San Mateo County Health's Public Health, Policy and Planning Division has exciting field-based opportunities for well-qualified and motivated Nurse Practitioners to provide both primary and urgent care to adults from vulnerable populations within our community—people experiencing homelessness and/or in shelters, monolingual Spanish speakers and other new immigrants, and those without insurance or underinsured. These services are provided through various Mobile Clinic modalities and venues, including but not limited to street medicine, shelter medicine, and the mobile coach. We are looking for collaborative Nurse Practitioners to work in and lead various multidisciplinary service teams in the field to provide comprehensive, low-threshold, client-centered care and engagement.

Teams are staffed by a Nurse Practitioner (NP), Psychiatrist, Medical Services Assistant, AOD Case Manager, and Patient Services Assistants. These teams work closely with our community partners from the Human Services Agency and community-based organizations who locate and identify clients needing medical and/or mental health services, as well as provide additional health, housing, and social services in the field.

Under general supervision, Nurse Practitioners provide urgent care and primary health care services such as physical examinations, chronic disease management (e.g., diabetes and hypertension), and referrals/linkages to specialty clinics as appropriate.

Currently, Public Health Clinics has one full-time, regular, 40-hour-per-week vacancy for a bilingual Spanish Nurse Practitioner and two 40-hour-per-week, Limited-Term Nurse Practitioner vacancies, requiring flexible schedules and field-based work at different locations. Sixty-five percent of the NP’s time will be in the field, with the remainder spent in the office documenting notes, placing orders, and coordinating with other staff as needed.

Duties may include, but are not limited to, the following:

• Provide low-threshold, comprehensive care for San Mateo County’s most marginalized and vulnerable populations, including people experiencing homelessness, those in shelters, migrant and immigrant farmworkers, the recently incarcerated, the uninsured, and those with financial challenges.

• Provide comprehensive and urgent care in a variety of non-traditional settings, including homeless encampments, outdoor locations, shelters, and a mobile RV clinic.

• Demonstrate the ability to work closely and collaboratively with a small team in often challenging environments (e.g., homeless encampments, shelters, extreme weather, individuals under the influence, or those with mental health challenges).

• Take stewardship of medical care for assigned geographic populations and/or shelters.

• Outreach to and build long-term, trust-based relationships with vulnerable populations.

• Assess patients’ physical, emotional, and social status through medical history, physical examinations, and diagnostic screenings; evaluate clinical findings and lab results.

• Identify and manage common illnesses and other health problems.

• Refer urgent issues beyond scope to appropriate specialists.

• Deliver longitudinal care in assigned areas.

• Collaborate with other health professionals following protocols for diagnosis and treatment.

• Build relationships with community partners to improve health outcomes and expand services.

• Counsel families regarding health maintenance and care.

• Coordinate referrals to other health or social agencies.

• Maintain accurate records of assessments, follow-ups, and evaluations.

Qualifications

Education and Experience
Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying.

• Education: Completion of an advanced nursing degree from a program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC), with emphasis in a Nurse Practitioner specialty area.

• Experience: Minimum of one (1) year of paid experience in an acute and/or ambulatory care setting.

The City of San Bruno Fire Department offers an exciting opportunity to serve as a Fire Cadet. Our one-year Fire Cadet program is an unpaid, volunteer work experience program that offers hands-on training, and introductory knowledge of firefighting skills, fire service tools and equipment. Cadets respond to emergency calls, render Basic Life Support and participate in training, and participate in the daily station duties.

Cadets must be willing to commit to 24-hour shifts, two to four times per month. The cadet program is one year in length. Upon completion, this program meets the manipulative training and experience requirements in obtaining a California State Fire Marshal’s Office Firefighter II certification.

The Department

The Human Services Department (HSD) delivers and supports services by forging long-term community partnerships; engaging with and building the capacity of the community to do its own problem-solving; and leveraging financial and volunteer resources.

The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

San Mateo Medical Center (SMMC) is seeking an experienced Patient Services Specialist (PSS) - Extra-Help.
 
The successful hire will support the clinical team with all registration, eligibility, enrollment, authorization, other administrative tasks, and Admission, Discharge, and Transfer (ADT) activities for SMMC. The PSS serves as the ambassador for SMMC to validate information and educate our patients and guests with the financial process upon arrival to the facility. The PSS verifies all demographic information, validates eligibility, obtain authorizations, collects co-payments, assist with all payor notifications and communication, and enroll patients if needed. The hire will be expected to work holidays and rotating weekends, if needed, to support department needs.
 
The ideal candidate for this position will possess excellent communication skills, including the ability to explain complex medical terminology and medical insurance requirements to individuals with varying levels of medical knowledge. Additionally, they should have strong skills in conflict resolution and problem-solving.

The ideal candidate will also be able to collaborate effectively with the clinical team, work with challenging patients, and assist those who may have language or communication barriers. This position requires keen attention to detail, strong organizational skills, and the ability to multi-task. Furthermore, the ideal candidate must be flexible and available for various shifts throughout the week.

NOTE: This is an extra-help, at-will assignment, paid on an hourly basis. Extra-help hours are dependent on the business needs of the department and therefore work hours may vary from week to week. Extra help employees shall not exceed 1,040 hours of work per fiscal year. Some extra help positions are eligible for benefits under the Affordable Care Act. The Manager, Track & Structures reports to the Deputy Director of Infrastructure Engineering and is responsible for overseeing railroad track and structures engineering support for the Peninsula Corridor Joint Powers Board (Caltrain)

ABOUT THE ROLE

Reporting to the Executive Director, the Deputy Director will oversee department-wide administrative operations and strategic initiatives including organizational planning, program performance, budget development, financial accounting, procurement, contracting, human resources, recruitment, and other administrative functions. This role will supervise three staff members in the Operations Division and closely collaborate with other staff to achieve the Commission’s strategic goals.

• A Master's degree in public administration, business administration, public policy, or a closely related field of study, or a juris doctor degree.

• Two or more years of demonstrated experience with public sector management, budgeting, financial accounting, procurement, contracting, and/or recruitment functions.
• Experience in the fields of campaign finance and governmental ethics.
• Experience working objectively, efficiently, and effectively under strict deadlines and highly visible, sensitive, or politicized settings.
• Extensive knowledge and expertise in organizational policies, systems, and issues in a government setting.
• Proficiency with computers and software applications, including with Microsoft Office tools, electronic filings systems, collaboration and project management tools (SharePoint, MS Teams, Jira or similar), financial & accounting systems, digital signature tools (DocuSign or similar), and content management systems (WordPress or similar).
• Familiarity with data analysis skills including advanced Excel, PowerBI, Tableau, or other statistical/analytics tools. Comfortable working with complex datasets and databases.
• Strong leadership and management skills and experience working effectively with diverse stakeholders in fast-paced, sensitive, and politicized settings.
• Ability to operate as an effective tactical and strategic thinker.
• Exceptional written and verbal communication skills with excellent presentation and organizational skills.
• Adaptability and flexibility in handling changing circumstances in dynamic work environments.
• Ability to plan, prioritize, exercise initiative, and complete multiple competing projects within established timeframes while also ensuring accurate, high-quality work products.

At a 24 hour a day police dispatch center, you will be using a sophisticated Computer Aided Dispatch terminal. A Communications Dispatcher receives emergency calls from the public requesting police, fire, medical, or other emergency service; determine priority, and dispatch police, or other emergency units as necessary and in accordance with established procedures. 

Typical duties may include, but are not limited to the following:

• Maintaining contact with all units on assignment and maintaining status and location of police field units.
• Coordinating emergency calls (911) and relaying information and assistance requests involving other agencies.
• Entering, updating, and retrieving information from networks relating to wanted persons, stolen property, vehicle registration, stolen vehicles and other information.
