The Position 

The San Bruno Police Department is recruiting for Public Safety Dispatchers. The San Bruno Police Department has a long-standing tradition of small-town service with big-city dedication to public safety.  Public Safety Dispatchers are responsible for public safety dispatch and related records management activities. This position operates on a 24-hour, 365 days per year basis, and may be assigned to either a day, swing, or graveyard shift on a regular, rotational basis that includes nights, weekends, and holidays.  Public Safety Dispatchers perform a wide range of duties involving radio, telephone, and computers, applying various procedures and codes. Dispatchers receive and transmit emergency and non-emergency telephone and radio traffic as well as process communication reports, records, and radio logs, while maintaining a high level of confidentiality.  Dispatchers may also support the Police Department by performing clerical tasks, training, assisting with fingerprinting, and being accountable for financial transactions.  This position is integral in supporting the goals and objectives of the City and Police Department.   

Essential Duties 

• Receives and transmits emergency telephone and radio traffic.

• Operates a dispatch console, uses Computer Aided Dispatch (C.A.D.) System and other computer programs to process information and records.

San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the position of In-Home Supportive Services Social Worker II/III. Social Workers provide a wide variety of welfare services in such areas as information and referral, protective services, out-of-home services, health related services, chore services, and other welfare services within the Health and the Human Services Agency.
 
The current vacancy is in the In-Home Supportive Services (IHSS) Program that provides in-home care for persons who cannot safely remain in their own home without such assistance. The recipients of IHSS are aged, blind and/or disabled and eligible for Medi-Cal. Disabled and/or blind minor children may also be eligible for IHSS. The Social Worker will make a home visit to determine eligibility for the program and to conduct an assessment to determine a range of service hours for particular tasks. The IHSS recipient or guardian is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive.  The Social Worker will travel throughout the County to conduct home visits, attend meetings and perform other duties of the position.  The Social Worker will work in the San Mateo, CA Aging and Adult Services Office.
 
This IHSS Social Worker will conduct intake and/or on-going activities for IHSS recipients. Some of those activities include:
 

• Conduct initial IHSS assessments using a detailed State assessment tool.
• Complete necessary documents, paperwork, and work within different information technology systems.
• Conduct annual reassessments.
• Educate, support, and perform administrative duties for IHSS recipients and care providers to understand/comply with program regulations.
• Provide education, outreach, and community resources information.
• Participate in quality improvement efforts.
• Collaborate with quality assurance and program integrity to ensure proper adherence to regulations.
• Prepare for and attend Fair Hearings.

Social Worker II:   $6,564.13 - $8,205.60/month

Social Workers provide highly skilled, intensive casework services to individuals and their families and provide casework consultation within the department and to outside agencies.  Incumbents of positions at this level work with minimal supervision and are expected to have acquired the high degree of assessment skills required to perform care coordination and referral tasks in complex casework areas. Lead supervisory responsibilities may be assigned as circumstances dictate.

THE POSITION

Tri-City is looking for one (1) individual to serve as a Psychiatrist I/II for the Adult Outpatient Clinic. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

Examples of Duties

- Manage a caseload of 28-32 clients comprised of older individuals with complex medical conditions.
- Conduct comprehensive client assessments, and develop and monitor service plan.
- Collaborate with team members, Human Service programs, and community agencies.
- Conduct home visits and provide other community interventions as needed.
- Conduct inter-agency and/or family conferences.
- Advocate for clients in accessing services.
- Monitor services provided for each client.
- Maintain timely electronic documentation of services, reporting, and billing.
- Participate in peer chart audits and outside audit preparation.
- Prepare and present training, outreach, and educational sessions and materials.
- Supervise interns in graduate and undergraduate programs.
- Prepare written and statistical technical reports.
