San Mateo County Health's Public Health, Policy and Planning Division has exciting field-based opportunities for well-qualified and motivated Nurse Practitioners to provide both primary and urgent care to adults from vulnerable populations within our community—people experiencing homelessness and/or in shelters, monolingual Spanish speakers and other new immigrants, and those without insurance or underinsured. These services are provided through various Mobile Clinic modalities and venues, including but not limited to street medicine, shelter medicine, and the mobile coach. We are looking for collaborative Nurse Practitioners to work in and lead various multidisciplinary service teams in the field to provide comprehensive, low-threshold, client-centered care and engagement.

Teams are staffed by a Nurse Practitioner (NP), Psychiatrist, Medical Services Assistant, AOD Case Manager, and Patient Services Assistants. These teams work closely with our community partners from the Human Services Agency and community-based organizations who locate and identify clients needing medical and/or mental health services, as well as provide additional health, housing, and social services in the field.

Under general supervision, Nurse Practitioners provide urgent care and primary health care services such as physical examinations, chronic disease management (e.g., diabetes and hypertension), and referrals/linkages to specialty clinics as appropriate.

Currently, Public Health Clinics has one full-time, regular, 40-hour-per-week vacancy for a bilingual Spanish Nurse Practitioner and two 40-hour-per-week, Limited-Term Nurse Practitioner vacancies, requiring flexible schedules and field-based work at different locations. Sixty-five percent of the NP’s time will be in the field, with the remainder spent in the office documenting notes, placing orders, and coordinating with other staff as needed.

The ideal candidate has experience:

Examples of Duties

Duties may include, but are not limited to, the following:

• Provide low-threshold, comprehensive care for San Mateo County’s most marginalized and vulnerable populations, including people experiencing homelessness, those in shelters, migrant and immigrant farmworkers, the recently incarcerated, the uninsured, and those with financial challenges.

• Provide comprehensive and urgent care in a variety of non-traditional settings, including homeless encampments, outdoor locations, shelters, and a mobile RV clinic.

• Demonstrate the ability to work closely and collaboratively with a small team in often challenging environments (e.g., homeless encampments, shelters, extreme weather, individuals under the influence, or those with mental health challenges).

• Take stewardship of medical care for assigned geographic populations and/or shelters.

• Outreach to and build long-term, trust-based relationships with vulnerable populations.

• Assess patients’ physical, emotional, and social status through medical history, physical examinations, and diagnostic screenings; evaluate clinical findings and lab results.

• Identify and manage common illnesses and other health problems.

• Refer urgent issues beyond scope to appropriate specialists.

• Deliver longitudinal care in assigned areas.

• Collaborate with other health professionals following protocols for diagnosis and treatment.

• Build relationships with community partners to improve health outcomes and expand services.

• Counsel families regarding health maintenance and care.

• Coordinate referrals to other health or social agencies.

• Maintain accurate records of assessments, follow-ups, and evaluations.

Qualifications

Education and Experience
Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying.

• Education: Completion of an advanced nursing degree from a program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC), with emphasis in a Nurse Practitioner specialty area.

• Experience: Minimum of one (1) year of paid experience in an acute and/or ambulatory care setting.

Position Summary

Under general direction of the Secretary of the District, performs complex and highly analytical work for the Secretary of the District, members of the Board of Directors and associated committees.   Departmental lead on government tort claims, legal documents and communications and participating in complex, sensitive, and detailed analytical work in the areas of budgets, historical preservation and economic, regulatory and administrative policy. Essential Responsibilities

• Serves as the primary analytical support to the District Secretary;

The Manager, Track & Structures reports to the Deputy Director of Infrastructure Engineering and is responsible for overseeing railroad track and structures engineering support for the Peninsula Corridor Joint Powers Board (Caltrain)

Essential Functions & Duties:

• Acts as agency lead on matters related to track and structures.
• Oversee the track and structures engineering tasks for design/construction of Caltrain Capital and SOGR projects and provide technical support for planning on grade separation projects.

The Position:

The Position:
The Accountant performs professional work involved in first line supervision of accounting functions, complex account development, maintenance, modification, and reporting. Under direction, the position is responsible for applying generally accepted accounting principles to analyze, maintain, reconcile and verify a variety of budgetary, accounting and other fiscal records. The position supervises daily operations in a variety of functional accounting areas and maintains adequate oversight and internal controls over such operations. Public contact is extensive and involves staff at various levels within the organization, local government agencies and other outside entities for the purpose of exchanging technical accounting procedural and policy information. A high degree of independent judgment and creativity are required to analyze data and develop resolutions to a variety of minor and major problems that occur. The consequences of errors in judgment can be costly in employee time, public relations and/or institutional funding; however, administrative controls limit the risk of serious consequences in loss of revenue. An Accountant can direct the work of accounting paraprofessional, clerical and other staff as assigned.

Duties and Responsibilities:

• Uses a variety of spreadsheet and other software to analyze complex financial, statistical data
• • Composes and prepares correspondence, recommendations and reports using a variety of computer software
• Posts revenue, abatement, expenses, accounts receivable and liabilities to the ledger
• Monitors and records various investments, certificates of deposit, and other fund instruments as assigned
• Monitors, reconciles and balances expenditure and revenue control accounts and general ledger accounts
• Provides regular financial reports
• Researches, compiles data for, and prepares complex fiscal reports to local, state, and federal agencies as required
• Provides input into the development and modification of database programs, screens, data formats and record keeping
• Prepares cash reconciliations
• Prepares monthly income analysis summaries, cash flow statements, and fund balance statements as needed
• Maintains chart of accounts
• Sets up and maintains database and other online resources of data
• Plans and reviews report data and conclusions with management staff
• Monitors and evaluates performance of internal and external computerized financial record systems and develops recommendations, in conjunction with information technology staff, to implement system improvements and upgrades and to maintain effective integration of systems
