San Mateo County Health is seeking an experienced individual for the position of Public Health Clinics Strategic Projects Manager. This management position will report to the Public Health Clinics Manager, overseeing Edison Clinics and Mobile Clinics and will have broad project management responsibility for key strategic efforts affecting the Public Health clinics.
 
There is currently one regular vacancy in the Public Health, Policy and Planning Division located in San Mateo, CA.
 
Primary responsibilities will include, but are not limited to, the following:  

• Quality assurance and improvement:  build and visualize data in management reports and dashboards; convert data to information and use it to guide process improvement.
• Process improvement:  facilitate, document, track, organize, measure progress; e.g., conduct improvement work within daily huddles.
• Project management:  lead projects and provide ad hoc support of projects.
• Knowledge management:  inventory and maintain infrastructure of core programmatic documents and processes, how files are stored and shared.
• Partnership building:  support internal staff operational needs; coordinate with other stakeholders, such as staff from Behavioral Health & Recovery Services, Human Services Agency, Health IT, San Mateo Medical Center, grant makers, state agencies, etc.

• Contract and MOU support:  assist Clinics Manager to write scopes of work, produce required reports and invoices for categorical funding and operational agreements.
• Revenue optimization support:  oversee billing quality assurance and improvement, facilitate braided funding opportunities and oversight.
• Policy and procedures facilitation:  maintain inventory and lead update processes through completion, including occasional initial literature reviews or research and coordination of stakeholders.
• Change management and leadership synchronization:  coordinate staff engagement, training, feedback to ensure successful system changes, take meeting minutes, contribute to agendas, track on-going project work in support of meeting goals.

San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the position of In-Home Supportive Services Social Worker II/III. Social Workers provide a wide variety of welfare services in such areas as information and referral, protective services, out-of-home services, health related services, chore services, and other welfare services within the Health and the Human Services Agency.
 
The current vacancy is in the In-Home Supportive Services (IHSS) Program that provides in-home care for persons who cannot safely remain in their own home without such assistance. The recipients of IHSS are aged, blind and/or disabled and eligible for Medi-Cal. Disabled and/or blind minor children may also be eligible for IHSS. The Social Worker will make a home visit to determine eligibility for the program and to conduct an assessment to determine a range of service hours for particular tasks. The IHSS recipient or guardian is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive.  The Social Worker will travel throughout the County to conduct home visits, attend meetings and perform other duties of the position.  The Social Worker will work in the San Mateo, CA Aging and Adult Services Office.
 
This IHSS Social Worker will conduct intake and/or on-going activities for IHSS recipients. Some of those activities include:
 

• Conduct initial IHSS assessments using a detailed State assessment tool.
• Complete necessary documents, paperwork, and work within different information technology systems.
• Conduct annual reassessments.
• Educate, support, and perform administrative duties for IHSS recipients and care providers to understand/comply with program regulations.

Social Workers provide highly skilled, intensive casework services to individuals and their families and provide casework consultation within the department and to outside agencies.  Incumbents of positions at this level work with minimal supervision and are expected to have acquired the high degree of assessment skills required to perform care coordination and referral tasks in complex casework areas. The Bilingual Spanish Psychiatric Social Worker / Marriage and Family Therapists will provide specialized psychiatric assessment, brief treatment, and clinical case management services for adult clients with Severe Mental Illness. Principal duties will include mental health and substance use assessment and treatment; engaging clients to services; consultation with providers; and linkage/referrals to other community providers. The main role of this position will be working within a dynamic multi-disciplinary team to provide specialized psychiatric casework and care coordination, provide individual and group psychotherapy, case management, and refer clients to Private Provider Network and/or community agencies for additional services.

Crisis Response Clinician I: Under direct supervision of the Crisis Response Supervisor, provides field and office based professional behavioral health counseling, psychoeducation, outreach and engagement, case management, consultation, and quality assurance review of assigned cases; and performs related other work as required or assigned.

MINIMUM QUALIFICATIONS

DISTINGUISHING CHARACTERISTICS
Contract Administrator I is the entry and first working level class in the Contract Administrator series. Initially, under close supervision, incumbents perform basic contract administration duties while learning County and programmatic policies, procedures, and guidelines. As experience is gained, there is greater independence of action within established guidelines. This class is flexibly staffed with the Contract Administrator II and incumbents may advance to the higher level after gaining experience and demonstrating proficiency, which meets the qualifications of the higher-level class.

Contract Administrator II is the fully experienced, journey-level class in this series, capable of independently performing the full range of contract administration duties. Employees are normally given substantial latitude in determining approach, procedures, systems, and methods in accomplishing the work goals. They make basic programmatic decisions that require the exercise of independent judgment and are responsible for coordinating multiple functions or processes. Incumbents at this level are typically assigned the more complex contracts.

 

The ideal candidate will demonstrate the ability to:

• Support and/or lead complex procurement processes by applying knowledge of spending thresholds and competitive requirements, and County policies.
• Draft clear and comprehensive purchasing documents, including scopes of work, contracts, and amendments.

• Coordinate the full lifecycle of contract development and amendments in collaboration with division managers, including drafting scopes of work, facilitating formal competitive processes, securing approvals, and maintaining comprehensive contract files.

San Mateo County Health is seeking a highly qualified candidate for the position of Edison Clinic Therapist (Psychiatric Social Worker I/II).

Under general supervision, the Clinic Therapist will work approximately 40 hours per week, providing a wide variety of therapeutic services and, if time permits, up to 10 hours of social work case management services. Responsibilities include screening, assessment, evaluation, and psychotherapy, with a focus when appropriate on harm reduction therapeutic interventions and support services for clients in need of, or currently receiving, substance abuse therapy and treatment.

Additional duties include developing and implementing activities related to substance abuse prevention, intervention, and rehabilitation, as well as performing other related work. The position also involves providing social work services to non-therapy clients in support of other on-staff social work case managers.

The Position
The position will be embedded at the Public Health, Policy and Planning Edison Clinic at the San Mateo Medical Center, and possibly one morning per week at the Fair Oaks Health Center, with clinic hours Monday through Friday. 

This position will engage individuals with a range of mental health and substance use disorders to provide a range of services and supports, including but not limited to, diagnosis of issues to be focus of clinical attention, including substance use and other behavioral health needs; screening/assessing clients' needs using assessment criteria to determine medical necessity and identification of key clinical interventions; determination of stage of change; and engaging clients by using Motivational Interviewing and Harm Reduction practices.

The provider will provide therapy for primary case load, and as a backup also provide case management services, including transportation, client advocacy, client-driven treatment planning, and care coordination with other care providers; navigation of medical and mental health services, including medication assisted treatments; assisting clients with enrolling into benefits, including health coverage for which they are eligible; participate in a multidisciplinary team; use clinical consultation care coordination meetings; and partner with key referral sources and other service providers including but not limited to, San Mateo Medical Center (SMMC) Emergency Department (ED), Psychiatric Emergency Services (PES), and Primary Care and Specialty Clinics, BHRS clinics and programs, Substance Use Disorder treatment providers, including Palm Avenue Detox, shelters, Probation, and Correctional Health.

The Ideal Candidate
The ideal candidate will have significant knowledge, skills, experience, and training in providing therapy as well as case management; assessing and evaluating adult clients with mental health and substance use issues; determining appropriate referrals and recommending treatment plans; documenting in electronic medical records; establishing appropriate clinical relationships; and demonstrating strong understanding of confidentiality and ethics.

Three years of experience in clinical social work, psychotherapy, or a related field, particularly with diverse populations affected by mental health and substance use issues is highly desirable.
