San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III - Discharge Planner/Case Manager to provide highly skilled, intensive discharge planning and case management to individuals and their families, and provide casework consultation within SMMC and to outside agencies.

 

The Social Worker III - Discharge Planner/Case Manager position will support SMMC to provide services related to discharge planning/case management on various units of the hospital, including the Medical/Surgical unit, Intensive Care Unit, Acute Psychiatry unit, Short Term Rehabilitation unit, Emergency Room, and Psychiatric Emergency Services. This includes needs assessment, assistance in financial resource management, crisis management, service delivery planning and follow up, and referrals to public, community-based services and programs. The candidate should be familiar with the rules and regulations governing skilled nursing and assisted living facilities. The ideal candidate will have direct experience working in an acute care hospital setting performing discharge planning, including appropriate documentation in accordance with all Medicare and Medi-Cal regulations, and should be experienced working with Electronic Medical Records (EMR) and LEAN principles.

• Provide information and assistance to patients of SMMC.
• Conduct initial screenings for patients and develop appropriate discharge/service plans to address identified issues.
• Provide skilled casework services to patients and their families.
• Schedule and conduct family care conference meetings with patients, families, and treatment team for short-term and long-term placement needs.
• Coordinate referrals, both internal and external, as part of the social service plan.
• Follow up on referrals and document outcomes as well as completion of items.
• Provide crisis intervention to patients and their families.

• Assist patients with applications and securing of private and public benefits.

Qualifications

Education:

• Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience:

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Role description

Under general supervision, operates a variety of transit vehicles such as diesel and electric motor coaches, cable cars, streetcars and light rail vehicles; transports passengers along a specific route and makes designated stops as required; accepts fares, issues transfers, gives general directional information; and performs related duties as required.

Examples of Important and Essential Duties
According to Civil Service Commission Rule 409, the duties specified below are representative of the range of duties assigned to this job code/class and are not intended to be an inclusive list.

1. Conducts pre-operational inspection of the transit vehicle, in accordance with federal and state laws and departmental regulations, to ensure it is functioning properly.
2. Inspects the passenger compartment of the vehicle to ensure it is free from hazards (such as spills, loose handrails and stanchions, defective seats, etc.) and that the video camera lenses are not obscured.
3. Safely navigates the vehicle through traffic along assigned routes, under various environmental conditions, in accordance with traffic laws and departmental regulations.
4. Operates the disability access lifts to assist people in wheelchairs onto the vehicle and properly secures their wheelchairs when they are on the vehicle.
5. Collects fares from passengers and issues transfers; checks passenger transfers, fast passes, and electronic passes to ensure they are valid.
6. Uses vehicle's intercom system to communicate instructions to passengers and to call out vehicle stops and transfer points.
7. Responds to customers' questions regarding routes, directions, fares and transfers.
8. Conducts a post-operational inspection of the outside of the vehicle to detect defects and body damage, broken windows, and graffiti.
9. Contacts Central Control regarding various emergencies such as medical emergencies, vehicle malfunctions, vehicle evacuations etc. and/or circumstances that may affect service such as heavy traffic, road hazards, road obstructions etc.
10. Completes accident, incident and/or miscellaneous reports to document unusual occurrences.
11. May include additional duties as assigned.

Nature of Work: Incumbents in this class may be required to work rotating shifts, holidays and weekends; to work outdoors in all weather conditions; and to use computers. In addition, may require standing and/or sitting for prolonged periods of time; performing different physical activities such as pulling poles, cranking switches in the subway, resetting retrievers to return disabled transit vehicles to service and assisting in the movement of disabled vehicles when possible. 2.  Possession of a valid High School Diploma, GED or California High School Proficiency Certificate AND
3.  One (1) year of verifiable satisfactory public contact or customer service experience AND
4.  Possession of a valid class C Driver License. Must have been a licensed driver for at least three (3) continuous years from April 30, 2021 through April 30, 2024 AND
5.  Possession of or the ability to obtain a Class B - Driver License with a Passenger (P) Endorsement and no restrictions that prohibit operation of a San Francisco Municipal Transportation Agency public transit vehicle AND

The San Francisco Municipal Transportation Agency (SFMTA) has determined that Class 9163 Transit Operator is a “safety-sensitive” position, under regulations issued by the Federal Transit Administration (49 CFR Part 655). Federal law requires that all transit employees who perform safety-sensitive functions be subject to random, post-accident, reasonable suspicion, return-to-duty and follow-up drug and alcohol testing. All applicants for Safety-Sensitive positions shall undergo urine drug testing prior to employment and within 90 days prior to performing Safety-Sensitive functions for the first time. Receipt by the SFMTA of a negative test result is required prior to the employee being placed on the payroll. Attendance to all training sessions is required. If a candidate misses any dates of training without providing proper notification, they will be released from their training class. If proper notification was provided, a candidate released during the training period due to attendance may request to be placed back on the eligible list to be considered for future classes, if the list from which they were hired is still active. IT Operations Supervisor will work closely with various users in San Mateo County Transit District (SamTrans), the Peninsula Corridor Joint Powers Board (Caltrain), and the San Mateo County Transportation Authority (TA) and other departments and is responsible for managing all procedures related to the identification, prioritization, and resolution of end user help requests, including the monitoring, tracking, and coordination of Help Desk functions. The role will also contribute to problem resolution by giving in-person, hands-on support to end users at the desktop level.

EXAMPLES OF ESSENTIAL FUNCTIONS:
• Supervise help desk support staff’s daily activities ensuring timely and professional delivery of technical support for calls on computers, core software products, and related hardware peripherals.
• Plan, organize, and direct the work of help desk support staff, including Help Desk Specialists; schedule shift coverage; evaluate and document performance and recommend disciplinary action when necessary; and review staff work for accuracy and timely completion of assigned duties.
• Act as the subject matter expert and point of contact in problem and major incident resolution.
• Document, track, and report on software and hardware malfunctions; ensure that detailed descriptions of user’s problems are documented in the ITSM; prioritize requests and label according to established procedures.
• Train help desk staff on areas of responsibility, new technology, and support procedures. Ensure help desk staff provides consistent, high-quality, and professional service, including communication and telephone skills.
• Proactively identify process improvement opportunities, including end-user training tools, onboarding, and documentation.
• Monitor help desk activities and respond to inquiries; provide technical troubleshooting, determine type of request, diagnose and provide solutions or escalate complex problems to appropriate personnel as required, and ensure proper problem notification procedures are being followed.
• Develop queries to track and follow-up on reported problems; notify users of completion of requests; ensure accurate contact and problem resolution records are maintained.

• Provide management reports on help desk operations and issues, including recommendations for areas of service or technology improvements.
• Manage the inventory of IT assets and coordinate purchase of equipment.
• Supervise staff. Hire, mentor and take appropriate corrective action and/ or disciplinary action. Ensure EEO policies and procedures are followed. Participate in selection of staff. Coordinate staff training and professional development. Establish performance objectives. Monitor and evaluate employee performance.

EXAMPLES OF DUTIES:
• Manage escalations of user issues and assist in resolving complex technical issues.
• Provide on call support as required. Some weekend or after-hours work may be required.
• Develop reports and extract data from Help Desk Service Management system.
• Establish and maintain cooperative and effective working relationships with others.
• Maintain current knowledge of various systems applications and operations.
• Operate a variety of office machines and equipment, including computer hardware and software.
• Troubleshoot, diagnose, and repair basic network, systems, and applications malfunctions.
• Perform all job duties and responsibilities in a safe manner to protect one’s self, fellow employees, and the public from injury or harm. Promote safety awareness and follow safety procedures in an effort to reduce or eliminate accidents.
• Perform other duties as assigned.

SUPERVISION: Works under the general supervision of the IT Operations Manager who establishes goals and objectives and evaluates performance.
