San Mateo County Health's Public Health, Policy and Planning Division has exciting field-based opportunities for well-qualified and motivated Nurse Practitioners to provide both primary and urgent care to adults from vulnerable populations within our community—people experiencing homelessness and/or in shelters, monolingual Spanish speakers and other new immigrants, and those without insurance or underinsured. These services are provided through various Mobile Clinic modalities and venues, including but not limited to street medicine, shelter medicine, and the mobile coach. We are looking for collaborative Nurse Practitioners to work in and lead various multidisciplinary service teams in the field to provide comprehensive, low-threshold, client-centered care and engagement.

Teams are staffed by a Nurse Practitioner (NP), Psychiatrist, Medical Services Assistant, AOD Case Manager, and Patient Services Assistants. These teams work closely with our community partners from the Human Services Agency and community-based organizations who locate and identify clients needing medical and/or mental health services, as well as provide additional health, housing, and social services in the field.

Under general supervision, Nurse Practitioners provide urgent care and primary health care services such as physical examinations, chronic disease management (e.g., diabetes and hypertension), and referrals/linkages to specialty clinics as appropriate.

Currently, Public Health Clinics has one full-time, regular, 40-hour-per-week vacancy for a bilingual Spanish Nurse Practitioner and two 40-hour-per-week, Limited-Term Nurse Practitioner vacancies, requiring flexible schedules and field-based work at different locations. Sixty-five percent of the NP’s time will be in the field, with the remainder spent in the office documenting notes, placing orders, and coordinating with other staff as needed.

The ideal candidate has experience:

Examples of Duties

Duties may include, but are not limited to, the following:

• Provide low-threshold, comprehensive care for San Mateo County’s most marginalized and vulnerable populations, including people experiencing homelessness, those in shelters, migrant and immigrant farmworkers, the recently incarcerated, the uninsured, and those with financial challenges.

• Provide comprehensive and urgent care in a variety of non-traditional settings, including homeless encampments, outdoor locations, shelters, and a mobile RV clinic.

• Demonstrate the ability to work closely and collaboratively with a small team in often challenging environments (e.g., homeless encampments, shelters, extreme weather, individuals under the influence, or those with mental health challenges).

• Take stewardship of medical care for assigned geographic populations and/or shelters.

• Outreach to and build long-term, trust-based relationships with vulnerable populations.

• Assess patients’ physical, emotional, and social status through medical history, physical examinations, and diagnostic screenings; evaluate clinical findings and lab results.

• Identify and manage common illnesses and other health problems.

• Refer urgent issues beyond scope to appropriate specialists.

• Deliver longitudinal care in assigned areas.

• Collaborate with other health professionals following protocols for diagnosis and treatment.

• Build relationships with community partners to improve health outcomes and expand services.

• Counsel families regarding health maintenance and care.

• Coordinate referrals to other health or social agencies.

• Maintain accurate records of assessments, follow-ups, and evaluations.

• Supervise subordinate health workers and participate in meetings.

• Actively participate in quality improvement efforts.

Qualifications

Education and Experience
Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying.

• Education: Completion of an advanced nursing degree from a program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC), with emphasis in a Nurse Practitioner specialty area.

• Experience: Minimum of one (1) year of paid experience in an acute and/or ambulatory care setting.

Position Summary

Under general supervision of the Director of Planning and Principal Planners, manages projects and performs work tasks to support the department’s work program, which includes service and operations planning, short- and long-range planning, facilities planning, and compliance reporting for all three of the District’s operating divisions.

This position advances the District’s business interests by analyzing complex operating conditions and recommending detailed changes, producing reports and planning studies, researching and interpreting relevant regulations, and meeting with internal staff and external stakeholders on a wide variety of topics. Work may require the use of specialized software, such as GIS and Hastus, and travel to District facilities and off-site meeting locations. May supervise and assign work to Associate Planner(s) and Planner(s) as directed.

Essential Responsibilities

• Research, compile, analyze, and interpret complex technical, demographic, economic, financial, and other statistical data for the development of transit plans and projects, such as the District's transit service plan, transit capital improvement program, and proposed bridge toll or transit fare changes.
• Monitor, forecast, and evaluate transit service performance and patterns of bridge traffic, parking, and transit service patronage and recommend changes where necessary.
• Prepare or supervise preparation of routine evaluations of the efficiency and effectiveness of bus and ferry routes, schedules, major transit facilities, and bus stops. Recommend specific improvements based on these evaluations.
• Prepare or oversee preparation of major compliance documents, including the Short-Range Transit Plan (SRTP), National Transit Database (NTD), Title VI, and other federal and state reports, including the organization of internal stakeholder meetings to provide input on these reports.
• Prepare official recommendations and informational reports for consideration by the public, the District’s Board of Directors, and technical staff of the District or other agencies and groups.
• Prepare or assist with verbal presentations to staff groups, public groups, and the boards of local municipalities and advise District personnel on matters related to interactions with these groups.
• Support and coordinate staff responsibilities regarding other internal and external coordination meetings as directed. May participate in public meetings and hearings.

Position Summary

Under general supervision, operates a variety of transit vehicles to transport passengers along specific, assigned routes according to the District's requirements. Provide service in a safe, courteous and reliable manner. Responsible for written reports and other information as required. Upon completion of the Golden Gate Transit Training Program, the employee operates regularly scheduled assignments. All runs will originate in either Santa Rosa, Novato or San Rafael.

Hourly Pay: $31.29/hour to $36.81/hour

San Mateo County Behavioral Health and Recovery Services (BHRS) seeks a board eligible or board-certified psychiatrist to serve as a staff Child and Adolescent Psychiatrist within BHRS. The position is full time, and provides child psychiatric diagnostic and therapeutic services to clients in various divisions of San Mateo County Behavioral Health and Recovery Services. Potential service sites include our outpatient behavioral health clinics, the Canyon Oaks Youth Center (youth residential program), the Youth Services Center, the Therapeutic Day School programs, the Transitional Age Youth program, and BHRS’s integrated behavioral health team co-located within the County’s pediatric clinics. Medical Services Assistants II who are assigned to the clinics and perform advanced patient care duties as defined in the classification specification shall receive a differential of 6.2% in addition to all other compensation.

• Accurately take and record the condition, respiration, pulse, temperature and blood pressure of patients; record other pertinent information as observed or measured.

Qualifications

Certification Requirement:

• Must show proof of current enrollment in a California P.O.S.T. Basic Training Police Academy, and must successfully complete academy prior to appointment.

• Must currently be employed as a full time paid, sworn police officer for at least two years, or have been employed for at least 24 months as a full time paid sworn police officer in a civilian governmental jurisdiction prior to application; or  

• Possession of a valid California P.O.S.T. Basic Certificate or equivalent as approved by P.O.S.T. 

• Must have successfully completed a current California P.O.S.T. Basic Training Police Academy or equivalency.

Preschool Aide

Under the direction of a Recreation Supervisor, the Preschool Aide supports the lead teacher in providing a nurturing and educational environment for young children. Position Summary

Minimum Qualifications

• Graduation from high school or GED equivalent.  

• Certification in personal training or group fitness instruction required (e.g., ACE, NASM, or other recognized certification).  

• At least one year of experience in fitness instruction, personal training, or a related field preferred.  

• Experience in group instruction and one-on-one training is highly desirable. 

Knowledge and Skills

This position requires a knowledge of general fitness principles, exercise techniques, and wellness practices, instructing and motivating individuals or groups in a fitness setting, excellent communication to provide clear instruction and positive feedback and assessing client needs and creating individualized fitness programs. The position also requires ability to operate and maintain fitness equipment safely. 

Special Requirements: 

• Current certification in First Aid and Cardiopulmonary Resuscitation (CPR). 

• Fitness certifications (e.g., Personal Trainer, Group Fitness Instructor) from a nationally recognized organization.
