This position will perform the following essential duties, including, but not limited to:

THE POSITION

Tri-City is recruiting to fill the position of Quality Assurance Specialist II within our Best Practices Department. The Quality Assurance Specialist II is responsible for facilitating the Agency's Electronic Health Record (EHR) training and carry out duties to maintain Tri-City's Quality Assurance Program in accordance with specified county, state, and federal laws and regulations, including the Health Insurance Portability and Accountability Act (HIPAA).  This position will report to the Quality Assurance Supervisor.

This position will perform the following essential duties, including, but not limited to:

• Assists with maintaining the Authority’s quality assurance program in accordance with federal, state, and local laws and regulations, Authority rules, guidelines and values; develops systems and standards for compliance evaluation; implements training, monitoring, and processes to ensure that Authority activities are in compliance with specified laws; recommends ways to integrate the quality assurance message into all aspects of the Authority’s business and human relations.

• Assists in determining the type and frequency of training needs; collaborates with the Quality Assurance Supervisor in the design and delivery of training; develops job specific training manuals to be used during staff training for each health center implementation and ongoing support.

• Coordinates with the Quality Assurance Supervisor in the implementation of policies and procedures concerning best practices and compliance issues.

• Reviews charts in the Electronic Health Records (EHR) system to monitor clinical staff records and to ensure compliance; trains staff and interns on documentation standards, new processes and on the chart review process.

• Assists with the facilitation of all internal and external quality assurance audits; assists program staff in preparing for audits; assists with collaborating on the implementation of any corrective action as a result of the audit.

• Participates in any needed training, meeting, committee, and compliance groups on monthly or quarterly basis; facilitates quality assurance meetings, workshops, and committees in order to enhance documentation quality.

• Perform other duties as assigned.

The Department

The Human Services Department (HSD) delivers and supports services by forging long-term community partnerships, engaging with and building the capacity of the community to do its own problem-solving, and leveraging financial and volunteer resources. The Department’s nationally and internationally recognized programs support thousands of residents throughout their life course, from infants to elders. The Administration Division of HSD specifically supports a robust grants program that administers federal and local funding that addresses the needs of lower income Fremont residents. The Department receives approximately $2.2 million in annual federal funding from the Department of Housing and Urban Development (HUD) and an additional $800,000 in local funding to administer to non-profit and community agencies that provide vital services to Fremont’s most vulnerable residents.

The Position

The Senior Accounting Specialist is responsible for a variety of advanced level fiscal clerical assignments. This position provides administrative support of federal Community Development Block Grant (CDBG) and HOME Investment Partnership Programs (HOME) programs related to affordable housing and services to low income residents.

Examples of Job Duties

• Prepare and execute contracts for CDBG, ESG, HOME, and SSG Grant Programs.
• Provide contract administration/clerical support, which includes keeping and maintaining detailed records and information required by federal grant programs.
• Prepare and execute purchases orders, process invoices, and keep grant accounting records.
• Provide support of the CDBG Citizen Advisory Committee (CAC) meetings and provide assistance through the coordination of the meeting schedule, preparation of meeting agendas, summary notes of meetings, and other materials (requires evening attendance).
• Assist in the Request for Proposal (RFP) Process for all grant programs and provide assistance in operating Zoom Grants for the intake, review, and final scoring of grant proposal applications.

The position description:

Tri-City Mental Health Authority is looking for to fill multiple vacancies for the Bilingual Behavioral Health Specialist (BHS) for the Adult Services Department. The BHS will be performing central scheduling duties and provide behavioral health support services to individuals as a part of these departments. The BHS will provide support to the team assisting with duties related to network adequacy, data tracking,  processing service requests, screenings, and linkage to mental health services. The BHS will report to their direct supervisor.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Provides individual, group, family, and collateral rehabilitative support services to facilitate behavior modification including skill building, needs assessment, self-care/safety, and coping strategies and techniques; may provide case management services in the office and in the field.

• Develops, monitors, and evaluates client rehabilitative and case management goals and increases/decreases in targeted behavior according to client needs; monitors goal progress/regression.

• Provides linkages, resources, and referrals to address basic needs and other barriers to wellness and recovery including food, housing, clothing, transportation, and other financial resources.

• Encourages and supports clients to fully engage in services; provides outreach and assists clients in addressing and overcoming barriers to treatment.

• Plans and facilitates and/or co-facilitates group sessions for clients and community members focused on skill building; tracks client attendance and follows-up with individual clients, as necessary.

• Provides advocacy for clients to ensure inclusion in all aspects of the community including socialization, employment, education, and housing to meet client needs.

San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the position of In-Home Supportive Services Social Worker II/III. Social Workers provide a wide variety of welfare services in such areas as information and referral, protective services, out-of-home services, health related services, chore services, and other welfare services within the Health and the Human Services Agency.
 
The current vacancy is in the In-Home Supportive Services (IHSS) Program that provides in-home care for persons who cannot safely remain in their own home without such assistance. The recipients of IHSS are aged, blind and/or disabled and eligible for Medi-Cal. Disabled and/or blind minor children may also be eligible for IHSS. The Social Worker will make a home visit to determine eligibility for the program and to conduct an assessment to determine a range of service hours for particular tasks. The IHSS recipient or guardian is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive.  The Social Worker will travel throughout the County to conduct home visits, attend meetings and perform other duties of the position.  The Social Worker will work in the San Mateo, CA Aging and Adult Services Office.
 
This IHSS Social Worker will conduct intake and/or on-going activities for IHSS recipients. Some of those activities include:
 

• Conduct initial IHSS assessments using a detailed State assessment tool.
• Complete necessary documents, paperwork, and work within different information technology systems.
• Conduct annual reassessments.
• Educate, support, and perform administrative duties for IHSS recipients and care providers to understand/comply with program regulations.
• Provide education, outreach, and community resources information.
• Participate in quality improvement efforts.
• Collaborate with quality assurance and program integrity to ensure proper adherence to regulations.
• Prepare for and attend Fair Hearings.
• Engage in interdisciplinary team meetings or other activities to support San Mateo County's vision for integrated services.
• Maintain oversight of case management needs for a caseload focused on serving IHSS recipients who are minors.
• Collaborate and facilitate service with care providers and other partner agencies.

 The ideal candidate will possess the following:
 

• Master's degree in Social Work.
• Experience providing clinical assessment for children, youth and adults with disabilities. 
• Understanding of human development and behavior within diverse populations.
• Ability to independently develop and implement sound case plans of action.
• Experience balancing client-centered care coordination and the accompanying administrative documentation.
• Familiarity in implementing program regulations.
• Superior organizational skills to manage a large caseload and related documents.
• Proficient computer skills to use several independent data base systems as well as Microsoft Excel and Word.
• Ability to quickly learn and adopt new technology, policies and procedures.
• Excellent writing/documentation skills.
• Fluency in Cantonese, Mandarin, Tagalog, Vietnamese or Farsi is a plus.

SALARY
Social Worker II:   $6,564.13 - $8,205.60/month
Social Worker III:  $7,741.07 - $9,673.73/month

Social Workers provide highly skilled, intensive casework services to individuals and their families and provide casework consultation within the department and to outside agencies.  Incumbents of positions at this level work with minimal supervision and are expected to have acquired the high degree of assessment skills required to perform care coordination and referral tasks in complex casework areas. Lead supervisory responsibilities may be assigned as circumstances dictate.

NOTE:  The list created from this recruitment may be used to fill future vacancies, including classified, unclassified, term, full-time, part-time, temporary and extra help vacancies. Unclassified positions are at-will and not covered by the San Mateo County Civil Service Rules.  Although the current Aging and Adult Services position is in IHSS, this list may also be used to fill positions in other programs such as Adult Protective Services (APS), Multipurpose Senior Services Program (MSSP) and Linkages; as well as positions in the San Mateo Medical Center and Human Services Agency.

 

Qualifications

Licensure/Certification:  California class C driver license or equivalent.

Education:
Social Worker II: Satisfactory completion of 30 semester units of social welfare and/or appropriate behavioral science courses.

Social Worker III: Satisfactory completion of 36 semester units of social welfare and/or appropriate behavioral science courses.

Experience: 
Social Worker II: Two years experience performing duties comparable to those of a Social Worker I or higher in San Mateo County. Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work.

Social Worker III:  Three years experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.  Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Desirable Experience: Experience in high level complex, high volume recruitments, and compensation analysis. The Position

Under the direction of the Youth and Family Services Administrator, the Senior Program Coordinator plays a unique role overseeing the early childhood mental health program, providing direct counseling services and supervising several staff counselors and interns.

The Senior Program Coordinator typically has specialized experience, training and qualifications that enable the position to work effectively with children 0-8 years, parents and youth caregivers.

The Senior Program Coordinator works closely with the division’s Clinical Supervisors and Quality Assurance Specialist to ensure that clinical services comply with all policies, procedures and legal regulations, and that client services are delivered at the highest standards. The position also coordinates with other key positions in the Human Services Department to help clients have easy and timely access to coordinated services throughout the Human Services Department and/or with other partner agencies.

Examples of Duties

- Oversee and coordinate the direct services under the early childhood mental health program, working closely with the counselors and interns assigned to the program.
- Provide administrative and clinical supervision and consultation to YFS counselors and interns/trainees who are providing direct counseling services to children 0-8 years, parents, and family members, while maintaining close cooperation with the Youth and Family Services Division’s Clinical Supervisors and Quality Assurance Specialist.
- Provide direct counseling and early childhood consultation services to children 0-8 years, parents, and family members.
- Together with the YFS Administrator, understand policies and trends related to the provision of early childhood mental health services; and represent the division at County and/or school district meetings.
- Ensure direct service staff and interns are familiar with and fully utilize client Electronic Health Record System and becomes a key user representative on system implementation team.
- Oversee clinical documentation and compliance with State Medi-Cal documentation standards in partnership with Clinical Supervisors and Quality Assurance Specialist.
- Manage the client intake and referral process for the early childhood mental health program.
- Ensure that staff meet contractual and quality assurance goals and audit requirements.
- In partnership with YFS Administrator, Clinical Supervisors, and the Human Services Department’s Fund Development Officer respond to requests for proposals, develop grant work plans, narratives, and budgets.

The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

The Position

The Case Manager will perform professional and clinical social work with individuals and/or families who are unhoused or housing insecure, and who may have complex social and medical needs including health care, financial assistance, employment opportunities, as well as housing assistance. As a member of the Human Services Department, this position reports to a Clinical Supervisor, under direction of the division manager of Homeless Care and Response (HCR), and is characterized by the responsibility to provide comprehensive psycho- social assessments and develop and implement care plans. These plans may focus on connecting clients  to public benefits, obtaining budgeting skills, accessing medical care, and obtaining more stable living arrangements. The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits in the company of another staff member in the field to locations where homeless persons may reside,  to medical facilities, and other community locations.

Examples of Duties

- Manage a caseload of clients comprised of individuals with complex medical and co-occurring conditions, including SUD, mental illness, or homelessness.
- Conduct comprehensive client assessments to develop appropriate service plans and monitor them closely.
- Assist clients to develop support systems to maintain independent living, self-sufficiency and family stabilization.
- Provide crisis intervention.
