The City of Sunnyvale’s Department of Public Works is recruiting for two Senior Maintenance Worker positions within the Street Operations division. One Senior Maintenance Worker position will be assigned to the pavement team responsible for maintenance, repair, sweeping and cleaning of City streets, curbs, gutters, and similar right-of-way infrastructure. One Senior Maintenance Worker position will be assigned to the traffic markings and signs team responsible for a variety of tasks involving installing and maintaining traffic signs, painting pavement markings and striping, and repairing concrete sidewalk for the city. San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

The current vacancy is an on-site, Full-Time, Regular position. In addition to the regular vacancy, we also have an Extra-Help vacancy on site. Experience with psychiatric discharge planning is strongly preferred.

The ideal candidate will be:

• An experienced social worker with the ability to plan and execute complex treatment plans in a community- or acute care hospital-based setting.

Examples Of Duties

Duties may include, but are not limited to the following:

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.
• Assist patients in meeting a variety of emergency needs, including financial, housing, and emotional crises.
• Assist patients with applications and securing of private and public benefits.
• Gather and maintain current information on community resources and disseminate this information to patients and staff.

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

• Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Licensure/Certification :

• California class C driver license or equivalent.

Job Description

Under direct supervision, the Development Strategy & Finance Sr. Analyst works within the Port’s Real Estate and Development Division leading analytical work related to the Port’s Public-Private Partnership Development Projects. Key tasks will include preparing complex financial analyses and reports, analyzing development pro formas and appraisals, coordinating the issuance and disbursement of bonds and associated record keeping documentation, supporting with development negotiations, and managing division-wide contracts, billings, and budgets.

The Sr. Analyst will support Project Managers with all aspects of Port Development projects. The role will receive support in learning and performing all tasks with the goal of independently managing specific areas over time. The Development team and this position are collaborative in nature, and the Sr. Analyst will work with both internal and external stakeholders including all Port Divisions, specifically Port Finance and the Port’s Resilience team, the Office of Economic and Workforce Development, the Office of Public Finance, and private development partners.

The essential functions of this position include:

• Analyzes Port-wide development opportunities to prioritize projects and determine value to support Port-wide Sea Level Rise resilience projects.
• Reviews project pro formas, appraisals, and other financial analyses to determine fiscal feasibility and economic value to the Port.
• Supports Port Development, Finance, and Office of Public Finance teams with tax district formation and bond issuances.
• Tracks ongoing Project finances to ensure projects stay on budget, public sources are sufficient for funding, and Port meets revenue expectations.
• Identifies cost-saving measures for development projects and support the implementation of these actions with other City departments
• Supports Project Managers on development negotiations (term sheets, leases, community benefits) by conducting research tasks and financial projections
• Assists in the drafting of relevant development deal documents (term sheet, leases, Development Agreements)
• Assists in preparation of memos, staff reports, and presentations for Port executives, Port Commission, and Board of Supervisors.
• Coordinates billing and payments for developer reimbursables and Development-related invoices with Development Finance Manager.
• Manages Development team’s as-needed economic consultant contracts.

Qualifications

Education

Baccalaureate degree from an accredited college or university; AND

Experience

Three (3) years full-time equivalent experience performing professional-level analytical work. Qualifying professional-level analytical work includes analysis, development, administration, and reporting in major programs and functions of an organization in the areas of budgets, contracts, grants, policy, or other functional areas related to the duties of positions in the 1823 Class series; AND

License and Certification

Possession of a valid California driver’s license (to be presented at time of appointment).  Employees must be insurable under the Port’s automobile liability insurance.  Insurability must be maintained throughout employment.

Substitution

Education Substitution: Additional qualifying work experience as described above may be substituted for the education requirement on a year-for-year basis, up to a maximum of 2 years (1 year of work experience is equal to 30 semester units or 45 quarter units).

San Mateo County Health's Behavioral Health and Recovery Services Division is seeking a responsible, innovative leader and skilled clinician for the position of Supervising Mental Health Clinician - East Palo Alto Services. This Supervising Mental Health Clinician will supervise and coordinate the work of mental health professionals and interns in the East Palo Alto and East Menlo Park geographic area in San Mateo County. This position works closely with community members and community providers to develop and sustain programming that is culturally and community responsive.

This position will participate in program administration, planning and evaluation. The Supervising Mental Health Clinician supervises and guides the clinical team who provides services to seriously emotionally disturbed youth and their families, and seriously mentally ill adults. Services provided include: medication assessment and management; individual, group and family therapy; psychological assessment; case management; rehabilitation services; and crisis intervention.

The current vacancy is in the East Palo Alto Community Counseling Center, located in East Palo Alto but will travel throughout San Mateo County for client visits, trainings and meetings.

The East Palo Alto Community Counseling Center offers a unique setting to provide services to a diverse population in a community-oriented clinic.

This position supervises a multidisciplinary team that provides clinical outpatient services to adults and children and their families. The team may include psychiatrists, psychologists, psychiatric nurses and social workers and marriage and family therapists. The clinical teams also include master and doctoral level interns and psychiatric residents. In providing clinical services team members also relate to peer counselors and parent partners.

San Mateo County Behavioral Health and Recovery Services (BHRS), Child and Youth Services https://www.smchealth.org/bhrs, provides a broad spectrum of Trauma Informed mental health and substance use services for children, youth, and families focusing on prevention, early intervention and treatment of youth diagnosed as Severely Emotionally Disturbed (SED) and/or substance use disorders. We provide services through specialty teams outpatient clinics, in schools, and field-based teams in Daly City, San Mateo, Half Moon Bay, Redwood City and East Palo Alto. BHRS also provides services through a network of community agencies and independent providers who offer outpatient and wraparound services. BHRS supports treatment of the whole person to achieve wellness and recovery, and to promote the health and well-being of the people, families and communities we serve. Additionally for Residential Youth Treatment at Canyon Oaks Youth Center, Community Care Licensing requires providers working in this facility to maintain 40 hours of annual training related to the field, most of which are provided by the facility.
