Responsibilities

• Perform initial diagnostic evaluation noting behavioral symptoms which may indicate organic and/or mental disorders in children and in interpersonal/ family dysfunctions related to child and adult patients/clients.
• Determine and develop appropriate treatment plans for all clients in the caseload.
• Conduct individual, group and family counseling/therapy sessions for clients toward the goal of achieving more adequate, satisfying and productive adjustments in the community and provide case management services as appropriate.
• Participate in developing individual and group treatment plans utilizing a variety of psychotherapeutic, psychosocial and psycho educational modalities.
• Perform suicide/risk assessments.
• Conduct home visits and/or field-based services.
• Crisis Intervention. Make client referrals and plan discharge procedures.
• Assess progress of clients on a selected basis and propose modifications to rehabilitation plans in order to meet goals agreed upon by the clients/patients.
• Prepare and maintain psychosocial case histories for use in diagnostic evaluations and of patient progress.
• Supervise graduate interns.
• Provide outreach counseling and refer clients to appropriate mental health program service resources.
• Collect and evaluate medical or psychosocial data such as case histories, behavior patterns and family relationships of patients/clients.
• Analyze factors which exert an adverse effect upon health problems, treatment, or recovery, or which may contribute to or are responsible for mental illness or affect the potential for rehabilitation.
• Conduct treatment groups in schools and other community settings with emotionally and behaviorally disturbed children toward the goal of achieving higher functioning in interpersonal relationships and learning.
• Work as a psychiatric team member with other treatment disciplines.
• Utilize Trauma Informed Care and other Evidence-based practices.
• Provide consultation and education services.
• Ability to participate in an on-call rotation and responding to after-hours calls (Youth Services Center).
• Ability to participate in an on-call rotation and responding to after-hours calls from Psychiatric Emergency Services (Youth Case Management).
• Perform related duties as assigned.

NOTE: This is an extra-help, at-will assignment, paid on an hourly basis. Extra-help hours are dependent on the business needs of the department and therefore work hours may vary from week to week. Extra help employees shall not exceed 1,040 hours of work per fiscal year. Some extra help positions are eligible for benefits under the Affordable Care Act. Extra help employees are not guaranteed permanent status at the end of the assignment.

OASIS is community field-based mental health services provided to older adults at their place of residence. This home visit services include medication evaluation and monitoring, individual counseling/therapy, and case management. This multiple discipline team with language capacity in English, Spanish, and Chinese, consists of Psychiatrists, Licensed Clinical Social Workers, Licensed Marriage & Family Therapists, a Community Mental Health Nurse, and a Peer Support Worker. Services are available to San Mateo County residents age 60 and over with issues of mobility and mental health conditions that impact their day-to-day functioning.  

Clinicians will use evidence-based approach to perform assessment, diagnosis, make medical necessity treatment plans, case management, individual therapy, and work with family members of clients' and other community providers for clients' mental health follow-up care needs. These interventions are provided in field setting or in clients' place of residence, and/or as needed to clients who are living in the community.

Clinicians are committed to providing medical necessity treatment that supports the clients' goals of independence and self-sufficiency with other service providers' support to continually live in the community with better quality of life. San Mateo County Health is seeking an experienced individual for the position of Public Health Clinics Process Improvement Manager that plans, oversees, and provides a wide variety of administrative, analytical, and work-flow support on a County-wide level; analyzes practices and procedures and makes recommendations for organizational, operational, policy, and procedural improvements; conducts needs analysis, feasibility studies, and program evaluation for assigned projects and programs; develops, summarizes, and maintains administrative and fiscal records; fosters cooperative working relationships among County departments and acts as liaison with various community, public, and regulatory agencies; and performs related work as required.

This management position will report to the Public Health Clinics Manager, overseeing Edison Clinics and Mobile Clinics and will have broad project management responsibility for key strategic efforts affecting the Public Health clinics, and will exercise direct and general supervision over assigned professional, technical, and office support staff.

The Public Authority for In-Home Supportive Services (IHSS) is seeking well-qualified individuals for the position of Public Authority Registry Social Worker II/III with fluency in Mandarin or Cantonese. There is one vacancy in the Public Authority. The current vacancy would be responsible for supporting IHSS consumers who do not have caregiver resources, in hiring a homecare provider from the Public Authority Registry and helping the client and provider establish a successful working relationship. The position is located in San Mateo, CA, but will require travel throughout the County to perform duties of the positions.

NOTE: The Public Authority Registry Social Worker II/III is not a County position; the Public Authority unit reports to the San Mateo County Director of Aging and Adult Services.

PUBLIC AUTHORITY

The Public Authority serves as the employer of record for IHSS homecare workers, with the IHSS consumer retaining the right to hire, fire and train their worker. The Public Authority's responsibilities include operating a Registry that recruits, screens and refers potential homecare providers to IHSS consumers needing personal assistance in their home; screening the backgrounds of IHSS providers in accordance with IHSS policies and regulations; providing training opportunities to IHSS providers to enhance their skills in serving IHSS consumers; and offering support and assistance to Registry consumers and providers to ensure the employer-employee relationship between the Registry consumer and provider are functioning well. The IHSS consumer is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive. Consumers who do not have caregiver resources, may choose to hire a caregiver from the Public Authority Registry. The Public Authority Social Worker will assist those recipients with the hiring of a Public Authority Registry worker and help them to manage their role as the employer.

These positions would provide outpatient and/or inpatient services physical therapy evaluations and treatments for a variety of orthopedic and neurologic conditions.

Physical Therapists manage patients with a wide range of diagnoses, chronic and acute, and are part of a dynamic treatment team. In addition, Physical Therapists identify patient challenges and develop goals and a care plan with reasonable outcomes for the individual's life/work settings. Daily opportunities are created in this assignment through the variety of illnesses/injuries seen in our patient population.

Physical Therapists work at SMMC as part of a County rehabilitation program, which provides evaluation, education and treatment. The therapeutic program at SMMC is oriented toward inpatients and outpatients with acute and chronic problems while providing therapy to inpatients with a need for long or short-term rehabilitation.

The ideal candidate will possess a minimum of one year of work experience providing care in an outpatient setting with an interest in covering inpatient when the census justifies. Qualifications

Licensure/Certification:

• Eligibility for or registration as an Occupational Therapist by the National Board for Certification in Occupational Therapy. Failure to obtain this licensure within twelve months of employment may result in termination.
• Possession of a Certificate of completion for a course in Basic Life Support as approved by the American Red Cross or American Heart Association; or must be obtained within six months of appointment.

Occupational Therapist II: One year of supervised experience in occupational therapy.

Knowledge of:

• Principles and methods of occupational therapy, as applicable to all types of cases found in a general hospital providing acute care or in a rehabilitative center.
• Physical and psychological problems of physically disabled, aged, gero-psychiatric and chronically ill patients.
• Principles of splint design and fabrication.
• Use and care of therapeutic tools and equipment commonly used in occupational therapy.
• Professional ethics and the fundamental laws of practice for occupational therapy.

• Administer and document occupational therapy treatments.
• Assess patient needs, analyze situations accurately and implement an effective plan of action.
• Adapt equipment to patient needs.
• Integrate occupational therapy services with other medical and institutional programs.

San Mateo County Health is seeking an experienced individual for the position of Public Health Clinics Strategic Projects Manager. This management position will report to the Public Health Clinics Manager, overseeing Edison Clinics and Mobile Clinics and will have broad project management responsibility for key strategic efforts affecting the Public Health clinics.
 
There is currently one regular vacancy in the Public Health, Policy and Planning Division located in San Mateo, CA.
 
Primary responsibilities will include, but are not limited to, the following:  

• Quality assurance and improvement:  build and visualize data in management reports and dashboards; convert data to information and use it to guide process improvement.
• Process improvement:  facilitate, document, track, organize, measure progress; e.g., conduct improvement work within daily huddles.
• Project management:  lead projects and provide ad hoc support of projects.
• Knowledge management:  inventory and maintain infrastructure of core programmatic documents and processes, how files are stored and shared.
• Partnership building:  support internal staff operational needs; coordinate with other stakeholders, such as staff from Behavioral Health & Recovery Services, Human Services Agency, Health IT, San Mateo Medical Center, grant makers, state agencies, etc.
• Management of administrative staffing:  supervise intermittent interns and volunteers.
• Contract and MOU support:  assist Clinics Manager to write scopes of work, produce required reports and invoices for categorical funding and operational agreements.
• Revenue optimization support:  oversee billing quality assurance and improvement, facilitate braided funding opportunities and oversight.
• Policy and procedures facilitation:  maintain inventory and lead update processes through completion, including occasional initial literature reviews or research and coordination of stakeholders.
• Change management and leadership synchronization:  coordinate staff engagement, training, feedback to ensure successful system changes, take meeting minutes, contribute to agendas, track on-going project work in support of meeting goals.

Ideal candidate will possess:  

• Experience with project management and process improvement as the lead.
• Knowledge and/or experience with:  County of San Mateo; health; people experiencing homelessness; HIV and Sexually Transmitted Infections (STIs).

Equivalent to graduation from an accredited four-year college or university with major coursework in business administration, public administration, or a related field. At least one year of professional administrative experience is typically required at the Associate Management Analyst level, and at least 3 years for the Management Analyst level.

Licenses and Certifications: 

• Depending upon assignment, possession of a valid California Driver's License.
