San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the position of In-Home Supportive Services Social Worker II/III. Social Workers provide a wide variety of welfare services in such areas as information and referral, protective services, out-of-home services, health related services, chore services, and other welfare services within the Health and the Human Services Agency.
 
The current vacancy is in the In-Home Supportive Services (IHSS) Program that provides in-home care for persons who cannot safely remain in their own home without such assistance. The recipients of IHSS are aged, blind and/or disabled and eligible for Medi-Cal. Disabled and/or blind minor children may also be eligible for IHSS. The Social Worker will make a home visit to determine eligibility for the program and to conduct an assessment to determine a range of service hours for particular tasks. The IHSS recipient or guardian is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive.  The Social Worker will travel throughout the County to conduct home visits, attend meetings and perform other duties of the position.  The Social Worker will work in the San Mateo, CA Aging and Adult Services Office.
 
This IHSS Social Worker will conduct intake and/or on-going activities for IHSS recipients. Some of those activities include:
 

• Conduct initial IHSS assessments using a detailed State assessment tool.
• Complete necessary documents, paperwork, and work within different information technology systems.
• Conduct annual reassessments.
• Educate, support, and perform administrative duties for IHSS recipients and care providers to understand/comply with program regulations.
• Provide education, outreach, and community resources information.
• Participate in quality improvement efforts.
• Collaborate with quality assurance and program integrity to ensure proper adherence to regulations.
• Prepare for and attend Fair Hearings.
• Engage in interdisciplinary team meetings or other activities to support San Mateo County's vision for integrated services.
• Maintain oversight of case management needs for a caseload focused on serving IHSS recipients who are minors.
• Collaborate and facilitate service with care providers and other partner agencies.

 The ideal candidate will possess the following:
 

• Master's degree in Social Work.
• Experience providing clinical assessment for children, youth and adults with disabilities. 
• Understanding of human development and behavior within diverse populations.
• Ability to independently develop and implement sound case plans of action.
• Experience balancing client-centered care coordination and the accompanying administrative documentation.
• Familiarity in implementing program regulations.
• Superior organizational skills to manage a large caseload and related documents.
• Proficient computer skills to use several independent data base systems as well as Microsoft Excel and Word.
• Ability to quickly learn and adopt new technology, policies and procedures.
• Excellent writing/documentation skills.
• Fluency in Cantonese, Mandarin, Tagalog, Vietnamese or Farsi is a plus.

SALARY
Social Worker II:   $6,564.13 - $8,205.60/month

Social Workers provide highly skilled, intensive casework services to individuals and their families and provide casework consultation within the department and to outside agencies.  Incumbents of positions at this level work with minimal supervision and are expected to have acquired the high degree of assessment skills required to perform care coordination and referral tasks in complex casework areas. Lead supervisory responsibilities may be assigned as circumstances dictate.

NOTE:  The list created from this recruitment may be used to fill future vacancies, including classified, unclassified, term, full-time, part-time, temporary and extra help vacancies. Qualifications

Licensure/Certification:  California class C driver license or equivalent.

Education:
Social Worker II: Satisfactory completion of 30 semester units of social welfare and/or appropriate behavioral science courses.

Social Worker III: Satisfactory completion of 36 semester units of social welfare and/or appropriate behavioral science courses.

Experience: 
Social Worker II: Two years experience performing duties comparable to those of a Social Worker I or higher in San Mateo County. Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work.

Social Worker III:  Three years experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.  Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

THE POSITION:

The City of San Rafael Library & Recreation Department, Child Care Division, is accepting applications for Child Care Instructor II to work in our School-Age Programs at Mary Silveira and Vallecito Children’s Centers.  Under the supervision of the Child Care Director, the Child Care Instructor II will provide developmental care and supervision to children enrolled in the childcare program; implement age-appropriate curriculum for children; establish and maintain supportive relationships with children and parents.  Incumbents in this position assume primary responsibility for planning, scheduling, and organizing curriculum and other activities for childcare participants.  In the absence of the Child Care Director, the Child Care Instructor II may assume those duties.

MINIMUM QUALIFICATIONS:
Education: Equivalent to a bachelor’s degree from an accredited four-year college or university in a field directly related to the area to which assigned.
For the Finance Systems Project Manager position the following are applicable fields of study: business administration, public administration, accounting, finance, management information systems, or a related field.

Experience: Eight (8) years of experience in the professional field required by the assigned special projects. The Finance Systems Project Manager position requires accounting support experience utilizing and supporting financial systems and/or two (2) years that involved responsibilities at a level equivalent to the District's classification of Project Manager II. Years of experience may be substituted for the educational requirement on a two for one basis (two years experience may be submitted for one year of education).

ADDITIONAL INFORMATION:
Physical Requirements: Must maintain the physical condition necessary to perform tasks in an office setting operating a computer, keyboard, and other peripheral equipment; safely drive a District automobile; and walk, stand, and climb short distances in construction settings in order to inspect the progress of assigned projects.

AC Transit is seeking a Project Manager I (Utility Location and Marking) in the Capital Projects department. Under general direction, serves as project manager for routine to moderately complex projects and assists higher-level project managers on more complex projects as needed. This is the experienced level within the Project Manager series. There are four (4) levels within the series, which are distinguished from one another by the technical complexity of the assigned duties and the level of discretion and independent judgment exercised. Positions at this level are responsible for performing project management activities for routine to moderately complex projects.

Positions in the Project Manager series are flexibly staffed to meet the needs of the District. Education: Equivalent to a Bachelor’s degree from an accredited four-year college or university in a field directly related to the area to which assigned. For Engineering/Capital Projects: Equivalent to a Bachelor’s degree in architecture, engineering, planning, or a related field.

The Position

Under the direction of the Youth and Family Services Administrator, the Senior Program Coordinator plays a unique role overseeing the early childhood mental health program, providing direct counseling services and supervising several staff counselors and interns.

The Senior Program Coordinator typically has specialized experience, training and qualifications that enable the position to work effectively with children 0-8 years, parents and youth caregivers.

The Senior Program Coordinator works closely with the division’s Clinical Supervisors and Quality Assurance Specialist to ensure that clinical services comply with all policies, procedures and legal regulations, and that client services are delivered at the highest standards. The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

As a member of the Human Services Department, this position reports to a Clinical Supervisor, under direction of the division manager of Homeless Care and Response (HCR), and is characterized by the responsibility to provide comprehensive psycho- social assessments and develop and implement care plans. These plans may focus on connecting clients  to public benefits, obtaining budgeting skills, accessing medical care, and obtaining more stable living arrangements. The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits in the company of another staff member in the field to locations where homeless persons may reside,  to medical facilities, and other community locations.
