THE POSITION

Tri-City is looking for one individual to serve as a Mental Health Worker (MHW) for the Adult Full Service Partnership (FSP) Program. The MHW will appeal to those who are passionate about working with adults with moderate or severe behavioral health challenges at their home, the shelter or elsewhere.  This is a field-based position.  The MHW will report to a Program Supervisor.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Assist professional staff by performing routine support services; act as a liaison between department staff, the community and clients;
   
• Obtaining important information about client background and tracking services provided on a case-by-case or group basis;
   
• Identifying client needs and assisting in meeting needs through referral to various Tri-City services that are available;
   
• Work as a partner to guide and provide advocacy and other supportive services; work directly with client’s and family members;
   
• Conducts formal and informal meetings to provide information to individuals regarding problems involved in daily living and/or job search; interviews clients to gather basic information;
   
• Responsible for non-billable activities such as non-clinical outreach caseload, group development, implementation and follow-up;
   
• Ensure and comply with PHI and HIPAA requirements; complete necessary documentation, data entry and participate in a multidisciplinary team; perform other duties as assigned, etc.

THE POSITION

Tri-City is looking for one (1) Mental Health Specialist (MHS) whom will provide mental health support services to mentally ill and emotionally disturbed person as part of the Adult Full Service Partnership (FSP) Program. The MHS will appeal to individuals who are inspired by presented in working with an engaged community by providing participant advocate and case management services to moderate to severe mentally ill adults and is a field based position.  The MHS will report to the Program Supervisor.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Plan and provide a variety of rehabilitative support services to mentally ill and emotionally disturbed persons to attain, restore, or improve physical, mental, or emotional functioning;
   
• Provide assistance to clinical staff; participate in group and individual support programs;
   
• Serve as a participant advocate; provide case management services;  the ability to work in the field to service clients at their home, shelter or elsewhere;
   
• Maintain records of all activities relating to client care and adhere to  billing compliancy standard;
   
• Perform other duties as assigned, etc.

Enterprise Resource Planning System (ERP) Project Manager 

Full Time - 2 Year Term Limited Assignment

The City of Sunnyvale is recruiting for an Enterprise Resource Planning System (ERP) Project Manager (Term Limited).  Under direction from the Executive Sponsor (Assistant City Manager) the ERP Project Manager will work with the ERP Steering Committee, the ERP Project Team, and the City's contract Systems Integrator to plan, coordinate, oversee, and manage the implementation of the Oracle Fusion Cloud Financials and Human Capital Management (HCM) suite.

The minimum qualifications for education and experience can be met in the following way:

A Bachelor's degree from an accredited college or university in computer science, information technology, business management, information systems or a related field; AND

Five years of technical or professional level experience, including two years of project management experience and some lead or supervisory experience.

THE POSITION

Tri-City is looking for one innovative and ambitious individual to serve as a Wellness Advocate Manager coordinating peer services throughout Tri-City system of care.  This position will be responsible to plan, coordinate, implement activities that promote peer/family involvement in order to improve access to care, reduce local area disparities.   The Wellness Advocate Manager will report to the Executive Director.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Create a pathway for persons with lived experience to volunteerism and/or employment via training and support; Will stay abreast of legislation and best practices in peer services and advise Tri-City administration on policy and protocols;

• Establishes, leads and maintains an active community-wide Peer Advisory Committee for Tri-City consumers, families/caregivers, and allies.

• Articulates and advocates for the mental health consumer and family/caregiver perspective across all levels of the organization; and makes consumer- and family/caregiver-oriented recommendations to the Agency administration on current and proposed policies, programs, and procedures throughout the entire Tri-City system of care;

• Develops, implements, coordinates and manages strategies aimed at meeting the needs of consumers and family/caregivers for information, education, support, and empowerment;

• Oversees the planning, development, and implementation of a comprehensive system for consumer and family/caregiver involvement within the Agency ranging from services recipient to paid employment; and other duties as assigned.

THE POSITION

Tri-City is looking for one individual to serve as an Occupational Therapist for the Child and Family Clinic. The Occupational Therapist will appeal to those who are passionate about working with children with functional impairments in conjunction with mental health conditions.  This position will be responsible for providing a full range of occupational therapy services to children and transitional age youth and will report to a Program Supervisor.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Evaluation, planning and providing occupational therapy treatment to children and transitional age youth. Plan, organize, and implement individual and group occupational therapy programs.
   
• Work as part of a mental health team and provide services to maximize reduction in mental health related difficulties in children and transitional age youth within a family treatment setting.

• Provide therapeutic techniques, including prevocational exploration and sensory integration, commonly used in occupational therapy.

• Use data to develop a plan of care and goals per child and transitional age youth needs; including short and long‐term goals and methods to achieve identified goals.

• Select treatment methods and techniques that are compatible with child and adolescent clients' needs and capacities by assigning activities of interest which will develop desired motions, responses, attitudes, behaviors, or mental function; treatment programs may include individual and group activities such as manual arts, crafts, games, music, exercises, and sensory-motor integration, and practice in functional, pre-vocational, homemaking skills and activities of daily living and other duties as assigned.  

• Maintain and disseminate medical records and reports in compliance with local, county, federal, State, and TCMHA regulations including Health Insurance Portability and Accountability Act (HIPAA) and The Health Information Technology for Economic and Clinical Health Act, Public Law 111-005 (“HITECH Act”).
   
• Maintain Service Standard Hours in accordance with TCMHA requirements.

• Maintain records of all activities relating to clients’ care; complete clinical documentation in EHR   and compile periodic reports; and confer with supervisor on formulation of policies and procedures.
