clinical social work with individuals and/or families who are unhoused or housing insecure, and who may have complex social and medical needs including health care, financial assistance, employment opportunities, as well as housing assistance. As a member of the Human Services Department, this position reports to a Clinical Supervisor, under direction of the division manager of Homeless Care and Response (HCR), and is characterized by the responsibility to provide comprehensive psycho- social assessments and develop and implement care plans. These plans may focus on connecting clients  to public benefits, obtaining budgeting skills, accessing medical care, and obtaining more stable living arrangements. The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits in the company of another staff member in the field to locations where homeless persons may reside,  to medical facilities, and other community locations.

Role Description

As a Clinical Psychologist for the County Adult Assistance Program(CAAP), you will conduct professional-level work in screening and assessing CalWORKS and CAAP clients to identify and document disabling mental health conditions and associated functional limitations that enable them to qualify for Social Security Benefits.

Essential Duties Include:

• Conducts screening and assessment of clients with mental health related complaints, collateral interviews with primary care and behavioral health staff, and medical records review to support clients’ application for Social Security Benefits.
• Selects, administers, scores, and interprets appropriate psychological tests of intelligence, personality, aptitude, achievement, interest and other specialized areas and conducts diagnostic clinical interviews with emotionally disturbed, mentally ill, chemically dependent, and intellectually disabled adults to assess their impairments to provide medical evidence in support of clients’ application for Social Security Benefits as needed.
• Prepares psychological reports presenting diagnostic findings, interview findings, functional assessments, clinical evaluations, and recommendations for the Clients’ SSI eligibility.
• Reviews and evaluates psychological, psychiatric and/or neuropsychological data and case material.
• Provides education, consultation, and information to community providers, and CAAP’s multi-disciplinary team on adult adjustment problems, mental illness, chemical dependence, culturally relevant services, and other topics related to SSI eligibility.
• Supervises psychology interns and subordinate clinical staff not including class 2574 Clinical Psychologist; conducts staff training programs both within the clinic or unit and for line agency personnel.

THE POSITION:

The City of San Rafael Library & Recreation Department, Child Care Division, is accepting applications for Child Care Instructor I to work in our School-Age Programs.  Under the supervision of the Child Care Director or Head Teacher, you will assist in the planning and implementation of daily activities for the school-age program.

The Position

The Human Resources Benefits & Payroll Manager is one of three division level manager positions reporting to the Human Resources Director.  The position is a working manager and has overall responsibility for planning, managing and evaluating the benefits and payroll activities, programs and staff (Human Resources Analysts and Technicians) within the division.  Major responsibilities include, but are not limited to:
- Manage and coordinate a diverse portfolio of medical, dental, vision and other employee benefits plans and programs, including short term and long term disability, flexible spending accounts, basic and supplemental life,  deferred compensation and many other benefits plan options.
- Monitor the developments and legislation related to human resources benefits and payroll-related matters, including evaluating the impact upon City operations, and recommends and implements policy and procedural improvements.

- Lead and engage the benefits and payroll team to update and modernize administrative policies and processes.

The Senior Educational Facilities Planner (Sr. Planner) provides professional planning services that guide the physical development of the campus and assists in strategic development and improvement of buildings and space to support UCSC's mission of research, teaching, and public service. As a senior member of the unit, the Sr. Planner is the resident expert on campus and UC policy regarding capital matters, and provides assistance and support in all areas of planning for CPSM staff, campus leadership. They also act as liaison to the Office of Capital Planning at the UC Office of the President.

The Sr. Planner uses skills as a seasoned, experienced planning professional with a full understanding of industry practices and procedures to resolve a wide range of planning issues; uses advanced planning concepts and objectives to resolve highly complex issues; develops short and long range capital plans, demonstrates good judgment in selecting methods and techniques for analyzing alternatives and recommending solutions; exhibits high levels of organization; recommends revisions to proceedures and policies to improve efficiency, and is able to operate with considerable independence.

The Sr. Planner provides high-level reports and analysis of campus space needs in an effort to ensure the efficient use of campus space; works with campus leadership and clients to provide in-depth programming, utilization and visioning studies for all types of campus space using a variety of planning tools: metrics, conceptual space plans, and test fit diagrams; coordinates with campus technical experts on the re-use of space; generates and manipulates large data sets, manages databases to comprehensively manage campus capital expenditures, leads and coordinates the annual updates to the multi-year Capital and Seismic Financial Plans, and generates analytics for capital planning, space management and capital outlay.

Capital Planning and Space Management (CPSM) is a unit within the Division of Business Administration (BAS.) As a member of BAS, CPSM supports the Office of the Chancellor and Campus Provost/Executive Vice Chancellor in the areas of capital financial planning and policy analysis. The Position

The Human Services Department currently has a part-time Counselor position in the Aging and Family Services (AFS) Division’s Senior Peer Counseling program.  The Counselor position is an integral member of the Senior Peer Counseling program offering supervision and training to the senior peer counselors who in return provide emotional and mental health support to homebound elders in their place of residence. In collaboration with a clinical supervisor and other members of a multidisciplinary team, such as AFS care managers, the Counselor will provide active guidance, leadership and training to the senior peer counselors and limited direct care services to the seniors in home. The Counselor will interact with seniors and their families by conducting in-home assessments, using the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM 5), to develop thoughtful and creative care plans. In addition, the Counselor will provide reports that are due as part of the contract. 

The Ideal Candidate

The ideal candidate will be a Licensed Marriage and Family Therapist, Licensed Clinical Social Worker, Licensed Professional Clinical Counselor, or Licensed Psychologist.  Additionally, the selected candidate will demonstrate excellent customer service; maintain effective working relationships with internal and external customers; exhibit skills in oral and written communication; demonstrate creative problem solving techniques; experience working with older adults and different ethnic and cultural groups; display keen observation and evaluation skills of clients; and supervisory experience and facilitation of groups is highly desirable.

Minimum Qualifications

Any combination of education and experience that would provide the knowledge, skills and abilities required to perform the essential duties of the job would be qualifying.  A typical way to accomplish this includes:

Education: Master’s degree in Psychology, Social Welfare, or comparable field.

Experience:

Two (2) years of progressively responsible experience providing counseling services.

Special Requirements:  This position may be required to work other than business hours and requires the ability to travel independently within and outside of City limits.

Behavioral Health and Recovery Services (BHRS) offers a broad spectrum of mental health and substance use services for children, youth, families, adults and older adults in San Mateo County in the prevention, early intervention and treatment of mental illness and/or substance use conditions. Mental health services are also available for emotionally disturbed children, older adults at risk of institutionalization, children in special education or at risk of out-of-home placement, uninsured residents of San Mateo County and people of any age in a major crisis.
 
Correctional Health Services provides integrated medical, dental, mental health, and chemical dependency treatment to San Mateo County's incarcerated adults, and medical and dental care to the juvenile hall population. Medical services include health screening, treatment, education, and 24-hour emergency response for juveniles and adults.

Public Health Policy and Planning includes Edison HIV Clinic and Mobile Health Clinic/Street and Field Medicine program. Edison Clinic provides comprehensive patient-centered HIV primary care services to persons living with HIV (PLWH) within San Mateo County as well as provides services in diagnosis and treatment of sexually transmitted infections (STIs) to all San Mateo county residents. The Mobile Health Clinic/Street and Field Medicine program provides field-based medical care focusing on homeless, formerly incarcerated, and farmworker/farmworker family member individuals. The program provides medical care in the field at encampments and homeless congregation sites, homeless shelter, peripheral sites outside of San Mateo County Clinics, and on a mobile medical clinic vehicle that parks at scheduled sites throughout the county.

The San Mateo Medical Center is a public hospital and clinic system fully accredited by the Joint Commission. The Medical Center operates outpatient clinics throughout San Mateo County and also an acute-care hospital. We serve the healthcare needs of County residents, serving as the healthcare home for underserved and underinsured residents, with an emphasis on education and prevention. Under general supervision, Nurse Practitioners will provide services such as conducting complete physical examinations, developing diagnostic impressions, and managing care of patients working in one or more specialty areas, such as geriatric, pediatric, family health, or OB/GYN practice.
 
Our nurses are part of a collaborative healthcare team dedicated to providing excellent care to our patients. At the County of San Mateo you'll find a supportive environment and opportunities to grow in your profession through training and education, including in-house Continuing Education units.  Basic Life Support (BLS) Certification preferred.
