The Position

Under the direction of the Youth and Family Services Administrator, the Senior Program Coordinator plays a unique role overseeing the early childhood mental health program, providing direct counseling services and supervising several staff counselors and interns.

The Senior Program Coordinator typically has specialized experience, training and qualifications that enable the position to work effectively with children 0-8 years, parents and youth caregivers.

The Senior Program Coordinator works closely with the division’s Clinical Supervisors and Quality Assurance Specialist to ensure that clinical services comply with all policies, procedures and legal regulations, and that client services are delivered at the highest standards. The position also coordinates with other key positions in the Human Services Department to help clients have easy and timely access to coordinated services throughout the Human Services Department and/or with other partner agencies.

Examples of Duties

- Oversee and coordinate the direct services under the early childhood mental health program, working closely with the counselors and interns assigned to the program.
- Provide administrative and clinical supervision and consultation to YFS counselors and interns/trainees who are providing direct counseling services to children 0-8 years, parents, and family members, while maintaining close cooperation with the Youth and Family Services Division’s Clinical Supervisors and Quality Assurance Specialist.
- Provide direct counseling and early childhood consultation services to children 0-8 years, parents, and family members.
- Together with the YFS Administrator, understand policies and trends related to the provision of early childhood mental health services; and represent the division at County and/or school district meetings.
- Ensure direct service staff and interns are familiar with and fully utilize client Electronic Health Record System and becomes a key user representative on system implementation team.
- Oversee clinical documentation and compliance with State Medi-Cal documentation standards in partnership with Clinical Supervisors and Quality Assurance Specialist.
- Manage the client intake and referral process for the early childhood mental health program.
- Ensure that staff meet contractual and quality assurance goals and audit requirements.
- In partnership with YFS Administrator, Clinical Supervisors, and the Human Services Department’s Fund Development Officer respond to requests for proposals, develop grant work plans, narratives, and budgets.
- Oversee early childhood grant deliverables and reporting for the division.

The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

Examples of Duties

- Manage a caseload of 28-32 clients comprised of older individuals with complex medical conditions.
- Conduct comprehensive client assessments, and develop and monitor service plan.
- Collaborate with team members, Human Service programs, and community agencies.
- Conduct home visits and provide other community interventions as needed.
- Conduct inter-agency and/or family conferences.
- Advocate for clients in accessing services.
- Monitor services provided for each client.
- Maintain timely electronic documentation of services, reporting, and billing.
- Participate in peer chart audits and outside audit preparation.
- Prepare and present training, outreach, and educational sessions and materials.
- Supervise interns in graduate and undergraduate programs.
- Prepare written and statistical technical reports.

The Position

The Case Manager will perform professional and clinical social work with individuals and/or families who are unhoused or housing insecure, and who may have complex social and medical needs including health care, financial assistance, employment opportunities, as well as housing assistance. As a member of the Human Services Department, this position reports to a Clinical Supervisor, under direction of the division manager of Homeless Care and Response (HCR), and is characterized by the responsibility to provide comprehensive psycho- social assessments and develop and implement care plans. These plans may focus on connecting clients  to public benefits, obtaining budgeting skills, accessing medical care, and obtaining more stable living arrangements. The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits in the company of another staff member in the field to locations where homeless persons may reside,  to medical facilities, and other community locations.

Examples of Duties

- Conduct comprehensive client assessments to develop appropriate service plans and monitor them closely.
- Assist clients to develop support systems to maintain independent living, self-sufficiency and family stabilization.

- Collaborate with supervisor, team members and other Human Services programs and community agencies.
- Make site visits and provide other community interventions as needed.
- Conduct inter-agency and/or family conferences.
- Provide supportive counseling and advocacy for clients.
- Monitor services provided for each client.
- Maintain timely electronic documentation of services, reporting, and billing.
- Prepare and present training and educational programs and publicity materials.
- Supervise interns in graduate and undergraduate programs placed at HCR.
- Prepare written and statistical technical reports.

DEFINITION: Under general supervision, provide personal fitness training to clients requesting service; maintain personal schedule at the front desk; enforce all rules and regulations pertaining to the safety and well being of all patrons; keep fitness floor tidy and clean. Perform related work as required, may include light cleaning and janitorial.

EXAMPLES OF DUTIES:

• Train clients safely according to current industry standards and training.
• Offer and conduct free fitness floor orientations to new customers during Fitness Floor shifts.
• Learn basic operation instruction for fitness equipment and teach these techniques to visitors. Provide corrective feedback and proper technique.
• Maintain certain class records and reports as required.
• Ensuring that a safe and healthy environment exists for exercising. Monitor guests’ activities in the fitness center.
• Enforce all rules and regulations.
• Recognize and respond professionally to emergencies, provide care as indicated under the scope of training.
• Assume assigned duties connected with care and maintenance of the facility.
• Provide basic maintenance, equipment care.
• Deal tactfully and courteously with the public.
• Establish and maintain effective working relationships with other city employees, other agencies, and the general public.
• Actively attend and participate in required staff training and in-service drills.
• Ability to communicate clearly both orally and in writing.
• The ability to speak English clearly and understandably.
• Perform related duties as required.

TRAINING & EXPERIENCE: Certified Personal Trainer required. Additionally, combination of education, training, and experience, that provides the required knowledge and abilities.  

PRE-EMPLOYMENT CONDITIONS: Candidates receiving a conditional job offer must pass a pre-placement fingerprint review. The employee must satisfactorily complete an evaluation period before obtaining regular status. Candidates must possess a current CPR/AED and First Aid certificate and be comfortable calling 911 in an emergency.

Role Description

As a Clinical Psychologist for the County Adult Assistance Program(CAAP), you will conduct professional-level work in screening and assessing CalWORKS and CAAP clients to identify and document disabling mental health conditions and associated functional limitations that enable them to qualify for Social Security Benefits.

Essential Duties Include:

• Conducts screening and assessment of clients with mental health related complaints, collateral interviews with primary care and behavioral health staff, and medical records review to support clients’ application for Social Security Benefits.
• Selects, administers, scores, and interprets appropriate psychological tests of intelligence, personality, aptitude, achievement, interest and other specialized areas and conducts diagnostic clinical interviews with emotionally disturbed, mentally ill, chemically dependent, and intellectually disabled adults to assess their impairments to provide medical evidence in support of clients’ application for Social Security Benefits as needed.
• Prepares psychological reports presenting diagnostic findings, interview findings, functional assessments, clinical evaluations, and recommendations for the Clients’ SSI eligibility.
• Reviews and evaluates psychological, psychiatric and/or neuropsychological data and case material.
• Provides education, consultation, and information to community providers, and CAAP’s multi-disciplinary team on adult adjustment problems, mental illness, chemical dependence, culturally relevant services, and other topics related to SSI eligibility.
• Supervises psychology interns and subordinate clinical staff not including class 2574 Clinical Psychologist; conducts staff training programs both within the clinic or unit and for line agency personnel.

This is a permanent at-will position with the City Manager’s Office reporting to the Director of the Office of Racial Equity. Bilingual in Spanish or Vietnamese is preferable but not required.

The City of Redwood City is looking for individuals interested in working on an on-call basis providing transportation for seniors attending functions at the Veterans Memorial Senior Center plus working with staff on any programmatic needs assistance.  Hours will vary based on Center needs and will not exceed 10-20 hours per week.

Fitness Floor Shift times: Monday-Friday 3:30-6:30pm

• Maintain certain fitness center records and reports as required

• Respond professionally to emergencies
• Perform related duties as required

TRAINING & EXPERIENCE: Any combination of education, training, and experience that provides the required knowledge, abilities and CPR within 3 months of hire.

The Newark Recreation and Community Services Department seeks motivated, self-driven individuals passionate about working with the public. Customer Service Representative involves substantial contact with customers, program contractors, instructors, and staff, working in the café or as a birthday party host.  Various tasks may be assigned, from the front desk to facility setup/clean up. 

This position requires you to work at our indoor water park and recreation center.

You must be able to work 15-20 hours a week between 5:30 am and 9:30 pm.
