San Mateo County Health's Public Health, Policy and Planning Division has exciting field-based opportunities for well-qualified and motivated Nurse Practitioners to provide both primary and urgent care to adults from vulnerable populations within our community—people experiencing homelessness and/or in shelters, monolingual Spanish speakers and other new immigrants, and those without insurance or underinsured. These services are provided through various Mobile Clinic modalities and venues, including but not limited to street medicine, shelter medicine, and the mobile coach. We are looking for collaborative Nurse Practitioners to work in and lead various multidisciplinary service teams in the field to provide comprehensive, low-threshold, client-centered care and engagement.

Teams are staffed by a Nurse Practitioner (NP), Psychiatrist, Medical Services Assistant, AOD Case Manager, and Patient Services Assistants. These teams work closely with our community partners from the Human Services Agency and community-based organizations who locate and identify clients needing medical and/or mental health services, as well as provide additional health, housing, and social services in the field.

Under general supervision, Nurse Practitioners provide urgent care and primary health care services such as physical examinations, chronic disease management (e.g., diabetes and hypertension), and referrals/linkages to specialty clinics as appropriate.

Currently, Public Health Clinics has one full-time, regular, 40-hour-per-week vacancy for a bilingual Spanish Nurse Practitioner and two 40-hour-per-week, Limited-Term Nurse Practitioner vacancies, requiring flexible schedules and field-based work at different locations. San Mateo County Health- San Mateo Medical Center seeks well-qualified candidates for the position of Program Coordinator I in the Patient Access Division. The current vacancy is a full-time, limited term vacancy. The duration of the assignment will last about 6 months.

 

Under the direction of the Patient Access Manager, the Program Coordinator will provide support to over 150 Patient Services Assistants across various departments, including Emergency, Admitting, Long-Term Care, Same-Day Surgery, and will average 27 Federally Qualified Health Center (FQHC) sites with 8-16 hours of weekly program refresher training on several publicly sponsored coverage options.

 

Additionally, this position will assist the team with several EPIC post-go-live projects, and work queues as we work to revamp processes and enhance collaboration for improved workflows and revenue-generating process efficiencies.

As a Supervising Public Health Nurse, you will have the opportunity to direct the Family Support Services Unit or the Communicable Disease Prevention and Control Unit. San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the unclassified position of In-Home Supportive Services Social Worker II/III. Social Workers provide a wide variety of welfare services in such areas as information and referral, protective services, out-of-home services, health related services, chore services, and other welfare services within the Health and the Human Services Agency.

The current unclassified vacancy is in the In-Home Supportive Services (IHSS) Program that provides in-home care for persons who cannot safely remain in their own home without such assistance. The recipients of IHSS are aged, blind and/or disabled and eligible for Medi-Cal. Disabled and/or blind minor children may also be eligible for IHSS. The Social Worker will make a home visit to determine eligibility for the program and to conduct an assessment to determine a range of service hours for particular tasks. The IHSS recipient or guardian is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive. The Social Worker will travel throughout the County to conduct home visits, attend meetings and perform other duties of the position. The Social Worker will work in the San Mateo, CA Aging and Adult Services Office.

This IHSS Social Worker will conduct intake and/or on-going activities for IHSS recipients. Some of those activities include:

• Conduct initial IHSS assessments using a detailed State assessment tool.

Job Description

Under the supervision of a pharmacist, the Pharmacy Technician supports the provision of pharmaceutical care to patients served by the San Francisco Department of Public Health. Assists registered pharmacists in performing nonprofessional duties in an institutional pharmacy; receives, inspects, stores, and fills orders for pharmaceutical supplies. Nature of work involves continuous close attention to detail and some potential exposure to hazardous substances.

Under supervision, the Pharmacy Technician performs the following essential job functions:

• Accurately prepares unit dose medications, compounds a variety of pharmaceuticals under the supervision of a Pharmacist, including intravenous admixtures according to proper aseptic technique and in accordance with, professional standards and departmental policies and procedures, in order to maintain required levels of supply and to provide patients with needed medications;
• Prepares or repackages drugs and pharmaceuticals under the supervision of a Pharmacist, in amounts needed to fill written orders/prescriptions in order to provide patients with needed medication and to maintain required levels of supply;
• Prepares medication labels based on written orders from physicians and other professional staff according to California State Board of Pharmacy requirements;
• Records billing information for accurate and appropriate billing of patients for medications, involving use of desktop computer keyboard for entering medication orders and billing information;
• Maintains drug and pharmaceutical stock levels by counting and checking stores of drugs and pharmaceuticals against standard, pre-set stock levels, ordering and verifying deliveries against invoices to ensure accuracy;
