Enhanced Lateral Supplemental

This supplemental questionnaire is designed to obtain additional information about your education, training, and experience as it related to the position of Police Officer. Resumes will not be accepted in lieu of this form. This form must be completed in its entirety. Incomplete applications will not be considered.
By submitting this supplemental application, the applicant agrees and understands that the City of Dixon or its agents may investigate the applicant’s background to ascertain any and all information of concern to applicant’s record. The applicant certified that this application was completed by the applicant and that all entries on it and information on it are true and complete to the best of applicant’s knowledge.

1. Are you a U.S. Citizen or resident alien and can you provide proof that you have applied for citizenship as required by California Government Code Section 1031 (a)?

2. Do you possess a valid California Driver's License? If yes, you MUST obtained a copy of your Driving Record from an official California DMV office and submit to your application by PDF.

3. A satisfactory driving record is a condition of initial and continued employment. Please explain any moving violations (moving violations for which there is a factual finding of innocence shall not be included), involvement as a driver in chargeable collisions, conviction of driving under the influence of alcohol and/or drugs, or suspension of driver's license. Failure to explain shall be disqualifying.

4. Are you currently employed as an active-duty, full-time peace officer? If yes, please list the agency(ies) where you have served or are serving as a sworn officer. .

5. Will you be able to provide a copy of an Intermediate POST Training Certificate or higher (Advanced preferred.) with your applications materials?

6. Have you worked for the City of Dixon within the last three (3) years.

7. Are you a graduate of a Police Academy outside of the State of California? If yes, you must submit appropriate POST documentation with your application for further consideration.
