San Mateo County Health's Public Health, Policy and Planning Division has exciting field-based opportunities for well-qualified and motivated Nurse Practitioners to provide both primary and urgent care to adults from vulnerable populations within our community—people experiencing homelessness and/or in shelters, monolingual Spanish speakers and other new immigrants, and those without insurance or underinsured. These services are provided through various Mobile Clinic modalities and venues, including but not limited to street medicine, shelter medicine, and the mobile coach. We are looking for collaborative Nurse Practitioners to work in and lead various multidisciplinary service teams in the field to provide comprehensive, low-threshold, client-centered care and engagement.

Teams are staffed by a Nurse Practitioner (NP), Psychiatrist, Medical Services Assistant, AOD Case Manager, and Patient Services Assistants. These teams work closely with our community partners from the Human Services Agency and community-based organizations who locate and identify clients needing medical and/or mental health services, as well as provide additional health, housing, and social services in the field.

Under general supervision, Nurse Practitioners provide urgent care and primary health care services such as physical examinations, chronic disease management (e.g., diabetes and hypertension), and referrals/linkages to specialty clinics as appropriate.

Currently, Public Health Clinics has one full-time, regular, 40-hour-per-week vacancy for a bilingual Spanish Nurse Practitioner and two 40-hour-per-week, Limited-Term Nurse Practitioner vacancies, requiring flexible schedules and field-based work at different locations. Sixty-five percent of the NP’s time will be in the field, with the remainder spent in the office documenting notes, placing orders, and coordinating with other staff as needed.

Duties may include, but are not limited to, the following:

• Provide low-threshold, comprehensive care for San Mateo County’s most marginalized and vulnerable populations, including people experiencing homelessness, those in shelters, migrant and immigrant farmworkers, the recently incarcerated, the uninsured, and those with financial challenges.

• Provide comprehensive and urgent care in a variety of non-traditional settings, including homeless encampments, outdoor locations, shelters, and a mobile RV clinic.

• Demonstrate the ability to work closely and collaboratively with a small team in often challenging environments (e.g., homeless encampments, shelters, extreme weather, individuals under the influence, or those with mental health challenges).

• Take stewardship of medical care for assigned geographic populations and/or shelters.

• Outreach to and build long-term, trust-based relationships with vulnerable populations.

• Assess patients’ physical, emotional, and social status through medical history, physical examinations, and diagnostic screenings; evaluate clinical findings and lab results.

• Identify and manage common illnesses and other health problems.

• Refer urgent issues beyond scope to appropriate specialists.

• Deliver longitudinal care in assigned areas.

• Collaborate with other health professionals following protocols for diagnosis and treatment.

• Build relationships with community partners to improve health outcomes and expand services.

• Counsel families regarding health maintenance and care.

• Coordinate referrals to other health or social agencies.

• Maintain accurate records of assessments, follow-ups, and evaluations.

• Supervise subordinate health workers and participate in meetings.

• Actively participate in quality improvement efforts.

Qualifications

Education and Experience
Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying.

• Education: Completion of an advanced nursing degree from a program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC), with emphasis in a Nurse Practitioner specialty area.

• Experience: Minimum of one (1) year of paid experience in an acute and/or ambulatory care setting.

THE ORGANIZATION

The City of Pacifica is a General Law City incorporated in 1957 and operates under the Council/Manager form of government. Legislative authority is vested in a five-member City Council elected by district for four-year overlapping terms. The Mayor is selected each year by a majority vote of the other Council members. The Council appoints the City Manager, City Attorney, and members of advisory commissions and committees.

Pacifica is a full-service city (approximately 188 FTE) with a proposed FY 2023/24 general fund budget of $40.1M. The City delivers municipal services through seven departments: the City Manager’s office (including the City Clerk, Economic Development, and contract City Attorney); Administrative Services (Finance, Human Resources and IT Divisions); Parks, Beaches, and Recreation; Planning; Public Works (Field Services & Engineering and Wastewater); Police; and Fire (via North County Fire Authority). Employees are represented by eight bargaining units.

Each year the City Council convenes in advance of the new fiscal year to review and amend as necessary City Council goals, and to develop a list of priorities to be implemented in the upcoming fiscal year to fulfill the goals. THE PUBLIC WORKS DEPARTMENT
The Public Works Department is organized into two Divisions: Field Services/Engineering and Wastewater.  The City has two Deputy Director positions, one that is responsible for Field Services/Engineering and the other that is responsible for Wastewater.  This position would be responsible for the Field Services/Engineering Division of Public Works, with a key emphasis on delivery of the Capital Improvement Program (CIP).  The Field Services section maintains parks, buildings, playing field and medians and keeps streets and trails clean and well-maintained.  Additionally, Field Services coordinates the tree program, cleans beaches and maintains the city’s storm drain system. The Engineering section implements the city’s CIP related to streets, roads, storm drains, facilities and coastal projects.  Additionally, the Engineering section pursues grants, manages consultants, reviews development projects and oversees the construction of the CIP and development within the City’s right of way. 

The Principal Civil Engineer will work under general direction, to direct, manage, supervise the Engineering Division; to perform highly complex professional engineering work; to plan, director, and supervise a variety of engineering projects and programs; and to conceptualize, develop, design, and implement all phases of difficult design and engineering research projects, to supervise and review the work professional and technical staff engaged in the preparation and/or review of plans, specifications, maps, designs, estimates, and inspections of capital improvement, construction, development and maintenance projects; coordinate the work of Engineering Division with the rest of Public Works.  

 

The Public Works Department is supported by 56 Full-Time staff and an operating budget of $21 million, along with FY2023-24 CIP appropriation of $26 million. The Engineering Division has an annual operating budget of $5 million, along with a FY2023-24 CIP appropriation of $18.5 million and is supported by 28 full-time staff. The Principal Civil Engineer reports to the Deputy Director of Public Works.

• Develop, plan, and implement goals and objectives of the Public Works Engineering Capital Improvement Projects; recommend and administer policies and procedures.
• Supervise, coordinate, and review all Public Works engineering activities including engineering design, construction inspection and surveying, as well as contract administration.

• Ensure that quality assurance and quality control plans are in place and being followed for engineering work. Verify computations, plans, specifications, and estimates for Public Works projects, review engineering drawings, and land record maps.
• Attend and participate in meetings and conferences, make project presentations to the City Council and other City commissions and committees, City administrators, public officials, professional organizations, contractors, County, Regional and State regulatory agencies regarding matters relating to areas of assigned responsibilities.

San Mateo Medical Center (SMMC) is seeking an experienced Patient Services Specialist (PSS) - Extra-Help.
 
The successful hire will support the clinical team with all registration, eligibility, enrollment, authorization, other administrative tasks, and Admission, Discharge, and Transfer (ADT) activities for SMMC. The PSS serves as the ambassador for SMMC to validate information and educate our patients and guests with the financial process upon arrival to the facility. The PSS verifies all demographic information, validates eligibility, obtain authorizations, collects co-payments, assist with all payor notifications and communication, and enroll patients if needed. The hire will be expected to work holidays and rotating weekends, if needed, to support department needs.
 
The ideal candidate for this position will possess excellent communication skills, including the ability to explain complex medical terminology and medical insurance requirements to individuals with varying levels of medical knowledge. Additionally, they should have strong skills in conflict resolution and problem-solving.

The ideal candidate will also be able to collaborate effectively with the clinical team, work with challenging patients, and assist those who may have language or communication barriers. This position requires keen attention to detail, strong organizational skills, and the ability to multi-task. Furthermore, the ideal candidate must be flexible and available for various shifts throughout the week.

NOTE: This is an extra-help, at-will assignment, paid on an hourly basis. Extra-help hours are dependent on the business needs of the department and therefore work hours may vary from week to week. Extra help employees shall not exceed 1,040 hours of work per fiscal year. Some extra help positions are eligible for benefits under the Affordable Care Act. Currently we have one Full-time Regular vacancy for a Spanish Speaking Psychiatric Social Worker or Marriage and Family Therapist in Daly City. Experienced clinicians and those looking to gain experience are encouraged to apply.

 

The outpatient clinician will provide specialty outpatient mental health services to adults living in the Northern regions of San Mateo County. The target population are adults 18 years of age and older presenting with a serious mental illness and severe functional impairments. Principal duties will include assessment and diagnosis, care coordination, case management, group and individual psychotherapy, crisis intervention and rehabilitation services. The Clinician will provide treatment in an outpatient clinic setting.

 

The ideal candidates will possess: 

• A current license as either a Licensed Clinical Social Worker or Marriage and Family Therapist;

General

 

The Engineering Manager will report to the Director, Bus Infrastructure and Capital Projects and will be responsible for the engineering functions of the infrastructure and capital project development department for the San Mateo County Transit District (SamTrans). 

 

The Bus Infrastructure and Capital Projects Department oversees the delivery of major bus fleet and facilities infrastructure projects for SamTrans. The department’s projects include design and construction of zero emission infrastructure (battery electric bus (BEB) charging infrastructure and hydrogen fuel cell electric bus (FCEB) infrastructure) for SamTrans's revenue and non­revenue fleet of nearly 400 vehicles, sea level rise and flood mitigation structures, bus administration, transportation, and maintenance buildings, and other structures at SamTrans' North Base and South Base Maintenance Facilities.

The Position:

The Assistant City Manager works directly for the City Manager in the Office of the City Manager. The Bilingual Spanish Psychiatric Social Worker / Marriage and Family Therapists will provide specialized psychiatric assessment, brief treatment, and clinical case management services for adult clients with Severe Mental Illness. Principal duties will include mental health and substance use assessment and treatment; engaging clients to services; consultation with providers; and linkage/referrals to other community providers. The main role of this position will be working within a dynamic multi-disciplinary team to provide specialized psychiatric casework and care coordination, provide individual and group psychotherapy, case management, and refer clients to Private Provider Network and/or community agencies for additional services.

 

The primary duties will include care coordination for a clinical case load, mental health assessments; treatment planning, case management; crisis management; individual psychotherapy; and collaborating with multidisciplinary team which includes psychiatrist, registered nurses, PSW SW and LMFT, and Case managers. Clinician will also perform “Officer of the Day” duties.

Supervising Mental Health Clinicians supervise and coordinate the professional work of mental health professionals, paraprofessionals and interns in San Mateo County and participate in program planning, administration, and evaluation; and provide specialized psychiatric casework counseling and therapeutic services to adults and youth in the field. The current vacancy is in Behavioral Health and Recovery Services (BHRS) Crisis and Outreach Team including the Psychiatric Emergency Response Team (PERT), the Crisis Response Team (CRT), Healthcare for the Homeless (HCH), and Homeless Engagement Assessment Linkage (HEAL) team.

 

Supervising Mental Health Clinicians are members of the Behavioral Health and Recovery Services Leadership Team and work with collaborative partners on system wide initiatives focused on improving services and service delivery. This position reports directly to a Clinical Services Manager.

 

Services provided include supervising mental health clinicians working in tandem with city law enforcement for behavioral health crises and/or homeless outreach mental health programs throughout the county, supervising the program specialist for the Crisis and Outreach Team, and conducting trainings and presentations for various partner agencies and the community.

The teams include psychiatrist, social workers, marriage and family therapists, SUD case manager, mental health counselor, and community workers who are consumers or family members. The clinical teams may also work with clinical interns.

 

The ideal candidate will possess:

• Knowledge of system of care principles.
• Knowledge of program management techniques and practices.
• Knowledge of crisis de-escalation, crisis evaluation, 5150-hold application, and community crisis response.
• Knowledge of crisis continuum of care.
• Strong supervisory/leadership skills, which demonstrate an ability to direct and motivate a multi-disciplinary team some of whom will be mobile and in disparate parts of the County.
• The ability to establish and maintain productive working relationships with consumers, families, professionals, law enforcement, Psychiatric Emergency Services (PES), homeless services providers, and the public; including close collaboration with and engagement of community partners to plan for, implement and coordinate needed services.
• Ability to make sound decisions based on the exercise of judgment and the consideration of all available information often with very little time during active crises.
• Demonstrated expertise and skill in clinical work with adults and youth with serious mental illnesses who may also have had contact with law enforcement and psychiatric emergency services.
• Knowledge of evidence based clinical practice modalities, e.g. motivational interviewing, solution focused brief treatment, cognitive behavioral therapy, wellness and recovery principles, and trauma informed care, for examples.
• Knowledge of Medi-Cal and Medicare regulations and managed mental health care.
• Strong knowledge of Avatar, EPIC and other tools or databases used by BHRS for information and charting.
• Strong clinical assessment, diagnostic and evaluation skills.
• Experience working with clients of diverse cultures and backgrounds.
• Inpatient and outpatient clinical experience.
• Experience working with clients on the streets and in shelters.
• Collaborative experience or knowledge of the Criminal Justice system.

 

As the Supervising Mental Health Clinician, your responsibilities will include, but are not limited to, the following:

• Consulting with clinicians in the field to determine appropriate courses of action during active crises that may involve law enforcement.
• Tracking data for established and pilot programs and reporting these statistics to leadership.
• Facilitating regular meetings with clinicians and system partners collaborating with the Crisis and Outreach Team.
• Providing ongoing clinical training for field clinicians and law enforcement, including the Quarterly Crisis Intervention Training (CIT).
• Responding to active crisis scenes when required.
• Addressing and supporting requests for the deployment of the BHRS Crisis Response Team (CRT).
• Overseeing the daily operations of programs under the Crisis and Outreach umbrella.
• Performing all other duties or projects as assigned.

 

Qualifications

Licensure/Certification : Possession of a California license in at least one of the following: Clinical Psychology, Clinical Social Worker, or Marriage and Family Therapist.
