San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the position of In-Home Supportive Services Social Worker II/III. Social Workers provide a wide variety of welfare services in such areas as information and referral, protective services, out-of-home services, health related services, chore services, and other welfare services within the Health and the Human Services Agency.
 
The current vacancy is in the In-Home Supportive Services (IHSS) Program that provides in-home care for persons who cannot safely remain in their own home without such assistance. The recipients of IHSS are aged, blind and/or disabled and eligible for Medi-Cal. Disabled and/or blind minor children may also be eligible for IHSS. The Social Worker will make a home visit to determine eligibility for the program and to conduct an assessment to determine a range of service hours for particular tasks. The IHSS recipient or guardian is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive.  The Social Worker will travel throughout the County to conduct home visits, attend meetings and perform other duties of the position.  The Social Worker will work in the San Mateo, CA Aging and Adult Services Office.
 
This IHSS Social Worker will conduct intake and/or on-going activities for IHSS recipients. Some of those activities include:
 

• Conduct initial IHSS assessments using a detailed State assessment tool.
• Complete necessary documents, paperwork, and work within different information technology systems.
• Conduct annual reassessments.
• Educate, support, and perform administrative duties for IHSS recipients and care providers to understand/comply with program regulations.
• Provide education, outreach, and community resources information.
• Participate in quality improvement efforts.
• Collaborate with quality assurance and program integrity to ensure proper adherence to regulations.
• Prepare for and attend Fair Hearings.
• Engage in interdisciplinary team meetings or other activities to support San Mateo County's vision for integrated services.
• Maintain oversight of case management needs for a caseload focused on serving IHSS recipients who are minors.
• Collaborate and facilitate service with care providers and other partner agencies.

 The ideal candidate will possess the following:
 

• Master's degree in Social Work.
• Experience providing clinical assessment for children, youth and adults with disabilities. 
• Understanding of human development and behavior within diverse populations.
• Ability to independently develop and implement sound case plans of action.
• Experience balancing client-centered care coordination and the accompanying administrative documentation.
• Familiarity in implementing program regulations.
• Superior organizational skills to manage a large caseload and related documents.
• Proficient computer skills to use several independent data base systems as well as Microsoft Excel and Word.
• Ability to quickly learn and adopt new technology, policies and procedures.
• Excellent writing/documentation skills.
• Fluency in Cantonese, Mandarin, Tagalog, Vietnamese or Farsi is a plus.

SALARY
Social Worker II:   $6,564.13 - $8,205.60/month
Social Worker III:  $7,741.07 - $9,673.73/month

Social Workers provide highly skilled, intensive casework services to individuals and their families and provide casework consultation within the department and to outside agencies.  Incumbents of positions at this level work with minimal supervision and are expected to have acquired the high degree of assessment skills required to perform care coordination and referral tasks in complex casework areas. Lead supervisory responsibilities may be assigned as circumstances dictate.

NOTE:  The list created from this recruitment may be used to fill future vacancies, including classified, unclassified, term, full-time, part-time, temporary and extra help vacancies. Unclassified positions are at-will and not covered by the San Mateo County Civil Service Rules.  Although the current Aging and Adult Services position is in IHSS, this list may also be used to fill positions in other programs such as Adult Protective Services (APS), Multipurpose Senior Services Program (MSSP) and Linkages; as well as positions in the San Mateo Medical Center and Human Services Agency.

 

Qualifications

Licensure/Certification:  California class C driver license or equivalent.

Education:
Social Worker II: Satisfactory completion of 30 semester units of social welfare and/or appropriate behavioral science courses.

Social Worker III: Satisfactory completion of 36 semester units of social welfare and/or appropriate behavioral science courses.

Experience: 
Social Worker II: Two years experience performing duties comparable to those of a Social Worker I or higher in San Mateo County. Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work.

Social Worker III:  Three years experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.  Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

THE POSITION:

THE POSITION

Tri-City is looking for one (1) individual to serve as a Psychiatrist I/II for the Adult Outpatient Clinic. This position requires demonstrated three (3) years of experience in plant nursery or grounds maintenance or other relevant field work to demonstrate sufficient expertise with plant identification, installation, monitoring, and maintenance.

JOB SPECIFICATIONS

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job. While performing the duties of this job, the employee is regularly required to life and carry items weighing up to 90 pounds for 75 feet over rough terrain multiple times per day, frequently operate light- and medium-duty equipment, work in outdoor weather conditions, frequently move about work sites, communicate and exchange information, observe site conditions, transport equipment, materials, and tools between vehicle and work sites on a daily basis, and occasionally ascend/descend a ladder to access areas of work.

Tuesday through Friday, 10 hours per day, 40 hours per week. Occasionally start/end times may vary. Occasional overtime work upon prior approval by Supervisors. The headquarters of the MRCA Restoration Division is in the Calabasas area at King Gillette Ranch; however, most days employees drive directly to a different job site. Based on vehicle availability, most crew members are assigned a truck and gas card soon after a probationary period.

The majority of MRCA properties are open to the general public. Parking lots, shared public/office restrooms and in-person meeting rooms are open during regular business hours. The MRCA is committed to protecting its staff by ensuring appropriate physical distancing and proper infection control while supporting equitable access to services. The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

- Manage a caseload of 28-32 clients comprised of older individuals with complex medical conditions.

The Position

The Case Manager will perform professional and clinical social work with individuals and/or families who are unhoused or housing insecure, and who may have complex social and medical needs including health care, financial assistance, employment opportunities, as well as housing assistance. As a member of the Human Services Department, this position reports to a Clinical Supervisor, under direction of the division manager of Homeless Care and Response (HCR), and is characterized by the responsibility to provide comprehensive psycho- social assessments and develop and implement care plans. These plans may focus on connecting clients  to public benefits, obtaining budgeting skills, accessing medical care, and obtaining more stable living arrangements. The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits in the company of another staff member in the field to locations where homeless persons may reside,  to medical facilities, and other community locations.

Examples of Duties

- Manage a caseload of clients comprised of individuals with complex medical and co-occurring conditions, including SUD, mental illness, or homelessness.
- Conduct comprehensive client assessments to develop appropriate service plans and monitor them closely.
- Assist clients to develop support systems to maintain independent living, self-sufficiency and family stabilization.
- Provide crisis intervention.
- Collaborate with supervisor, team members and other Human Services programs and community agencies.
- Make site visits and provide other community interventions as needed.
- Conduct inter-agency and/or family conferences.
- Provide supportive counseling and advocacy for clients.
- Monitor services provided for each client.
- Maintain timely electronic documentation of services, reporting, and billing.
- Prepare and present training and educational programs and publicity materials.
- Supervise interns in graduate and undergraduate programs placed at HCR.
- Prepare written and statistical technical reports.
