We are seeking two clinic managers who will operate under the general direction of the Deputy Director of Ambulatory Care. The clinic managers plan, coordinate, direct, manage, supervise and evaluate the full range of programs and services offered at the community-based health clinics; recommend, develop, implement and evaluate goals, objectives, policies and procedures related to clinics; provide leadership and coordination to community groups, public and private agencies, advisory boards and volunteers in planning and implementing clinic services; and provide highly complex staff support to senior-level management. The clinic managers also lead and participate in complex performance improvement activities; develop systems to track the progress of strategic initiatives, departmental operational goals, and communicate progress in key forums; develop productive relationships with labor partners while navigating operational needs and developing and sustaining effective employee engagement processes. The clinic managers exercise direct and indirect supervision over a diversified clinical and support staff team.

The ideal candidates will have working knowledge of clinic operations, particularly in outpatient clinics that offer primary care, adolescent care, and pediatrics, as well as in budgeting and scheduling. The ideal candidates will also be experienced in change management, improving patient satisfaction scores, and continuous process improvement (Lean). Examples Of Duties

Duties may include, but are not limited to, the following:

• Plan, direct, monitor, coordinate and evaluate the operations and service delivery programs of a regional health clinic or clinics, including needs assessment, program design and planning, implementation, evaluation, and regulatory enforcement.

• Direct, coordinate, and participate in the development and implementation of program goals, objectives, policies, procedures, and priorities that balance both mandated services and community health care needs.

• Assess the quality of patient care; develop and generate community support for changes or enhancements to service delivery methods; advocate for patients with medical staff to ensure appropriate care is provided.

• Monitor census/patient flow and project future patient care demands; develop staffing schedules and assignments to provide the necessary level and type of care.

• Develop, present, and advocate for clinic funding; administer and monitor the approved budget to ensure the accomplishment of clinic care objectives within budget restrictions.

• Act as liaison between senior-level management, advisory boards and commissions, regulatory bodies, and program staff to ensure that all appropriate policies and guidelines are followed, and that programs are funded with adequate resources to maintain mandated and expected levels of service.

• Maintain an effective working relationship with a variety of public and private organizations, including other Health Services Agency staff, County departments, and agencies.

• Consult with other program managers on relevant program issues; develop alternative strategies for dealing with community health needs and assist in the implementation of solutions as necessary.

• Act as advocate and spokesperson in the community in support of programs and services; interpret, explain, and direct the enforcement of community health-related standards, codes, and regulations.

• Provide highly complex staff assistance to senior-level management and related commissions and advisory boards on program issues.

• Prepare a wide variety of detailed written reports, policies, procedures, and contracts.

• Perform related duties as assigned.

Qualifications

Education and Experience: Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying. A typical way to qualify is:

• Three years of administrative or managerial experience in a healthcare delivery, social, or community services organization, including significant responsibility for one or more of the following: program and budget development, program service delivery, funding, utilization, coordination of community resources, or program evaluation.

San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the unclassified position of In-Home Supportive Services Social Worker II/III. Social Workers provide a wide variety of welfare services in such areas as information and referral, protective services, out-of-home services, health related services, chore services, and other welfare services within the Health and the Human Services Agency.

The current unclassified vacancy is in the In-Home Supportive Services (IHSS) Program that provides in-home care for persons who cannot safely remain in their own home without such assistance. The recipients of IHSS are aged, blind and/or disabled and eligible for Medi-Cal. Disabled and/or blind minor children may also be eligible for IHSS. The Social Worker will make a home visit to determine eligibility for the program and to conduct an assessment to determine a range of service hours for particular tasks. The IHSS recipient or guardian is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive. The Social Worker will travel throughout the County to conduct home visits, attend meetings and perform other duties of the position. The Social Worker will work in the San Mateo, CA Aging and Adult Services Office.

This IHSS Social Worker will conduct intake and/or on-going activities for IHSS recipients. Some of those activities include:

• Conduct initial IHSS assessments using a detailed State assessment tool.
• Complete necessary documents, paperwork, and work within different information technology systems.
• Conduct annual reassessments.
• Educate, support, and perform administrative duties for IHSS recipients and care providers to understand/comply with program regulations.
• Provide education, outreach, and community resources information.
• Participate in quality improvement efforts.
• Collaborate with quality assurance and program integrity to ensure proper adherence to regulations.
• Prepare for and attend Fair Hearings.
• Engage in interdisciplinary team meetings or other activities to support San Mateo County's vision for integrated services.
• Maintain oversight of case management needs for a caseload focused on serving IHSS recipients who are minors.
• Collaborate and facilitate service with care providers and other partner agencies.

The ideal candidate will possess the following:

• Master's degree in Social Work.
• Experience providing clinical assessment for children, youth and adults with disabilities.
• Understanding of human development and behavior within diverse populations.

• Experience balancing client-centered care coordination and the accompanying administrative documentation.
• Familiarity in implementing program regulations.

• Excellent writing/documentation skills.
• Fluency in Cantonese, Mandarin, Tagalog, Vietnamese or Farsi is a plus.

Social Workers provide highly skilled, intensive casework services to individuals and their families and provide casework consultation within the department and to outside agencies. Incumbents of positions at this level work with minimal supervision and are expected to have acquired the high degree of assessment skills required to perform care coordination and referral tasks in complex casework areas. Lead supervisory responsibilities may be assigned as circumstances dictate.

This is an unclassified position. Unclassified positions are at-will for a limited duration, typically associated with grant or other special funding. These positions are not covered by San Mateo County Civil Service Rules; and there is no formal probation period. Job Description

Under the supervision of a pharmacist, the Pharmacy Technician supports the provision of pharmaceutical care to patients served by the San Francisco Department of Public Health. Assists registered pharmacists in performing nonprofessional duties in an institutional pharmacy; receives, inspects, stores, and fills orders for pharmaceutical supplies. Nature of work involves continuous close attention to detail and some potential exposure to hazardous substances.

Under supervision, the Pharmacy Technician performs the following essential job functions:

• Accurately prepares unit dose medications, compounds a variety of pharmaceuticals under the supervision of a Pharmacist, including intravenous admixtures according to proper aseptic technique and in accordance with, professional standards and departmental policies and procedures, in order to maintain required levels of supply and to provide patients with needed medications;
• Prepares or repackages drugs and pharmaceuticals under the supervision of a Pharmacist, in amounts needed to fill written orders/prescriptions in order to provide patients with needed medication and to maintain required levels of supply;
• Prepares medication labels based on written orders from physicians and other professional staff according to California State Board of Pharmacy requirements;
• Records billing information for accurate and appropriate billing of patients for medications, involving use of desktop computer keyboard for entering medication orders and billing information;
• Maintains drug and pharmaceutical stock levels by counting and checking stores of drugs and pharmaceuticals against standard, pre-set stock levels, ordering and verifying deliveries against invoices to ensure accuracy; 
• Inspects and stores delivered stock in the pharmacy patient care areas by sorting and shelving them for optimal retrieval;
• Delivers, retrieves, and exchanges medications and pharmaceutical items to and from specified areas, or for dispensing by cart to ensure that clinical and patient needs are met;
• Provides information to patients, public and other healthcare providers within the scope of Technician activities through a variety of methods, including personal contact, at the Pharmacy window, in patient care areas, and by telephone or video.  This does not include medication counseling or providing drug information; 
• Orients and trains new personnel, pharmacy students, or others on departmental policies and procedures appropriate to a Technician’s scope of work;
• Uses, maintains, and troubleshoots automated dispensing equipment or relevant pharmacy-related software and other equipment necessary for pharmacy operations;
• Accurately prepares, dispenses, and documents controlled substance orders, and maintains accurate inventory records.

The 2409 Pharmacy technician also performs other related duties as assigned.

Qualifications

Minimum Qualifications

1.   Experience: One (1) year (equivalent to 2,000 hours) of experience (in the last 5 years) assisting in the preparation of drug and pharmaceutical items in a licensed pharmacy;

AND

2.   License: Possession of a valid Pharmacy Technician License/Registration issued by the California State Board of Pharmacy. (Please attach a copy)

• License status must be "Current" according to search.dca.ca.gov/

Substitution: Possession of a valid Intern Pharmacist License/Registration issued by the California State Board of Pharmacy may substitute for the required experience and license.
