Medical Services Assistants II who are assigned to the clinics and perform advanced patient care duties as defined in the classification specification shall receive a differential of 6.2% in addition to all other compensation.

Qualifications

Certification Requirement:
Possession of a Medical Assistant Certificate or a certificate of completion or degree from an accredited medical assisting program is required. A certificate of completion of an approved medical assisting program must be submitted at the time of application.

Bilingual Fluency:
Some positions may require bilingual skills. Fluency in both English and Spanish is required to be considered for a bilingual position.

Education and Experience:
Any combination of education and experience that would likely provide the required knowledge, skills and abilities is qualifying.  A typical way to qualify is:

The City of San Jose, Animal Care and Services Division is seeking two (2) full-time Animal Shelter Veterinarians and multiple part-time Animal Shelter Veterinarians to work at the Animal Care Center. The Animal Shelter Veterinarian may perform all aspects of shelter medicine in a large animal shelter. Duties may include routine spay/neuter surgeries on cats, dogs, and other animals as indicated. THE POSITION

Tri-City is looking for one (1) individual to serve as a Psychiatrist I/II for the Adult Outpatient Clinic. The Psychiatrist I/II will provide medical and psychiatric treatment and diagnostic services to clients in the caseload of Tri-City and provide collaborative work with a multidisciplinary team in a public mental health agency. Provide integrated, effective, and high quality behavioral health care to the residents of Pomona, Claremont and La Verne with moderate to severe functional impairment due to mental illness.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Perform intake and diagnostic interviews on a scheduled and emergent basis.
• Participate in diagnostic staff evaluation of clients.
• Provide medical psychiatric treatment of clients.

The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

The Position

The Case Manager will perform professional and clinical social work with individuals and/or families who are unhoused or housing insecure, and who may have complex social and medical needs including health care, financial assistance, employment opportunities, as well as housing assistance. As a member of the Human Services Department, this position reports to a Clinical Supervisor, under direction of the division manager of Homeless Care and Response (HCR), and is characterized by the responsibility to provide comprehensive psycho- social assessments and develop and implement care plans. These plans may focus on connecting clients  to public benefits, obtaining budgeting skills, accessing medical care, and obtaining more stable living arrangements. The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits in the company of another staff member in the field to locations where homeless persons may reside,  to medical facilities, and other community locations.

Job Description

Under the supervision of a pharmacist, the Pharmacy Technician supports the provision of pharmaceutical care to patients served by the San Francisco Department of Public Health. Assists registered pharmacists in performing nonprofessional duties in an institutional pharmacy; receives, inspects, stores, and fills orders for pharmaceutical supplies. Nature of work involves continuous close attention to detail and some potential exposure to hazardous substances.

Under supervision, the Pharmacy Technician performs the following essential job functions:

• Accurately prepares unit dose medications, compounds a variety of pharmaceuticals under the supervision of a Pharmacist, including intravenous admixtures according to proper aseptic technique and in accordance with, professional standards and departmental policies and procedures, in order to maintain required levels of supply and to provide patients with needed medications;
• Prepares or repackages drugs and pharmaceuticals under the supervision of a Pharmacist, in amounts needed to fill written orders/prescriptions in order to provide patients with needed medication and to maintain required levels of supply;
• Prepares medication labels based on written orders from physicians and other professional staff according to California State Board of Pharmacy requirements;

Minimum Qualifications

1.   Experience: One (1) year (equivalent to 2,000 hours) of experience (in the last 5 years) assisting in the preparation of drug and pharmaceutical items in a licensed pharmacy;
