The City of Redwood City Parks, Recreation, and Community Services Department invites you to apply for the casual position of Human Services Assistant at the Fair Oaks Community Center (FOCC). THE POSITION

Under general supervision of a Police Sergeant, the Reserve Police Officer is a sworn position assigned to field operations to patrol for the prevention and detection of crime and enforcement of laws and regulations within the philosophy of community oriented policing.  Incumbents are normally expected to solve most work problems independently and to refer to supervisory personnel only those matters which involve policy decisions, technical questions and unusual problems at the entry level and make recommendations at the advanced level.

The Bilingual Spanish Psychiatric Social Worker / Marriage and Family Therapists will provide specialized psychiatric assessment, brief treatment, and clinical case management services for adult clients with Severe Mental Illness. Principal duties will include mental health and substance use assessment and treatment; engaging clients to services; consultation with providers; and linkage/referrals to other community providers. The main role of this position will be working within a dynamic multi-disciplinary team to provide specialized psychiatric casework and care coordination, provide individual and group psychotherapy, case management, and refer clients to Private Provider Network and/or community agencies for additional services.

 

The primary duties will include care coordination for a clinical case load, mental health assessments; treatment planning, case management; crisis management; individual psychotherapy; and collaborating with multidisciplinary team which includes psychiatrist, registered nurses, PSW SW and LMFT, and Case managers. Clinician will also perform “Officer of the Day” duties.

 

Collaboration and coordination of services with other providers is an essential piece of the services to be provided, therefore candidate should have excellent oral and written communication skills. The clinician may consult and collaborate with other teams in BHRS and community agencies and contract providers.

San Mateo County Health's Public Health, Policy and Planning Division has exciting field-based opportunities for well-qualified and motivated Nurse Practitioners to provide both primary and urgent care to adults from vulnerable populations within our community—people experiencing homelessness and/or in shelters, monolingual Spanish speakers and other new immigrants, and those without insurance or underinsured. These services are provided through various Mobile Clinic modalities and venues, including but not limited to street medicine, shelter medicine, and the mobile coach. We are looking for collaborative Nurse Practitioners to work in and lead various multidisciplinary service teams in the field to provide comprehensive, low-threshold, client-centered care and engagement.

Teams are staffed by a Nurse Practitioner (NP), Psychiatrist, Medical Services Assistant, AOD Case Manager, and Patient Services Assistants. These teams work closely with our community partners from the Human Services Agency and community-based organizations who locate and identify clients needing medical and/or mental health services, as well as provide additional health, housing, and social services in the field.

Under general supervision, Nurse Practitioners provide urgent care and primary health care services such as physical examinations, chronic disease management (e.g., diabetes and hypertension), and referrals/linkages to specialty clinics as appropriate.

Currently, Public Health Clinics has one full-time, regular, 40-hour-per-week vacancy for a bilingual Spanish Nurse Practitioner and two 40-hour-per-week, Limited-Term Nurse Practitioner vacancies, requiring flexible schedules and field-based work at different locations. Sixty-five percent of the NP’s time will be in the field, with the remainder spent in the office documenting notes, placing orders, and coordinating with other staff as needed.

The ideal candidate has experience:

• In family practice addressing chronic disease management

• In the diagnosis and treatment of sexually transmitted infections

• Collaborating with interdisciplinary teams

• With the underserved population, including those who are homeless

• In a flexible, dynamic work environment

• Using Spanish in a medical context

Examples of Duties

Duties may include, but are not limited to, the following:

• Provide low-threshold, comprehensive care for San Mateo County’s most marginalized and vulnerable populations, including people experiencing homelessness, those in shelters, migrant and immigrant farmworkers, the recently incarcerated, the uninsured, and those with financial challenges.

• Provide comprehensive and urgent care in a variety of non-traditional settings, including homeless encampments, outdoor locations, shelters, and a mobile RV clinic.

• Demonstrate the ability to work closely and collaboratively with a small team in often challenging environments (e.g., homeless encampments, shelters, extreme weather, individuals under the influence, or those with mental health challenges).

• Take stewardship of medical care for assigned geographic populations and/or shelters.

• Outreach to and build long-term, trust-based relationships with vulnerable populations.

• Assess patients’ physical, emotional, and social status through medical history, physical examinations, and diagnostic screenings; evaluate clinical findings and lab results.

• Identify and manage common illnesses and other health problems.

• Refer urgent issues beyond scope to appropriate specialists.

• Deliver longitudinal care in assigned areas.

• Collaborate with other health professionals following protocols for diagnosis and treatment.

• Build relationships with community partners to improve health outcomes and expand services.

• Counsel families regarding health maintenance and care.

• Coordinate referrals to other health or social agencies.

• Maintain accurate records of assessments, follow-ups, and evaluations.

• Supervise subordinate health workers and participate in meetings.

• Actively participate in quality improvement efforts.

• Perform related duties as assigned.

Qualifications

Education and Experience
Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying.

• Education: Completion of an advanced nursing degree from a program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC), with emphasis in a Nurse Practitioner specialty area.

• Experience: Minimum of one (1) year of paid experience in an acute and/or ambulatory care setting.

Position Summary

Under general direction of the Secretary of the District, performs complex and highly analytical work for the Secretary of the District, members of the Board of Directors and associated committees.   Departmental lead on government tort claims, legal documents and communications and participating in complex, sensitive, and detailed analytical work in the areas of budgets, historical preservation and economic, regulatory and administrative policy. Provides internal customer service by assisting managers and executive level staff in complex analysis.  This position requires a high degree of accuracy, discretion, confidentiality, excellent communication skills, and robust customer service skills. 
 

Essential Responsibilities

 

• Serves as the primary analytical support to the District Secretary;
• Interprets meeting proceedings and prepares draft meeting minutes, with minimal oversight and as assigned, may attend meetings in the absence of the District Secretary;
• Performs legal functions of the office and guidance to office staff in the absence of the District Secretary;
• Performs a wide variety of complex legal analysis, research and briefings about claims for the District, Bridge, Bus and Ferry Divisions, and works with legal counsel in the areas such as government tort claims, subpoenas, Brady requests, small claims and lawsuits;
• Provides a wide variety of complex, confidential analytical duties in support of the District Secretary, Board of Directors, and standing Board committees;
• Independently composes, researches, compiles, summarizes, and prepares a variety of complex correspondence, reports and documents; reviews finished materials for completeness and accuracy; ensures documents comply with District standards, policies and procedures;
• Perform special studies for the District, and coordinates assigned activities with other District departments, and divisions, and outside entities;
• Independently responds to and resolves difficult and sensitive citizen inquiries and complaints;
• Coordinates and assists in the development of the annual budgets of the Board and District Secretary’s Office and monitors, reviews and approves department expenditures, and performs financial analysis for budgetary purposes;
• Assists the District Secretary with providing direct support in varied, analytical administrative projects related to the Board and its committees;
• Independently prepares and presents a variety of oral and written reports, correspondence, and statistical analyses with extreme attention to detail and accuracy;
• Works with Procurement to select vendors and purchase department services as needed for projects, and participates on panels for various District procurements especially those related to the District’s third party administrators for public liability and workers compensation, insurance advisors and e-document conversion;
• Has an in-depth understanding of the department’s functions and especially its role with the Board of Directors such as the steps involved in planning and executing Board events and maintaining key Board documents such as Rules of the Board, Master Ordinance and record of the Board’s proceedings;
• Autonomously draft client correspondence and legal documents, using proper format and appropriate language and content;
• Oversees newspaper advertisements for public hearings/meetings;
• Monitors activities and ensures maintenance of files related to insurance and accident claim matters to fulfill department records retention schedule;
• Engages with a wide variety of District representatives including the Board of Directors, Executive Team, mid-managers and staff in a wide variety of roles including those engaged in specialized trades;
