ABOUT SFHN PRIMARY CARE: Primary Care is at the center of San Francisco Health Network’s complete system of care. Our fourteen (14) health centers/programs, provide high-quality, compassionate care to the City’s children, youth, adults, and elders regardless of immigration status or ability to pay.

On-site specialty services include prenatal care and family planning services; integrated behavioral health and substance use services; dental, gender health, nutrition, pharmacy, and podiatry services; integrative health services such as acupuncture; and care for people experiencing homelessness and refugees/asylum seekers.

Our Primary Care teams partner with University of California, San Francisco (UCSF) faculty and staff who provide acute, specialty, skilled nursing, and rehabilitation care; teach health care trainees; and conduct research at Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG).

Job Description

Under general medical direction of the site medical director and/or Primary Care Chief Medical Officer, the 2230 Physician Specialist functions within an organized system for the delivery of health care in association with other members of the health care team and performs specialized duties in clinical settings. The essential job functions of the 2230 Physician include the following: 

  • Provides primary care, urgent care and relevant office-based procedures to patients consistent with their medical specialty area through in-person, telehealth, and/or group visits 
  • Provides telephone advice to patients 
  • Provides patients with health education, counseling, and self-management support 
  • Provides clinical case management for patients with complex care needs as indicated 
  • Maintains accurate, timely and complete electronic medical records  
  • Participates in staff meetings, project meetings, and conferences when indicated 
  • Participates in quality improvement initiatives 
  • Works effectively as part of a multidisciplinary team 
  • Provides consultative support to other members of the care team 
  • Collaborates with identified project staff on special projects 
  • Participates in after-hours remote on-call medical advice pool 
  • Performs other related duties as assigned/required 

Qualifications

Required

  1. Possession of a valid license to practice medicine issued by the Medical Board of California or the Osteopathic Medical Board of California*
    AND
  2. Successful completion** of a residency program accredited by the Accreditation Council for Graduate Medical Education or American Osteopathic Association in the appropriate medical specialty area for the assigned facility or division (i.e., Board Eligible)
    AND
  3. Possession of valid Drug Enforcement Agency registration with the United States Department of Justice***

*Applicants possessing a valid license to practice medicine issued from another state within the United States of America may apply, but if selected, the candidate will NOT be appointed/hired until they obtain a valid license to practice medicine issued by the Medical Board of California or the Osteopathic Medical Board of California.

**Applicants enrolled in a residency program may apply, but if selected, the candidate will NOT be appointed/hired until they demonstrate successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education or American Osteopathic Association in a Psychiatric medical specialty area.

Desirable 

  • Lived and/or work experience with Black, Indigenous, and/or People of Color (BIPOC) populations 
  • Ambulatory care experience in an urban environment serving diverse, low-income populations experiencing complex medical and psychosocial challenges 
  • Understanding of how systemic racism, white supremacy, and other forms of bias affect patient engagement, retention, and health outcomes 
  • Sensitivity to and experience working with LGBTQ+ individuals 
  • Experience working in multi-disciplinary team-based care and one or more of the following: trauma-informed care, health care for the homeless, harm reduction, or chronic care management models 
  • Fluency in one or more of the following languages: Cantonese, Mandarin, Russian, or Spanish 
  • Ability to work with staff from diverse social and economic backgrounds with respect, understanding, and teamwork -- including and especially staff from non-clinical disciplines 
  • High attention to detail and ability to meet deadlines 
  • Ability to manage multiple, complex tasks concurrently 
  • Ability to work in a fast-paced, demanding environment 
  • Excellent interpersonal communication and customer service skills 
  • Commitment to reducing health disparities and actively contributing to our goal to become an anti-racist organization 

Conditions of Employment
Some positions may require valid Board Certification in medical specialty areas including, but not limited to: Family Medicine, Internal Medicine, Pediatrics, etc. If you have valid Board Certification in a medical specialty area, please respond accordingly on the Required Questionnaire.

All qualified candidates who have been selected for appointment to positions in all specialty areas must be an eligible billable provider and will be required to meet the following criteria:

  • Be eligible to participate in Medicare, Medicaid and/or other federal health care programs;
  • Possess a National Provider Identifier (NPI);
  • Submit a completed credentialing application and/or required documentation for credentialing; AND
  • Possess a valid third-party billable provider certification (such as Medicare, Medi-Cal and/or private insurance) OR have submitted a completed billable provider application, along with the required documentation, in order to obtain the appropriate billable provider status.

These criteria and 'Conditions of Employment' must be met within two (2) weeks prior to the start work date to avoid delay of the appointment to the position and/or cancellation of an employment offer.
