San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the position of Social Worker II (SW II) to provide a wide variety of services in such areas as information and referral, supportive services, protective services, out-of-home services, health related services, and other welfare within San Mateo County Health and the Human Services Agency.
 
The current vacancy is in the Enhanced Care Management (ECM) / Partners for Independence (PFI) Unit - Community Services (CS) that provides CS services coordination for adults who have complex medical and psychosocial needs and are at-risk for out of home placement.  ECM/PFI CS is a voluntary program that may be offered to San Mateo County residents who have Medi-CAL, who are aged and/or disabled with CalAIM CS needs.  
 
Distinguishing Characteristics
The Social Worker II screens applicants’ eligibility for CalAIM CS services. The SW II will travel throughout the County to conduct home visits to educate eligible clients regarding services, participate at Interdisciplinary team (IDT) meetings, coordinate services to assist clients and to perform other duties of the position.

 The ideal candidate will possess the following:

• Master's degree in Social Work.
• Excellent communication skills with seniors, cognitive and behavior challenged clients and their caregivers to develop an appropriate respite schedule for clients.
• Experience using comprehensive assessment tools to identify the needs of CalAIM CS applicants/participants. 
• Understanding of human development and behavior within diverse populations.
• Ability to independently develop and implement sound case plans of action.
• Experience balancing client-centered care coordination and the accompanying administrative documentation.
• Familiarity in implementing program regulations.
• Superior organizational skills to manage a large caseload and related documents.
• Proficient computer skills to use several independent data base systems as well as Microsoft Excel and Word.
• Ability to quickly learn and adopt new technology, policies, and procedures.
• Excellent writing/documentation skills.
• Valid CA driver license is required.
• Fluency in other languages in addition to English is a plus 

Qualifications

Licensure/Certification:

• California class C driver license or equivalent.

Education: Satisfactory completion of 30 semester units of social welfare and/or appropriate behavioral science courses.

Experience: Two years experience performing duties comparable to those of a Social Worker I or higher in San Mateo County. Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work.

About the Role

The newly created Community Services Division Director position is an exciting and unique opportunity to build a new division of the City of San Rafael.

The Community Services Division Director will play a pivotal role in shaping and leading the City of San Rafael's efforts to address critical social issues, particularly focusing on homelessness at this time.  Reporting directly to the Assistant City Manager, the Division Director will oversee and develop the newly created Community Services Division, ensuring the effective development and implementation of programs that improve the quality of life for residents in need.

In this role, the Division Director will manage a broad range of responsibilities, from strategic planning and policy development to budget management and interdepartmental and interagency coordination. They will be a key liaison between the City and  our County partners, and various other stakeholders, including public agencies, community organizations, and residents. Their work will involve close collaboration with other City departments, public and private partners, and community groups to advance initiatives that address homelessness and related services.

In their first year, they’ll work to develop the City’s first homelessness strategic plan, setting priorities and policies to guide the City’s work in this space for the next five years.

One of the key, immediate priorities the Division Director will oversee is the implementation of the City’s $6M Encampment Resolution Fund Grant. This includes leading the contractor and staff team operating the City’s planned Sanctioned Camping Program and spearheading the identification of a site for a future interim shelter and/or transitional housing. Valley Water is the primary water resource agency for Santa Clara County, supplying wholesale water, providing flood protection, and serving as environmental steward for clean, safe creeks and healthy ecosystems. Valley Water serves approximately two million people in 15 cities: Campbell, Cupertino, Gilroy, Los Altos, Milpitas, Monte Sereno, Morgan Hill, Mt. View, Palo Alto, San Jose, Santa Clara, Saratoga and Sunnyvale, and the towns of Los Altos Hills and Los Gatos, and manages 10 dams and surface water reservoirs, three water treatment plants, a state-of-the-art water quality laboratory, 400 acres of groundwater recharge ponds, and 275 miles of streams.
 
The Assistant Water Plant Operator helps with all day-to-day plant operations, including operating and maintaining plant equipment, calibrating water quality monitoring equipment; taking and analyzing water and chemical samples; cleaning equipment and facilities; receiving bulk chemical deliveries; operating the sludge dewatering process; using the computer-based plant control, or SCADA system, completing data entry, initiating work orders, and maintaining plant operations logs and reports. Operators work a 4 to 6-month assigned rotations between day shift, night shift, and other shifts, as needed. The facility operates and is staffed continuously, which means 24 hours a day, 365 days a year. This position reports to the Water Plant Supervisor and works under direction from the Senior/Water Plant Operator.

• Regularly conduct rounds to monitor plant operations and maintain plant equipment and grounds; observe, evaluate, troubleshoot, and report condition of the facility and equipment, including pipes, pumps, valves, feeders, motors, basins, tanks, generators, analyzers, and sludge beds; take and record measurements and instrument readings, such as pressures, temperature, flow, volume, and concentrations; operate and adjust pumps and valves; calibrate and perform preventative maintenance on plant instruments; observe and report on the flow, volume, and quality of the water, including color, odor, and turbidity; and assist in maintaining grounds in a clean, orderly, and secure state. 
• Draw samples of water and other materials from the different phases of water treatment; measure and analyze the samples to determine compliance with Valley Water and regulatory standards for chlorine, ozone, phosphate, and ammonia residuals, pH, turbidity, taste, and odor. Receive and monitor deliveries of bulk chemicals and other plant supplies.
• Perform cleaning and maintenance procedures on operating equipment and facilities; including flocculators, basins, filters, sludge drying beds, washwater clarification system, and containment and delivery areas. Employees have a responsibility for safety; for following safety regulations and safety policies and procedures applicable to their work; and for carrying out those responsibilities with due diligence as a condition of employment.
• Use the plant's computer based Supervisory Control and Data Acquisition (SCADA) system to monitor plant operations; scan multiple computer monitors simultaneously; navigate a multitude of program screens to observe, track, and retrieve water, chemical, and equipment readings. Perform various administrative tasks, including data entry into spreadsheets, update standard operating procedures, initiate work orders, maintain, compile and update plant operations logs and reports.

Ideal Experience:

Level II: One (1) or more years of experience performing work as a licensed operator at a water treatment plant that is rated as a T-4 or higher facility.

• An Associate’s degree in engineering or chemistry from an accredited college may substitute for the training required at the time of application. 
• Possession of a Grade T1 Water Treatment Operator certificate issued by the California State Water Resources Control Board may substitute for the training requirement at the time of application.

The Budget Manager is a division-level manager responsible for developing, administering, and overseeing the City's sophisticated Operating and Capital Budgets and Twenty-Year Financial Plans. THE POSITION

Tri-City is looking for an individual to serve as a Clinical Therapist I/II for the Adult Outpatient Program (AOP). The Clinical Therapist I/II will appeal to individuals who are inspired by challenges and opportunities presented in working with an engaged and active community with moderate to severe behavioral health clients. 

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Provides individual, group, and family interventions, rehabilitation, and treatment services including counseling, therapy, case management, and referral and linkage to community services to a variety of clients including high acuity clients.

• Within scope of authority, assesses, diagnoses, and treats a variety of clients with mental health disorders; collaborates with clients and creates individualized treatments plans according to client’s needs and circumstances.

• Initiates and conducts individual, group, and family counseling and biopsychosocial therapy sessions utilizing a variety of techniques and modalities; provides case management services in the office and in the field.

• Performs routine and emergency intake evaluations according to Authority procedure; completes appropriate forms and documents relating to the intake process; and prepares medical and demographic case history for use in diagnosis.

• Assists clients and family members in understanding behavioral health diagnosis, contributing factors/causes, effects, prognosis, rehabilitation, and treatment.

• Works with clients to address stressors and barriers to treatment and/or social functioning; identifies and focuses on client strengths and personal resources to effectively engage and treat client.

The Community  Action Program Specialist determines client’s needs for program services and provides case management and/or crisis intervention in one or more various program areas. The Community Action Program Specialist II is the full journey level class within the series and is   distinguished from the entry level class by the performance of the full range of duties, including the more complex cases. This position is full-time and is scheduled for forty (40) hours per week.

WHAT YOU’LL DO

• Analyzes clients’ financial and eligibility information to determine initial or continuing eligibility for programs.
• Secures case documentation, records, and/or other information from other agencies as required.
• Interview and evaluates clients for specific program services eligibility and/or employment opportunities, including service needs assessment and crisis intervention, to assist individuals/families in achieving self-sufficiency. 
• Conducts orientation workshops for new and returning participants; explains regulations, rules and policies to clients and apprises them of their rights and responsibilities for program participation.
• Receives and responds to inquiries regarding program services from clients, other agencies, and the general public.

Under direction, provides lead direction to the staff engaged in the full range of assigned duties related to the City’s water distribution and treatment systems, and the City’s Wastewater Treatment Plant Facility.

The Senior Utility Worker is the advanced journey-level classification in the Utility Worker series. Incumbents perform skilled duties in the operations and maintenance of the water distribution and treatment system, and plant operations and maintenance of the wastewater treatment plant and provide lead direction and training to assigned crews.

Receives direction from the Utilities Superintendent and/or higher-level staff. Exercises technical and functional supervision over lower-level utilities staff.
