The Extra Help Employee - BH (Harmony House Coach) position provides support, education, and advocacy to persons receiving services through the Mental Health Services Act. The Harmony House Coach may also provide a variety of services related to the operations of the Harmony House Drop-In Center and the Full-Service Partnership Program. This position will provide welcoming and supportive activities, as well as perform tasks in support of facility operations at the center. This position is at-will, temporary, part-time, and scheduled for up to twenty-five (25) hours per week. 

WHAT YOU'LL DO

• Model unconditional commitment to assisting the consumers in "tough times", "over the long haul" and to do "whatever it takes" to provide service to the client/consumer.
• Perform group education for clients/consumers and/or support persons and provide ongoing support as needed. 
• Be available for consumers, in the event of a crisis, for support.
• Set up, prepare, and plan events for consumers and drop-in center guests.
• Make program presentations to community stakeholders and groups. 
• Coordinate activities with other agencies/individuals engaged in providing or developing support for the consumers who are in the program. 

The City of Foster City is recruiting for a part-time Public Works Maintenance Worker in the Public Works Department. The is a part-time non-benefited position working approximately 20 hours per week, and no benefits are provided. 

Important and Essential Responsibilities 
Operates hand and power tools, light and heavy construction equipment and specialized wastewater equipment. Handles and applies chemicals. Maintains, operates, inspects, repairs and installs water distribution system facilities including piping, storage tanks, meters, hydrants, valves, pressure reducing stations, pumping stations, pumps, controls, measuring devices, cathodic protection systems, communication systems, and related appurtenances.

Education and Training Guidelines: 
Any combination equivalent to experience and education that could likely provide the required knowledge, skills and abilities would be qualifying. A typical way would be:

Education: Completion of a job or technical training program in maintenance or construction; completion of high school or the equivalent. 

Experience: One year of experience in general maintenance or construction. 

Licenses, Certificates, Registration: 

• Possession of a valid California Class "C" Driver’s License and maintain license and a good driving record.

Selection Process
Applications will be reviewed for relevant education, experience, training and other job related qualifications. Based upon a review of employment applications received, a limited number of the qualified applicants will be invited to participate in the examination process. 

The process will consist of an Oral Panel Interview. An eligibility list will be established from those who pass the examination process with a score of 70% or higher. POSITION

Depending on assignment, essential duties may include but are not limited to the following:

• Removal and clearing of debris from gutters, culverts, and drainage structures.
• Perform custodial and general maintenance of City facilities.
• Perform pick and shovel work; assists in concrete work; operates power machinery and a variety of hand tools.
• Assists in the setup of equipment (tables, chairs, canopies, etc.) and bleachers for special events.
• Read and accurately record water consumption data for all accounts, including construction meters; investigate and resolve meter irregularity issues.
• Complete service orders such as service turn-ons, turn-offs, and re-reads; deliver door hanger notices as necessary, this may include interaction with customers to gain access and/or discuss related issues. 
• Perform meter and meter box maintenance functions to improve meter readability; test meters for proper operations; perform meter replacements and minor meter repair work.
• Assist Utilities Division with water and sewer maintenance and repair efforts, such as manholes, fire hydrants, valves, meters, vaults, boxes, and lids.
• Perform a variety of unskilled and semi-skilled work in the maintenance and repair of beach facilities, including dumping trash cans, picking up debris along waterline, cleaning rest rooms, driving 4-wheel drive trucks on beach, performing minor plumbing and building maintenance, removing graffiti, painting, hand-picking litter and other related duties as assigned.

• Conduct intake assessment which includes, psycho-social functioning, family and peer relationships, cultural/language needs and family strengths; develop care plans that respond to the individual’s or families’ values and strengths.
• Establish clinical diagnoses and identify list of problems and needs.
• Provide individual, family and group counseling to enhance clients’ mental and emotional health, social functioning and independent living skills, using a variety of therapeutic and counseling techniques.
• Provide therapy and supportive services to individuals /children and families from varying cultural backgrounds.

• Comply with City, state, county, funders and agency’s policies and procedures, legal and ethical standards, quality assurance goals and audit requirements.

• Write and maintain clinical records following City and Funders’ guidelines. Attend quality chart review, case consultation meetings.
• Meet and maintain billing goals on a monthly basis.

• Attend community meetings, participate in task forces and committees as assigned.
• Conduct classes and give presentations to community organizations as assigned.
• Conduct services where client is most comfortable and are most appropriate including office-based sessions, field based, hospital, telehealth, school and home-based sessions.
• May provide clinical supervision to entry level interns or trainees, training and assistance to other staff or volunteers.
• Utilize departmental electronic health record and billing system MyEvolv.
• May be required to work other than business office hours.

The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

Examples of Duties

- Manage a caseload of 28-32 clients comprised of older individuals with complex medical conditions.
- Conduct comprehensive client assessments, and develop and monitor service plan.
- Collaborate with team members, Human Service programs, and community agencies.
- Conduct home visits and provide other community interventions as needed.
- Conduct inter-agency and/or family conferences.
- Advocate for clients in accessing services.
