This position is located in the Public Health Division of the Health and Human Services Agency. The Program Manager position has managerial and supervisory responsibility for County programs in the Women, Infants, and Children (WIC) Division. Program Managers are held accountable for the success of the programs and may act on behalf of the Public Health Division. This position exercises direct and in-direct supervision over professional, technical, and clerical staff. This position is full-time, and is scheduled for forty (40) hours per week.

JOB DUTIES

• Evaluate operations and activities of department and/or program and recommend improvements and modifications.

San Mateo County Health's Public Health, Policy and Planning Division has exciting field-based opportunities for well-qualified and motivated Nurse Practitioners to provide both primary and urgent care to adults from vulnerable populations within our community—people experiencing homelessness and/or in shelters, monolingual Spanish speakers and other new immigrants, and those without insurance or underinsured. These services are provided through various Mobile Clinic modalities and venues, including but not limited to street medicine, shelter medicine, and the mobile coach. We are looking for collaborative Nurse Practitioners to work in and lead various multidisciplinary service teams in the field to provide comprehensive, low-threshold, client-centered care and engagement.

Teams are staffed by a Nurse Practitioner (NP), Psychiatrist, Medical Services Assistant, AOD Case Manager, and Patient Services Assistants. These teams work closely with our community partners from the Human Services Agency and community-based organizations who locate and identify clients needing medical and/or mental health services, as well as provide additional health, housing, and social services in the field.

Under general supervision, Nurse Practitioners provide urgent care and primary health care services such as physical examinations, chronic disease management (e.g., diabetes and hypertension), and referrals/linkages to specialty clinics as appropriate.

Currently, Public Health Clinics has one full-time, regular, 40-hour-per-week vacancy for a bilingual Spanish Nurse Practitioner and two 40-hour-per-week, Limited-Term Nurse Practitioner vacancies, requiring flexible schedules and field-based work at different locations. Sixty-five percent of the NP’s time will be in the field, with the remainder spent in the office documenting notes, placing orders, and coordinating with other staff as needed.

• Provide low-threshold, comprehensive care for San Mateo County’s most marginalized and vulnerable populations, including people experiencing homelessness, those in shelters, migrant and immigrant farmworkers, the recently incarcerated, the uninsured, and those with financial challenges.

• Provide comprehensive and urgent care in a variety of non-traditional settings, including homeless encampments, outdoor locations, shelters, and a mobile RV clinic.

Qualifications

Education and Experience

• Education: Completion of an advanced nursing degree from a program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC), with emphasis in a Nurse Practitioner specialty area.

San Mateo County Health- San Mateo Medical Center seeks well-qualified candidates for the position of Program Coordinator I in the Patient Access Division. The current vacancy is a full-time, limited term vacancy. The duration of the assignment will last about 6 months.

 

Under the direction of the Patient Access Manager, the Program Coordinator will provide support to over 150 Patient Services Assistants across various departments, including Emergency, Admitting, Long-Term Care, Same-Day Surgery, and will average 27 Federally Qualified Health Center (FQHC) sites with 8-16 hours of weekly program refresher training on several publicly sponsored coverage options.

San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the unclassified position of In-Home Supportive Services Social Worker II/III. Social Workers provide a wide variety of welfare services in such areas as information and referral, protective services, out-of-home services, health related services, chore services, and other welfare services within the Health and the Human Services Agency.

The current unclassified vacancy is in the In-Home Supportive Services (IHSS) Program that provides in-home care for persons who cannot safely remain in their own home without such assistance. The recipients of IHSS are aged, blind and/or disabled and eligible for Medi-Cal. Disabled and/or blind minor children may also be eligible for IHSS. The Social Worker will make a home visit to determine eligibility for the program and to conduct an assessment to determine a range of service hours for particular tasks. The IHSS recipient or guardian is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive. The Social Worker will travel throughout the County to conduct home visits, attend meetings and perform other duties of the position. The Social Worker will work in the San Mateo, CA Aging and Adult Services Office.

This IHSS Social Worker will conduct intake and/or on-going activities for IHSS recipients. Some of those activities include:

• Conduct initial IHSS assessments using a detailed State assessment tool.
• Complete necessary documents, paperwork, and work within different information technology systems.
• Conduct annual reassessments.
• Educate, support, and perform administrative duties for IHSS recipients and care providers to understand/comply with program regulations.

Social Workers provide highly skilled, intensive casework services to individuals and their families and provide casework consultation within the department and to outside agencies. Incumbents of positions at this level work with minimal supervision and are expected to have acquired the high degree of assessment skills required to perform care coordination and referral tasks in complex casework areas. Lead supervisory responsibilities may be assigned as circumstances dictate.

San Mateo County Health is seeking an experienced individual for the position of Public Health Clinics Process Improvement Manager that plans, oversees, and provides a wide variety of administrative, analytical, and work-flow support on a County-wide level; analyzes practices and procedures and makes recommendations for organizational, operational, policy, and procedural improvements; conducts needs analysis, feasibility studies, and program evaluation for assigned projects and programs; develops, summarizes, and maintains administrative and fiscal records; fosters cooperative working relationships among County departments and acts as liaison with various community, public, and regulatory agencies; and performs related work as required.

The County of San Mateo is actively recruiting highly qualified candidates for the role of Forensic Mental Health Psychiatric Social Worker I/II - Marriage & Family Therapist I/II. We currently have four vacancies at the Maguire and Maple Street Correctional Facilities. Fluency in Spanish is preferred.

Correctional Health Services (CHS) therapists provide clinical mental health services utilizing Trauma Informed Care to inmates with mental illness, serious mental illness, or substance use disorders and/or co-occurring disorders of mental illness, serious mental illness, and chemical dependency. They also provide case management services and discharge planning by interviewing inmate patients to assess mental health needs and formulate treatment plans. They function as a team member in a multi-disciplinary work setting where they collaborate with Sheriff's Office staff to manage inmate patients' daily activities. Positions require some weekend hours.

San Mateo County Health is seeking an experienced individual for the position of Public Health Clinics Strategic Projects Manager. This management position will report to the Public Health Clinics Manager, overseeing Edison Clinics and Mobile Clinics and will have broad project management responsibility for key strategic efforts affecting the Public Health clinics.
 
There is currently one regular vacancy in the Public Health, Policy and Planning Division located in San Mateo, CA.
 
Primary responsibilities will include, but are not limited to, the following:  

• Quality assurance and improvement:  build and visualize data in management reports and dashboards; convert data to information and use it to guide process improvement.
• Process improvement:  facilitate, document, track, organize, measure progress; e.g., conduct improvement work within daily huddles.
• Project management:  lead projects and provide ad hoc support of projects.
• Knowledge management:  inventory and maintain infrastructure of core programmatic documents and processes, how files are stored and shared.
• Partnership building:  support internal staff operational needs; coordinate with other stakeholders, such as staff from Behavioral Health & Recovery Services, Human Services Agency, Health IT, San Mateo Medical Center, grant makers, state agencies, etc.

• Contract and MOU support:  assist Clinics Manager to write scopes of work, produce required reports and invoices for categorical funding and operational agreements.
• Revenue optimization support:  oversee billing quality assurance and improvement, facilitate braided funding opportunities and oversight.
• Policy and procedures facilitation:  maintain inventory and lead update processes through completion, including occasional initial literature reviews or research and coordination of stakeholders.
• Change management and leadership synchronization:  coordinate staff engagement, training, feedback to ensure successful system changes, take meeting minutes, contribute to agendas, track on-going project work in support of meeting goals.

Ideal candidate will possess:

The Public Authority for In-Home Supportive Services (IHSS) is seeking well-qualified individuals for the position of Public Authority Registry Social Worker II/III with fluency in Mandarin or Cantonese. There is one vacancy in the Public Authority. The current vacancy would be responsible for supporting IHSS consumers who do not have caregiver resources, in hiring a homecare provider from the Public Authority Registry and helping the client and provider establish a successful working relationship. The position is located in San Mateo, CA, but will require travel throughout the County to perform duties of the positions.

NOTE: The Public Authority Registry Social Worker II/III is not a County position; the Public Authority unit reports to the San Mateo County Director of Aging and Adult Services.

PUBLIC AUTHORITY

The Public Authority serves as the employer of record for IHSS homecare workers, with the IHSS consumer retaining the right to hire, fire and train their worker. The Public Authority's responsibilities include operating a Registry that recruits, screens and refers potential homecare providers to IHSS consumers needing personal assistance in their home; screening the backgrounds of IHSS providers in accordance with IHSS policies and regulations; providing training opportunities to IHSS providers to enhance their skills in serving IHSS consumers; and offering support and assistance to Registry consumers and providers to ensure the employer-employee relationship between the Registry consumer and provider are functioning well. The IHSS consumer is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive. Consumers who do not have caregiver resources, may choose to hire a caregiver from the Public Authority Registry. The Public Authority Social Worker will assist those recipients with the hiring of a Public Authority Registry worker and help them to manage their role as the employer.

Examples of duties may include, but are not limited to, the following:

• Conduct home visits to support consumers' hiring needs.
• Educate, support, and perform administrative duties for IHSS consumers and care providers to understand/comply with program regulations.
• Provide conflict mediation between consumers and providers to help stabilize cases.

• Provide education, outreach, and community resources information for consumers.

OASIS is community field-based mental health services provided to older adults at their place of residence. This home visit services include medication evaluation and monitoring, individual counseling/therapy, and case management. This multiple discipline team with language capacity in English, Spanish, and Chinese, consists of Psychiatrists, Licensed Clinical Social Workers, Licensed Marriage & Family Therapists, a Community Mental Health Nurse, and a Peer Support Worker. Services are available to San Mateo County residents age 60 and over with issues of mobility and mental health conditions that impact their day-to-day functioning.  

Clinicians will use evidence-based approach to perform assessment, diagnosis, make medical necessity treatment plans, case management, individual therapy, and work with family members of clients' and other community providers for clients' mental health follow-up care needs. These interventions are provided in field setting or in clients' place of residence, and/or as needed to clients who are living in the community.

Clinicians are committed to providing medical necessity treatment that supports the clients' goals of independence and self-sufficiency with other service providers' support to continually live in the community with better quality of life.
