San Mateo County Health's Public Health, Policy and Planning Division has exciting field-based opportunities for well-qualified and motivated Nurse Practitioners to provide both primary and urgent care to adults from vulnerable populations within our community—people experiencing homelessness and/or in shelters, monolingual Spanish speakers and other new immigrants, and those without insurance or underinsured. These services are provided through various Mobile Clinic modalities and venues, including but not limited to street medicine, shelter medicine, and the mobile coach. We are looking for collaborative Nurse Practitioners to work in and lead various multidisciplinary service teams in the field to provide comprehensive, low-threshold, client-centered care and engagement.

Teams are staffed by a Nurse Practitioner (NP), Psychiatrist, Medical Services Assistant, AOD Case Manager, and Patient Services Assistants. These teams work closely with our community partners from the Human Services Agency and community-based organizations who locate and identify clients needing medical and/or mental health services, as well as provide additional health, housing, and social services in the field.

Under general supervision, Nurse Practitioners provide urgent care and primary health care services such as physical examinations, chronic disease management (e.g., diabetes and hypertension), and referrals/linkages to specialty clinics as appropriate.

Currently, Public Health Clinics has one full-time, regular, 40-hour-per-week vacancy for a bilingual Spanish Nurse Practitioner and two 40-hour-per-week, Limited-Term Nurse Practitioner vacancies, requiring flexible schedules and field-based work at different locations. Sixty-five percent of the NP’s time will be in the field, with the remainder spent in the office documenting notes, placing orders, and coordinating with other staff as needed.

Examples of Duties

Duties may include, but are not limited to, the following:

• Provide low-threshold, comprehensive care for San Mateo County’s most marginalized and vulnerable populations, including people experiencing homelessness, those in shelters, migrant and immigrant farmworkers, the recently incarcerated, the uninsured, and those with financial challenges.

• Provide comprehensive and urgent care in a variety of non-traditional settings, including homeless encampments, outdoor locations, shelters, and a mobile RV clinic.

• Demonstrate the ability to work closely and collaboratively with a small team in often challenging environments (e.g., homeless encampments, shelters, extreme weather, individuals under the influence, or those with mental health challenges).

• Take stewardship of medical care for assigned geographic populations and/or shelters.

• Outreach to and build long-term, trust-based relationships with vulnerable populations.

• Assess patients’ physical, emotional, and social status through medical history, physical examinations, and diagnostic screenings; evaluate clinical findings and lab results.

• Identify and manage common illnesses and other health problems.

• Refer urgent issues beyond scope to appropriate specialists.

• Deliver longitudinal care in assigned areas.

• Collaborate with other health professionals following protocols for diagnosis and treatment.

• Build relationships with community partners to improve health outcomes and expand services.

• Counsel families regarding health maintenance and care.

• Coordinate referrals to other health or social agencies.

• Maintain accurate records of assessments, follow-ups, and evaluations.

• Supervise subordinate health workers and participate in meetings.

• Actively participate in quality improvement efforts.

• Perform related duties as assigned.

Qualifications

Education and Experience
Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying.

• Education: Completion of an advanced nursing degree from a program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC), with emphasis in a Nurse Practitioner specialty area.

• Experience: Minimum of one (1) year of paid experience in an acute and/or ambulatory care setting.

Licensure

• California Registered Nurse license

• California Board of Registered Nursing (BRN) Nurse Practitioner license

• California BRN Furnishing Number

• DEA Number

For OB-GYN assignments, a Nurse Practitioner license may be substituted with a Nurse Midwife License and at least one certification in women’s health.

In 2014, the County of San Mateo established a Fellowship position in honor of former San Mateo County Counsel Michael P. Murphy. Mr. Murphy served in the County Counsel’s office from 1982 to 2011, the last four years as County Counsel. Vacancies in Bay Area county counsel offices typically attract many qualified applicants and new attorneys are generally not considered for these positions. Attorneys in the San Mateo County Attorney’s Office (previously known as the County Counsel’s Office) are generalists. ABOUT THE POSITION
The City of Gilroy is recruiting to fill the position of Senior Facilities Maintenance Specialist in the Administrative Services & Human Resources/Risk Management Department, Facilities Division.

You'll have the opportunity to lead initiatives that impact the community directly, such as energy efficiency upgrades, safety improvements, and facility renovations. These tasks include a range of semi-skilled construction and repair work involving the use of basic tools and trade-related equipment. Duties may involve repairing electrical equipment and wiring systems; fixing windows, doors, cabinets, and other building components; maintaining interior and exterior furniture such as tables, chairs, benches, and bookcases; performing general plumbing repairs and installations; and carrying out basic concrete and masonry work. 

Additionally, you must be able to diagnose mechanical problems and methodically troubleshoot any issues occurring with building systems, machines, and equipment. This role also serves as a trainer and provides leadership to the team on specific tasks and projects.

The Essentials:

Cross-Connection Control Specialist

• Two years of experience in water system maintenance, equivalent to the responsibilities of a Water Utility Worker in the City of Mountain View
• Equivalent to completion of the 12th grade supplemented by approved courses of instruction in the theory, laws, regulations, and techniques of backflow prevention and/or cross-connection control.
• Possession of a valid California Class C driver’s license.
• Possession of an AWWA Backflow Prevention Assembly Tester Certificate and a California Water Resources Control Board Water Distribution Operator Grade 1 Certificate.  
• Possession of an AWWA Cross-Connection Control Specialist Certificate within one year of appointment.  

Bonus Points:

• Experience administering a water system cross-connection inspection program is highly desirable.
• Completion of a recycled water site supervisor’s training class is highly desirable.
