Job Description

Under general supervision, operates, maintains, troubleshoots, and repairs a wide variety of pumping, heating, ventilating, air conditioning, refrigeration and auxiliary plant machinery and equipment in a large complex or multi-faceted facility. Ensures that all equipment, machinery and systems are functioning normally and safely, and complies with all applicable safety rules and regulations.

MAJOR, IMPORTANT, AND ESSENTIAL DUTIES

1. Operates, maintains and repairs a wide variety of pumping, heating, ventilating, air conditioning, refrigeration, and power generating equipment in a large complex or multi-faceted facility to ensure the safe, correct and efficient functioning of the facility and equipment may be required to operate computerized building management systems, preventative or predictive maintenance computer systems or Computerized Data Gathering System (SCADA).2. Inspects and tests: machinery and equipment for safe, efficient and proper operation by visual and audio inspection during walk-throughs and by standard monitoring; fire prevention equipment to ensure safe and proper functionality; water storage and multiple distribution systems, high pressure zones, wood and steel storage tanks, pumps and relief valves, and samples of water.

3. Makes entries and keeps records on shift operation by taking and entering gauge readings and accurate water laboratory analysis readings and temperatures on operating log sheets or into a computer data system to ensure preventative maintenance and equipment history; and changes charts and recordings accordingly.

4. Receives, records and stores large amounts of treatment chemicals and may handle hazardous chemicals in confined or enclosed areas; must be capable of safely and properly handling chemical spills; may also include handling and treatment of industrial or hazardous waste.

5. Contacts, communicates and is required to deal with contractors, building tenants and/or the general public in order to be able to discharge assigned duties.

6. Maintains keys, communications equipment, and security (in the absence of Security Personnel), in order to ensure the security of a plant and buildings and grounds, including public safety. As part of an emergency response team, recognizes, evaluates, and responds to potentially hazardous situations.

7. Performs control tests on water samples to determine chlorine and fluoride residual, pH, alkalinity, turbidity, color, saturation index and flocculation factors; under supervision, conducts water analysis and regulates treatment chemical dosages to maintain water quality in filtration plant, distribution system, or other large complex facility may also be required to perform water analysis on water/wastewater as required by the State of California; performs boiler water and cooling tower chemical tests, including pH, alkalinity TDS, etc., to monitor and control growth of organisms in water.

San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the position of In-Home Supportive Services Social Worker II/III. Social Workers provide a wide variety of welfare services in such areas as information and referral, protective services, out-of-home services, health related services, chore services, and other welfare services within the Health and the Human Services Agency.
 
The current vacancy is in the In-Home Supportive Services (IHSS) Program that provides in-home care for persons who cannot safely remain in their own home without such assistance. The recipients of IHSS are aged, blind and/or disabled and eligible for Medi-Cal. Disabled and/or blind minor children may also be eligible for IHSS. The Social Worker will make a home visit to determine eligibility for the program and to conduct an assessment to determine a range of service hours for particular tasks. The IHSS recipient or guardian is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive.  The Social Worker will travel throughout the County to conduct home visits, attend meetings and perform other duties of the position.  The Social Worker will work in the San Mateo, CA Aging and Adult Services Office.
 
This IHSS Social Worker will conduct intake and/or on-going activities for IHSS recipients. Some of those activities include:
 

• Conduct initial IHSS assessments using a detailed State assessment tool.
• Complete necessary documents, paperwork, and work within different information technology systems.
• Conduct annual reassessments.
• Educate, support, and perform administrative duties for IHSS recipients and care providers to understand/comply with program regulations.
• Provide education, outreach, and community resources information.
• Participate in quality improvement efforts.
• Collaborate with quality assurance and program integrity to ensure proper adherence to regulations.
• Prepare for and attend Fair Hearings.
• Engage in interdisciplinary team meetings or other activities to support San Mateo County's vision for integrated services.
• Maintain oversight of case management needs for a caseload focused on serving IHSS recipients who are minors.
• Collaborate and facilitate service with care providers and other partner agencies.

 The ideal candidate will possess the following:
 

• Master's degree in Social Work.
• Experience providing clinical assessment for children, youth and adults with disabilities. 
• Understanding of human development and behavior within diverse populations.
• Ability to independently develop and implement sound case plans of action.
• Experience balancing client-centered care coordination and the accompanying administrative documentation.
• Familiarity in implementing program regulations.
• Superior organizational skills to manage a large caseload and related documents.
• Proficient computer skills to use several independent data base systems as well as Microsoft Excel and Word.
• Ability to quickly learn and adopt new technology, policies and procedures.
• Excellent writing/documentation skills.
• Fluency in Cantonese, Mandarin, Tagalog, Vietnamese or Farsi is a plus.

SALARY
Social Worker II:   $6,564.13 - $8,205.60/month
Social Worker III:  $7,741.07 - $9,673.73/month

Social Workers provide highly skilled, intensive casework services to individuals and their families and provide casework consultation within the department and to outside agencies.  Incumbents of positions at this level work with minimal supervision and are expected to have acquired the high degree of assessment skills required to perform care coordination and referral tasks in complex casework areas. Lead supervisory responsibilities may be assigned as circumstances dictate.

NOTE:  The list created from this recruitment may be used to fill future vacancies, including classified, unclassified, term, full-time, part-time, temporary and extra help vacancies. Unclassified positions are at-will and not covered by the San Mateo County Civil Service Rules.  Although the current Aging and Adult Services position is in IHSS, this list may also be used to fill positions in other programs such as Adult Protective Services (APS), Multipurpose Senior Services Program (MSSP) and Linkages; as well as positions in the San Mateo Medical Center and Human Services Agency.

 

Qualifications

Licensure/Certification:  California class C driver license or equivalent.

Education:
Social Worker II: Satisfactory completion of 30 semester units of social welfare and/or appropriate behavioral science courses.

Social Worker III: Satisfactory completion of 36 semester units of social welfare and/or appropriate behavioral science courses.

Experience: 
Social Worker II: Two years experience performing duties comparable to those of a Social Worker I or higher in San Mateo County. Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work.

Social Worker III:  Three years experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.  Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Building Attendant I - This is the entry level class in the Building Attendant series.  Incumbents in this class work under close supervision.  As experience is gained, incumbents may be granted more independence of action under established guidelines.

THE POSITION

Tri-City is looking for one (1) individual to serve as a Psychiatrist I/II for the Adult Outpatient Clinic. The Psychiatrist I/II will provide medical and psychiatric treatment and diagnostic services to clients in the caseload of Tri-City and provide collaborative work with a multidisciplinary team in a public mental health agency. Provide integrated, effective, and high quality behavioral health care to the residents of Pomona, Claremont and La Verne with moderate to severe functional impairment due to mental illness.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Perform intake and diagnostic interviews on a scheduled and emergent basis.
• Participate in diagnostic staff evaluation of clients.
• Provide medical psychiatric treatment of clients.
• Maintain and disseminate medical records and reports in compliance with Federal, State and Agency regulations, including Health Insurance Portability and Accountability Act (HIPAA) and The Health Information Technology for Economic and Clinical Health Act, Public Law 111-005 (“HITECH Act”).
• Submit required statistical and other requested information on a timely basis.
• Consult and provide information to other agencies that are coordinating their work with that of the behavioral health service agency.

The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

Examples of Duties

- Manage a caseload of 28-32 clients comprised of older individuals with complex medical conditions.
- Conduct comprehensive client assessments, and develop and monitor service plan.
- Collaborate with team members, Human Service programs, and community agencies.
- Conduct home visits and provide other community interventions as needed.

The Position

The Case Manager will perform professional and clinical social work with individuals and/or families who are unhoused or housing insecure, and who may have complex social and medical needs including health care, financial assistance, employment opportunities, as well as housing assistance. As a member of the Human Services Department, this position reports to a Clinical Supervisor, under direction of the division manager of Homeless Care and Response (HCR), and is characterized by the responsibility to provide comprehensive psycho- social assessments and develop and implement care plans. These plans may focus on connecting clients  to public benefits, obtaining budgeting skills, accessing medical care, and obtaining more stable living arrangements. The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits in the company of another staff member in the field to locations where homeless persons may reside,  to medical facilities, and other community locations.

Examples of Duties

- Manage a caseload of clients comprised of individuals with complex medical and co-occurring conditions, including SUD, mental illness, or homelessness.
- Conduct comprehensive client assessments to develop appropriate service plans and monitor them closely.
- Assist clients to develop support systems to maintain independent living, self-sufficiency and family stabilization.
- Provide crisis intervention.
- Collaborate with supervisor, team members and other Human Services programs and community agencies.
- Make site visits and provide other community interventions as needed.
- Conduct inter-agency and/or family conferences.
- Provide supportive counseling and advocacy for clients.
- Monitor services provided for each client.
- Maintain timely electronic documentation of services, reporting, and billing.
