The purpose of this questionnaire is to assess your qualifications. Please answer the questions completely. Upload onto your CalOpps online application the copies of your POST Academy Graduate certificate or a letter from the Academy verifying current enrollment. Please upload your documentation onto CalOpps. An application without uploading your POST certificate or a letter from the Academy will NOT be considered.

For those who wish to claim Veteran's preference, upload a copy of your form DD214 onto your online application. Your DD214 should have an honorably discharged status and with a discharged date of within 10-year period prior to date of your application.

For Academy Graduates/Attendee Applicants: In addition to the minimum qualifications, do you possess a Certified POST Basic Course Certificate of at least 800 hours issued within 3 years of the date of your application? If yes, please upload a copy of this certificate. OR, are you currently enrolled in a POST Basic Academy as a "non-affiliate," of at least 800 hours? If yes, please upload a letter from the academy verifying your current enrollment. Please indicate below whether you are a graduate or an attendee.

4. Information Only: A copy of your POST Basic Course Certificate of at least 800 hours issued within 3 years OR a letter from the Police Academy MUST be uploaded/submitted with your online application. Failure to do so will subject you to disqualification and your application will not be considered any further.
