San Mateo County Health's Public Health, Policy and Planning Division has exciting field-based opportunities for well-qualified and motivated Nurse Practitioners to provide both primary and urgent care to adults from vulnerable populations within our community—people experiencing homelessness and/or in shelters, monolingual Spanish speakers and other new immigrants, and those without insurance or underinsured. These services are provided through various Mobile Clinic modalities and venues, including but not limited to street medicine, shelter medicine, and the mobile coach. We are looking for collaborative Nurse Practitioners to work in and lead various multidisciplinary service teams in the field to provide comprehensive, low-threshold, client-centered care and engagement.

Teams are staffed by a Nurse Practitioner (NP), Psychiatrist, Medical Services Assistant, AOD Case Manager, and Patient Services Assistants. These teams work closely with our community partners from the Human Services Agency and community-based organizations who locate and identify clients needing medical and/or mental health services, as well as provide additional health, housing, and social services in the field.

Under general supervision, Nurse Practitioners provide urgent care and primary health care services such as physical examinations, chronic disease management (e.g., diabetes and hypertension), and referrals/linkages to specialty clinics as appropriate.

Currently, Public Health Clinics has one full-time, regular, 40-hour-per-week vacancy for a bilingual Spanish Nurse Practitioner and two 40-hour-per-week, Limited-Term Nurse Practitioner vacancies, requiring flexible schedules and field-based work at different locations. Sixty-five percent of the NP’s time will be in the field, with the remainder spent in the office documenting notes, placing orders, and coordinating with other staff as needed.

Duties may include, but are not limited to, the following:

• Provide low-threshold, comprehensive care for San Mateo County’s most marginalized and vulnerable populations, including people experiencing homelessness, those in shelters, migrant and immigrant farmworkers, the recently incarcerated, the uninsured, and those with financial challenges.

• Provide comprehensive and urgent care in a variety of non-traditional settings, including homeless encampments, outdoor locations, shelters, and a mobile RV clinic.

• Demonstrate the ability to work closely and collaboratively with a small team in often challenging environments (e.g., homeless encampments, shelters, extreme weather, individuals under the influence, or those with mental health challenges).

• Take stewardship of medical care for assigned geographic populations and/or shelters.

• Outreach to and build long-term, trust-based relationships with vulnerable populations.

• Assess patients’ physical, emotional, and social status through medical history, physical examinations, and diagnostic screenings; evaluate clinical findings and lab results.

• Identify and manage common illnesses and other health problems.

• Refer urgent issues beyond scope to appropriate specialists.

• Deliver longitudinal care in assigned areas.

• Collaborate with other health professionals following protocols for diagnosis and treatment.

• Build relationships with community partners to improve health outcomes and expand services.

• Counsel families regarding health maintenance and care.

• Coordinate referrals to other health or social agencies.

• Maintain accurate records of assessments, follow-ups, and evaluations.

• Supervise subordinate health workers and participate in meetings.

• Actively participate in quality improvement efforts.

Qualifications

Education and Experience
Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying.

• Education: Completion of an advanced nursing degree from a program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC), with emphasis in a Nurse Practitioner specialty area.

• Experience: Minimum of one (1) year of paid experience in an acute and/or ambulatory care setting.

Licensure

There is one (1) full-time, regular hire Senior Civil Engineer vacancy in the Engineering Division. 

Under direction, the Senior Civil Engineer, plans, manages, and coordinates the engineering program. Using broad parameters of departmental policy guidelines, this position exercises substantial initiative, judgment, and expertise to meet program and project outcomes. The Senior Civil Engineer creates the scope on large civil engineering projects with complex budgets and timelines, integrating numerous elements that significant stakeholder involvement and sensitivity. This position provides direction and mentorship to more junior staff and oversees both technical and administrative work performed by their staff and ensures that projects are completed efficiently and effectively for the public good. Informing the public and Board of Supervisors, selecting consultants, preparing contract documents, reviewing project plans and specifications prepared by consultants, and administering contracts during construction are performed by the Senior Civil Engineer. 

Signing bonus:
In addition to a competitive compensations package, new, full-time, regular hire Senior Civil Engineers are eligible to receive a $10,000 signing bonus which will be split into two equal payments: 50% at time of hire (i.e. first paycheck) and the remaining 50% after successful completion of their probationary period.

Hybrid schedule:

Position Summary

Under general direction of the Secretary of the District, performs complex and highly analytical work for the Secretary of the District, members of the Board of Directors and associated committees.   Departmental lead on government tort claims, legal documents and communications and participating in complex, sensitive, and detailed analytical work in the areas of budgets, historical preservation and economic, regulatory and administrative policy. Provides internal customer service by assisting managers and executive level staff in complex analysis.  This position requires a high degree of accuracy, discretion, confidentiality, excellent communication skills, and robust customer service skills. 
 

Essential Responsibilities

• Serves as the primary analytical support to the District Secretary;
• Interprets meeting proceedings and prepares draft meeting minutes, with minimal oversight and as assigned, may attend meetings in the absence of the District Secretary;
• Performs legal functions of the office and guidance to office staff in the absence of the District Secretary;
• Performs a wide variety of complex legal analysis, research and briefings about claims for the District, Bridge, Bus and Ferry Divisions, and works with legal counsel in the areas such as government tort claims, subpoenas, Brady requests, small claims and lawsuits;
• Provides a wide variety of complex, confidential analytical duties in support of the District Secretary, Board of Directors, and standing Board committees;
• Independently composes, researches, compiles, summarizes, and prepares a variety of complex correspondence, reports and documents; reviews finished materials for completeness and accuracy; ensures documents comply with District standards, policies and procedures;
• Perform special studies for the District, and coordinates assigned activities with other District departments, and divisions, and outside entities;
• Independently responds to and resolves difficult and sensitive citizen inquiries and complaints;
• Coordinates and assists in the development of the annual budgets of the Board and District Secretary’s Office and monitors, reviews and approves department expenditures, and performs financial analysis for budgetary purposes;

• Monitors activities and ensures maintenance of files related to insurance and accident claim matters to fulfill department records retention schedule;

MINIMUM QUALIFICATIONS
Knowledge of: Office machines and specialized software applications; current philosophy, principles, and practices for services to clients with serious mental illness; individual, group and family psychodynamics as they relate to adults or children in crisis; psychological diagnostic methods and terminology; personality development from infancy through adulthood; human and health services systems in the County including private agencies and other community resources; applicable local, State and Federal laws, ordinances and rules.

Ability to: Provide crisis intervention treatment to adults, children or adolescents with various mental disabilities ranging from serious mental illness to dysfunctional behavior patterns, including victims of sexual or physical abuse; set realistic treatment goals and facilitate their attainment; evaluate the degree of mental disability or emotional distress of patients/clients; communicate effectively orally and in writing; provide consultation and technical assistance to agencies, schools and other organizations; operate modern office equipment including specialized software applications programs; interacts positively and cooperate with co-workers, responds politely to customers, works as a team member, functions under demanding time pressure, responds in a positive manner to supervision, attends work and perform duties on a regular and consistent basis.

Education: Master's degree from accredited college or university in Social Work, Social Welfare, Psychology, or a closely related field.

Experience:
Crisis Response Clinician I: One (1) year of experience working in the social work or counseling field with strong emphasis on social services, crisis response, severe mental illness, resources, and referrals using a collaborative and community framework.
Crisis Response Clinician II: Two (2) years of experience working in the social work or counseling field with strong emphasis on social services, crisis response, severe mental illness, resources, and referrals using a collaborative and community framework.

License:
Possession of a valid Class C California Driver's License.

The Department

The Manager, Track & Structures reports to the Deputy Director of Infrastructure Engineering and is responsible for overseeing railroad track and structures engineering support for the Peninsula Corridor Joint Powers Board (Caltrain)

Essential Functions & Duties:

• Acts as agency lead on matters related to track and structures.
• Oversee the track and structures engineering tasks for design/construction of Caltrain Capital and SOGR projects and provide technical support for planning on grade separation projects.
• Ensure the capital and maintenance projects comply with the contract agreement and implement the approved design for compliance with JPB standards and specifications. 
• Lead the advanced technical reviews of projects to assess constructability, examine design details, perform quantity audit, and recommend construction sequencing.
• Establish effective relationships and liaise with Caltrain operation contractors, consultants, internal departments, and other agencies (CPUC, FRA, UPRR, etc.) on railroad track and structures related to influence design and project development.
• Supervise staff.  Hire, mentor and take appropriate corrective and/or disciplinary action.  Ensure EEO policies and procedures are followed. Participate in selection of staff. Coordinate staff training and professional development. Establish performance objectives. Monitor and evaluate employee performance.

Examples of Essential Duties: 

• Plan, develop and implement the group’s work plan, budget, goals, and objectives to be consistent with the Engineering Department’s goals.
• Manage the design review of project documentation such as drawings, basis of design, specifications, technical reports, and calculations for compliance with Caltrain Standards and for cost effectiveness of the design. 
• Lead development of potential solutions to the design/construction team and presents for design optimization, risk management challenges, and constructability improvements.
• Assist with Maintenance Department to develop the annual and long-term railroad track engineering and maintenance plans, coordinate these plans with capital projects and support to implement the plans.
• Review and provide technical input for capital and maintenance projects for compliance and cost effectiveness of the design.
• Perform QA reviews of RFIs and submittals as well as participate in change order review and final inspections to construction. 
• Ensure track and structures comply with all return-to-service requirements following all construction activities.
• Review and evaluate project specific designs and provide technical recommendations to design variance approval process.
• Responsible for Caltrain Bridge Management Program.
• Responsible for updates to Caltrain Engineering Standards.
• Train, motivate and monitor staff activities, provide direction, and to identify and resolve the problems.
